Mechanisms of Adventitious Lung Sounds Crackles Mechanism: 1. Bubbling of air through airway secretion 2. Crackle 不 連 續 音, 常 出 現 在 吸 氣 期, 類 似 在 耳 鬢 搓 揉 頭 髮 的 嗶 嗶 啵 啵 聲 ( 捻 髮 音 ) 聽 見 crackle 時, 令 病 人 咳 嗽 將 痰 清 出 後, 再 聽 有 時 crackle 就 會 自 然 不 見 了 成 因 : 由 於 氣 道 內 異 常 積 水, 吐 氣 時 肺 泡 會 塌 陷, 但 吸 氣 時 又 會 再 被 撐 開 當 許 多 肺 泡 不 同 時 被 撐 開, 就 會 發 出 crackle 聲 可 見 於 心 衰 竭 肺 積 水 肺 炎 肺 纖 維 化 Rhonchus 連 續 低 音, 音 頻 <200Hz, 近 似 打 鼾 或 呻 吟 聲, 常 出 現 在 吐 氣 時 由 於 氣 流 通 過 變 窄 後 的 較 大 支 氣 管 所 形 成 可 見 於 氣 喘 病 支 氣 管 炎 等 病 20

Voice Sounds A normal air-filled lung transmits low frequency of voice sounds (<200Hz). Higher frequency of voice sounds are selectively filtered and attenuated. Alteration in lung pathology will change the transmission of voice sound, resulting in either increased or decreased transmission of voice resonance. 聽 診 時, 病 人 正 在 說 話 所 聽 到 的 異 常 聲 音 有 以 下 三 種 : Egophony Bronchophony Whispered pectoriloquy Voice Sounds Bronchophony Louder and clearer voice sounds Occur over areas of lung consolidation 正 常 聽 診 時 若 病 人 說 話, 從 聽 診 器 聽 到 的 聲 音 較 小 聲, 也 較 不 清 晰 但 是 若 聽 診 的 部 位 有 實 質 化 ( 如 肺 炎 肺 塌 陷 肺 癌 等 ) 說 話 的 聲 音 將 反 常 地 大 聲 且 清 晰, 此 謂 之 Bronchophony 22
Egophony Voice Sounds Voice sound increases in intensity and takes on a nasal quality e-e-e heard as a-a-a Often combines with bronchophony Occur over areas of lung consolidation 若 聽 診 的 部 位 有 實 質 化 (consolidation)( 如 肺 炎 肺 塌 陷 肺 癌 等 ) 令 病 人 發 出 伊 的 聲 音, 從 聽 診 器 聽 起 來 會 變 得 像 在 唸 A 的 聲 音, 此 謂 之 egophony Voice Sounds Whispered Pectoriloquy Whispering is normally heard as muffled, lowpitched sounds over chest wall by stethoscope.
