1 齒列矯正時牙根吸收的現象 發生機制與風險因素之考量 ( 下 ) The Consideration of the Phenomenon, Mechanism and Risk Factors of Root Resorption in Orthodontic Treatment 文>楊全斌 (4) 哪種施力比較不好 持續性或間歇性的力量? 矯正力量的衰減模式可分為持續力 間斷力與間歇力三種 根據文獻 1, 雖然目前還不清楚其中哪一種力量對於牙根傷害最少, 不過有一個研究相當有趣, Levander 等人針對動態治療開始 6 個月後發生牙根吸收的 40 個案例, 分為繼續治療與中斷 2-3 個月後才再繼續治療的兩部分, 並比較其最終牙根吸收量 結果發現中斷治療的部分, 其最終牙根吸收量明顯較少, 這個研究給了我們兩個臨床提示, 一個是包含沒有作用力期間的間歇或斷續的矯正可能比較不會發生牙根吸收, 另一個是發現牙根吸收後立即中斷治療, 可能可以抑制牙根吸收的進行, 這一點值得後續研究 Ellis 30 (2002) 認為固定裝置幾乎無法避免牙根變短 (root shortening is almost inevitable in patients with fixed appliances) 取自 :Acar A. 31 根據文獻 32,Maltha et al.(2004) 的報告中, 檢測持續力 (continuous force) 以及間歇力 (intermittent) 在顯微鏡下的差異 使用小獵犬在下顎使用 coil spring 近心移動小臼齒, 持續力分別為 : g; 間歇力為 10 以及 25g( 每天 16 小時 ) 研究結果指出, 重的力量可能不是最重要, 最重要的可能是持續力會引起比較嚴重的牙根吸收 (force magnitude may not be the most decisive etiologic factor responsible for root resorption, and that the severity of this condition is highly related to the regimen of force application. In this regard, intermittent forces cause less severe root resorption than continuously applied forces. 21 ) 為什麼間歇力比較不會引起牙根吸收呢? 因為間歇力讓組織有時間給 cementoblasts 產生 cellular cementum, 保護並修護 acellular cementum 的吸收 因此治療中如發現牙根吸收, 應停止治療約 6 個月, 讓組織有時間去修護牙根表面所產生的吸收, 之後再重新開始治療 (5) 牙根吸收是否與治療方法有關? 根據文獻 1, 比較 Standard edgewise techniques Begg techniques Self-ligation brackets 與 Straight archwire appliances, 結論是門牙區的牙根吸收量沒有差別, 比較 Edgewise technique 的 frictional mechanics(continuous arch 的 sliding mechanics) 與 non-frictional mechanics(sectional arch 的 loop mechanics) 之牙齒移動, 結果是無差異 但 McNab 等人則認為 Begg technique 比 Standard Edgewise technique 造成的大臼齒牙根吸收量較大 Brezniak N.;Wasserstein A. 33 (2008) 報告一個 卷 5 期

2 Invisalign 造成上顎門牙牙根吸收 2mm ~ 1/3 的個案, 其結論是該裝置與其他裝置都同樣會造成牙根吸收 the OIIRR phenomena can unpredictably appear with this treatment modality( 即 Invisalign), just as it does with all other orthodontic treatment modality 關於使用裝置, 一般認為 Cl II elastics 34 方線 34,35 與 head gear 都是危險因子, 但也有否定的看法, 因此和牙根吸收的因果關係還未確定 Sameshima 36 (2001) 認為各種 slot 尺寸 archwire 型別 elastics 的使用與否 擴大裝置的種類並無差異 (No differences were found for slot size, archwire type, use of elastics, and types of expansion) 關於 bracket 種類,Nigul 35 (2006) 認為樹脂型 brackets with a metal slot 的風險較高 (Cases of treatment with composite brackets with a metal slot had significantly more root resorption than cases treated with metal brackets. The reason for that might be the longer treatment time because of higher debonds and failure rate.) (6) 牙根吸收的原因可能是先天遺傳因素造成的嗎? 根據文獻 1, 實務上的經驗發現, 有些案例容易牙根吸收但有些則不會, 這表示牙根吸收的感受性是有個體差異 根據雙生兒及手足的流行病學研究, 遺傳因素是牙根吸收的重要因素, 然而哪些遺傳因素與牙根吸收的個體差異有關目前還未明 interleukin-1β 是一種 cytokine( 細胞激素 ), 和發炎 骨吸收有關, 根據最新研究, 分析 interleukin-1β 之遺傳密碼之鹼基多型性與牙根吸收關係的結果, 發現 IL-1β 基因的兩個多型中,1 型的人發生牙根吸收 2mm 以上的危險提高 5.6 倍, 本研究提出一個假說,interleukin-1β 基因 1 型容易造成齒槽骨吸收與牙齒移動減緩, 應力集中在根尖部的結果, 造成牙根容易被吸收 未來牙根吸收的基因因素分析更深入, 或許牙根吸收危險性的預測將變為可能 另外根據基因研究 37, IL-1B 的對偶基因 1(allele 1) 會減少 IL-1 cytokine 的生成, 並提高牙根吸收的風險 ( 圖 ) 取自 :Al-Qawasmi et al 37 取自 :Al-Qawasmi et al 37 Dr.Hartsfield 等人在 2004 年發表的文章中回顧並整理有關牙根吸收的文獻 結論指出 : 引起牙根吸收的可能因素中, 基因遺傳的因子至少佔了 50%, 也就是說, 如果接受矯正的病人其父母或親友在接受矯正後有牙根吸收的現象, 那該病人將來發生牙根吸收的機率也大為增加 Kokich VG 38 的看法是 : root resorption tends to run in families, 如果以前家長做矯正時發生過牙根吸收, 則現在子女做矯正發生牙根吸收的可能性會很高 Krishnan V., Davidovitch Z. 21 (2009) 認為在近未來可能會開始用基因檢查方式, 篩選出容易牙根吸收的患者, 作為擬定治療計畫時的參考 (it is not far fetched to think that in the near future there will be a genetic test that will determine rootresorption-associated polymorphisms in patients prior to orthodontic treatment. ) Harris, Kineret, and Tolley 39 (1997) 認為感受性 卷 5 期 41

3 無性別年齡差異, 牙根吸收感受性取決於個人的基因型, 有些人容易有些人不容易 相同血緣的手足感受性類似 (No evidence was found for a sex or age difference in susceptibility. The persons's genotype modulates his or her susceptibility to EARR; some people appear to be intrinsically endowed with resistance to apical resorption under the stress of mechanotherapy and some, at the other extreme, are prone to experiencing severe resorption under the same regime. Siblings experience similar levels of EARR in response to orthodontic treatment. A prior sibling's outcome should be a useful gauge of another sibling's risk of EARR. ) 牙根吸收的能力較強 (teeth with incomplete root formation have a higher resistance to root resorption than those with complete roots) (8) 牙根吸收是否與性別有關? 牙根吸收是否有性別差異, 目前尚無統一見解 有的認為女性較多, 有的認為牙齒有牙髓者則男性居多, 有的認為無性別差異 根據 Baumurind 等人的報告, 牙根吸收量男性為 2.29mm, 女性為 1.09mm, 兩者相差 1.20mm Spurrier 等人認為有牙髓的牙齒, 男性的吸收量有統計學上的意義, 無牙髓的牙齒則無性別差異 Sameshima 認為無性別差異 40 (2001): no difference between male and female. 另外根據以日本人為對象的研究, 大部分研究都否認性別差異 (9) 牙根吸收是否與牙齒種類有關? 根據文獻 1, 關於牙根吸收的牙齒種類差異, 目前已知上顎門牙的牙根吸收機率較高吸收量也大 上顎門牙容易發生牙根吸收的原因, 一般認為是美觀與功能因素造成牙齒負擔過大, 細圓錐型的牙根型態容易造成應力集中 至於正中門牙與側門牙哪一個比較容易發生牙根吸收, 則意見紛紜 取自 :Harris, Kineret, and Tolley 39 (1997) (7) 牙根吸收是否與年齡有關? 根據文獻 1, 關於治療開始年齡與牙根吸收的關係, 有些認為年齡越大越容易造成牙根吸收, 有些則認為兩者無關, 目前尚無結論 在牙根完成期, 包含根尖周圍血流的組織之局部環境, 可能變化較大, 因此要正確把握治療開始年齡與牙根吸收的關係, 就必須以牙根發育階段為基準, 比較牙根完成前後的不同, 另外檢討牙根完成後的時間變化 Linge and Linge 12 認為 11.5 歲以前的小孩較不會發生牙根吸收 (less resorption in patients treated before the age of 11.5, after 11.5 years, most of the upper incisor roots are fully developed.) Mavragani(2002) 則認為牙根未完成的牙齒其抗 牙根吸收在矯正治療前可能已存在,Lupi 等人報告, 矯正治療前四顆前牙的牙根吸收發生率, 包含輕微者大約為 15%, 矯正治療 12 個月後的發生率則增加到 73% 關於牙齒的矯正移動治療造成的牙根吸收機率, 根據報告, 包含輕微者, 上顎正中門牙大約為 52.7% ~ 97.3% 牙根吸收量( 牙根長度的減少量 ) 平均值, 最小為 1.36mm, 最大為 2.93mm, 差異相當大 Kaley and Phillips 10 (1991) 認為 :90% 以上的上顎正中門牙發生牙根吸收, 從 X 光片上可明顯看出, 嚴重性側門牙也差不多 (over 90% of the maxillary central incisor roots had resorption apparent on careful examination of panoramic radiographs, and the figure was nearly as high for maxillary laterals.) Segal 的 meta analysis, 上顎門牙的牙根吸收量之平均值為 1.421±0.448mm Krishnan(2005) 認為風險最高的是上顎正中門 卷 5 期

4 牙, 其次是上顎大臼齒與犬齒 (a number of studies have revealed that the maxillary central incisors are the most prone to the process, followed by the maxillary molars and canines) Apajalahti S 41 (2007) 則認為頻率最高的是上顎門牙, 其次是下顎門牙 (maxillary incisors showed apical root resorption most frequently, followed by the mandibular incisors) Kook 42 (2003) 認為錐形 小的側門牙發生牙根吸收的風險不會更高 (peg-shaped and small lateral incisors not at higher risk for root resorption.) (10) 牙根吸收與牙根型態的異常是否有關? 根據文獻 1, 有些研究認為無相關, 但一般認為牙根型態的異常是牙根吸收的風險因素之一 牙根型態異常包含滴管型的牙根 彎曲牙根 治療前已發生牙根吸收 牙根尖較圓 牙根短等 就上下顎門牙來看, 大約 45% 的牙齒有牙根型態上的異常 牙根型態異常容易造成牙根吸收的理由是, 應力容易過度集中在牙根尖, 另外牙根型態異常也可能容易伴隨牙骨質 牙本質基質的結構與成分上的異常 Sameshima 40 (2001) 認為 : Dilacerated maxillary lateral incisors and pointed teeth, in general, showed greater root resorption. Blunted teeth had less resorption. Root forms 取自 :Levander and Malmgren 34, short root; 2 blunt root; 3 root with apical bend(dilacerated root); 4 root with apical pipette shape. (11) 牙根吸收與牙髓活性或牙髓治療的有無是否 有關? 矯正開始前向患者或家長解釋治療計畫時, 如果家長問治療中是否牙髓容易壞死或做過根管治療的牙齒, 特別是上顎門牙, 是否容易發生牙根吸收的現象? 你怎麼回答? 關於前者, 理論上, 施加持續性的小矯正力量應不大會影響到牙髓, 雖然矯正治療引起的牙髓反應很小, 牙髓仍可能發生輕度發炎反應, 但通常無長期意義 有報告指出矯正期間牙髓失去活性的案例, 通常此牙有創傷病史, 而未適當控制的矯正力量可能引起牙髓壞死 一但牙齒承受連續性的大力量, 在下蝕性吸收增加骨頭變化後, 牙齒會突然移動, 根尖突然移動大距離可能會切斷根尖口處的血管 往遠心傾斜門牙時, 根尖往反方向移動, 如果傾斜程度大到根尖到齒槽突外面的程度, 亦可觀察到此門牙失去活性 這種移動可能會切斷進到牙髓管內的血管 Bauss 43 (2009) 認為 : 矯正中牙周損傷過大會造成牙髓堵塞, 進而牙髓壞死的風險 (teeth with severe periodontal injury during orthodontic therapy and subsequent total pulp obliteration have an increased risk of pulp necrosis during later orthodontic treatment stages.) 20 有教科書指出矯正中可能會造成牙髓壞死 適當矯正力可使牙髓組織輕度充血, 發生冷熱刺激痛, 甚至牙髓活力下降, 通常可在矯正完成後恢復 若矯正力過大則可導致牙髓炎, 成牙本質細胞層剝離, 部分或全部牙髓變性甚至壞死 如果死髓牙未發生根尖周炎, 它也可被移動 另外 Kokich VG 5 認為矯正治療不會造成牙髓壞死 (From a clinical perspective, I have not encountered a tooth with moderate to severe root resorption whose pulp became nonvital, although no studies have analyzed this relationship.) 取自 :Mirabella and Årtun 6 Criteria for subjective scoring of root form. N, normal; A, blunt; B, eroded; C, pointed; D,bent(deviated); E, bottle shaped. 關於後者, 可從兩方面來看 一是根管治療後自然發生牙根吸收的機率, 一是根管治療後做矯正而發生牙根吸收的機率 卷 5 期 43

5 a. 根管治療後發生牙根吸收的機率 : 根據文獻 1, 自從 Steadman 指出根管治療後的牙齒可能會被視為異物而慢慢發生牙根吸收以來, 有一些學者特別針對根管治療與牙根吸收的相關性作了研究 其中 Wickwire 等人報告 : 根管治療的牙齒與同顎另一側的有髓牙相比, 根管治療後的牙齒發生牙根吸收的機率較高 不過他們的研究中的牙齒大都是因為外傷而根管治療, 而且也無牙根吸收量的比較 其他定量的研究, 結果大多相反 Spurrier 等人的研究 : 有髓牙的牙根吸收量為 2.05mm, 無髓牙的牙根吸收量為 1.28mm, 梶井等人也有類似結果 Reitan 則認為根管治療的牙齒, 其牙根尖的牙骨質 牙本質的鈣化密度與硬度提高, 因此對於牙根吸收的抵抗力較強 b. 根管治療後做矯正而發生牙根吸收的機率 : 有些文獻認為根管治療後的牙齒在矯正治療期間比正常牙齒容易發生牙根吸收, 有些文獻則認為不會, 但是曾經在下壓 ( 內縮 ) 時承受嚴重創傷需要根管治療的牙齒, 可能會發生牙根吸收 如果需以矯正治療調整此牙位置, 以氫氧化鈣充填根管應該不會造成牙根吸收, 最好是移動好牙齒後, 再以永久性根管充填材料取代氫氧化鈣 (12) 牙根吸收的其他危險因素根據文獻 1,Overjet, Overbite 過大 開咬等咬合不正 拔牙治療 下顎結合處的寬度 咬唇癖 舌癖與弄指癖 咬指甲癖 外傷 外傷造成牙齒吸收 外傷性咬合 先天缺牙 嵌入牙 taurodont 牙冠型態異常 牙根與皮質骨的位置關係 齒槽骨的骨密度 埋伏犬齒的的牽引 牙齒異位萌出 過敏 荷爾蒙與營養等全身狀態 上顎骨的切骨手術等雖然被視為牙根吸收的危險因素, 但其中一部分因果關係尚未被確認 關於 Class II (Overjet>7mm),Brin et al. 44 (2003) 報告 : 上顎 11% 的正中門牙與 14% 的側門牙發生中度以上的牙根吸收 [11% of central incisors demonstrated moderate to severe(>2mm) EARR.] Sameshima 40 (2001) 認為 overjet 嚴重者牙根吸收較嚴重 (increased overjet, but not overbite, was significantly associated with greater root resorption) 關於先天缺牙,Levander 34 (1998) 認為先天缺四顆牙以上者風險較高 [there is a high risk of apical root resorption in patients with multiple aplasia.(four or more teeth )] 關於種族因素,Sameshima 40 (2001),Krishna n(2005),krishnan and Davidocitch(2006),Abass and Hartsfield(2007) 認為和白種人 西班牙人相比, 亞洲人比較不會發生牙根吸收 (there seems to be a racial predilection with Asians showing less resorption than whites and Hispanics) (13) 牙根吸收是不是發炎反應的結果? 取自 : 下野正基等 ¹ Brezniak 等 4 (2002) 認為矯正過程中產生的牙根吸收具有發炎反應的所有特徵, 因此應稱為矯正引起的發炎性牙根吸收 (orthodontically induced inflammatory root resorption, OIIRR) 而且矯正可能是牙科治療中唯一利用發炎過程來解決功能與美觀問題的工具 (orthodontics is probably the only dental specialty that uses the inflammatory process as a means of solving functional and aesthetic problems) 如圖所示 : 一些發炎因子如 IL-1 IL-6 TNF-α 等均在牙根吸收組織中發現, 證實它們和牙根吸收有關 IL-1 主要由單核細胞和巨噬細 卷 5 期

6 胞產生, 是最強的骨吸收因子, 能誘導破骨細胞形成, 增強其活性, 促進骨吸收 IL-6 參與活化破骨細胞, 促進骨吸收, 是骨吸收的調節因子之一 Han et al. 認為細胞激素 IL-6 在牙根吸收過程中扮演重要角色 Andeas 等認為 TNF-α 調節系統獨立於骨吸收 RANKL/OPG 調控系統,TNF-α 可溶性受體能有效阻斷破骨細胞的形成, 抑制牙根吸收 Low 等認為 RANKL/OPG 調控系統在矯正引起發炎性牙根吸收的過程中, 參與調控破骨細胞的活性 和牙周病的相關性 : 慢性牙周炎時牙周韌帶細胞產生大量 pro-inflammatory cytokines, 這些 cytokines 會增加牙根吸收的風險, 因此牙齦溝液中的 cytokines 濃度就是一個指標, 例如 Lowney et al(1995); Uematsu et al(1996) 報告 TNF-α 和 IL-6 的濃度在矯正治療患者的牙齦溝液中有明顯增加 (increased levels of cytokines such as TNF-αand IL-6 in the gingival crevicular fluid of orthodontically treated teeth in human.) 無關 (This research provides evidence that conventional orthodontic treatment is not associated with a systemic immune response in the factors investigated. ), 關於這一點, 從牙根吸收觀點, 我認為這個結論容易造成誤解, 因為容易發生牙根吸收的年齡層為青少年 ~ 成人 (12 歲以上 ) 而非兒童 (12 歲以下 ) Prostaglandin E2(PGE2), camp, cgmp, IL-1α,IL-6, TNF-α, RANKL, MMP-1,2,3,8,9,13, CGRP,SP 等 Cytokine: 細胞激素 ( 台 ) 細胞因子 ( 中 ) 由哺乳動物細胞釋放的一大類蛋白質中的任一種, 用作其他細胞的高效化學信使 Interleukin(IL): 白細胞介素在不同類型的白細胞間充當化學信使的一類細胞因子 所有的 IL 皆為蛋白質或糖蛋白 取自 :Krishnan V., Davidovitch Z 21 Prostaglandin(PG): 前列腺素一類脂肪酸衍生物, 最初是從人類前列腺分泌物中分離出來, 但現在已發現存在於所有組織中 發炎時, 其分泌可產生痛感 PDL 中的發炎因子 關於矯正中的發炎反應, 根據 in vivo 的研究,PDL 中的發炎因子有以下各項 :Prostaglandin E2(PGE2), camp,cgmp,il-1α,il-6,tnfα,rankl,mmp-1,2,3,8,9,13, CGRP,SP 等 在最近 45 的論文報告中,MacLaine 等人指出, 對於平均年齡為 13.1 歲的男女生所做的研究, 三種發炎指標 (CRP: C-reactive protein, TNF-α:tumor necrosis factor-α, IL-6:interleukin-6) 都無明顯增加, 因此結論認為傳統矯正治療與 ( 以上三種 ) 系統免疫反應 取自 : 下野正基等 ¹ 關於氣喘病患,McNab et al.(1999) 認為氣喘病患比較容易發生牙根吸收, 可以解釋為全身性慢性發炎容易在矯正治療中引起發炎反應, 進而造成牙根吸收 (systemic forms of chronic inflammation may exacerbate the inflammatory response during orthodontic tooth movement, and thus predispose 卷 5 期 45

7 the teeth to increase root resorption. ) 之後 Ong et al.(2000) 的報告認為以氣喘用藥治療, 可以減少牙根吸收 (as used in treatment and prevention of asthma, prednisolone treatment leads to significantly less root resorption during orthodontic tooth movement.) 9 Jerome 等人認為 Celebrex(COX-2 inhibitors) 可以稍為抵抗牙根吸收 (administration of Celebrex does not interfere with tooth movement and appears to offer some slight protection against root resorption.) Mavragani 等人 46 (2005) 認為低劑量的四環素可以抑制牙根吸收 (systemic administration of low-dose doxycycline in rats may have an inhibitory effect on orthodontically induced resorptive activity.) PGE2 被認為不僅對牙根吸收無效, 反而會促進牙根吸收 46 (prostaglandin E2 had no significant effect on the amount of OIRR, but has been shown to increase root resorption.) 根據動物實驗, 服用 bisphosphonate 可能有助於矯正中牙根吸收的預防 (the topical administration of bisphosphonate may be useful in preventing root resorption of teeth during orthodontic treatment) 47. 關於過敏,Owman-Moll 26 (2000) 認為可能有關 (allergy might play a role) Nishioka et al. 48 (2006) 認為過敏 牙根型態異常與氣喘可能有關 (allergy, root morphology abnormality, and asthma may be high-risk factors for the development of excessive root resorption during orthodontic tooth movement in Japanese patients. ) (14) 牙根吸收的預後 ( 矯正結束後, 牙根會持續吸收嗎?) 根據文獻 1,32, 矯正治療中發生的牙根吸收, 治療後吸收就會停止 Remington 49 (1989) 就 100 名矯正中出現重度牙根吸收案例, 探討其矯正結束後平均 14.1 年的長期變化 結論是牙根吸收後的粗糙面發生平滑化, 尖銳的牙根尖發生圓形化, 牙根表層發生局部 remodeling, 沒有案例顯示牙根吸收會進一步惡化 (the long-term evaluation showed no apparent changes after appliance removal except remodeling of rough and sharp edges) 另外牙根吸收嚴重的牙齒雖然會搖晃, 但功能上並不成問題 以上顎正中門牙的牙周膜纖維附著面積為例, 根尖 3mm 的吸收相當於齒槽骨頂 1mm 的骨吸收, 牙根嚴重吸收的牙齒, 不會因為如此而直接造成牙齒脫落 50 1 陸 牙根吸收的臨床考量與應變 1. 患者或家長應被事先告知, 牙根吸收是矯正治療中可能發生的副作用, 其發生原因 發生機制 發生率目前尚未明確 2. 根尖 X 光片 : a. 根尖 X 光片是矯正完整記錄中的重要部分 特別是可以用來比較術前 術中與術後的牙根吸收變化 b. 初診時應利用 X 光片檢查牙根長度 型態 有無吸收以及牙根與皮質骨的位置關係 c. 由於牙根吸收的發生無法預測, 因此有必要做定期 X 光片檢查 矯正開始 6 個月後, 應拍上下顎門牙的口內 X 光片, 檢討牙根的型態變化 如果患者本身存在有風險因素時, 治療開始 3 個月後就要拍 X 光片 d. 矯正治療後的 X 光片很重要, 可用來評估齒槽骨與牙根的變化 e. 接受轉診案例時, 最好先做全口 X 光檢查 3. 矯正治療的適當時機 : 發育中的牙根較不 會發生吸收現象, 因此矯正治療應盡早開始 另外年幼患者對於咬合變化, 其肌肉適應較佳 成人的適應能力則較差 換言之, 成人矯正時應特別注意 4. 綜合考量治療原理後, 減少矯正力量並盡量縮短治療時間 矯正力量應該輕且間歇性 ( 重且持續性的力量較容易發生牙根吸收 ) 樊 20 明文認為 : 臨床醫師應使用輕力與間歇力, 因為組織需要修復期 加力越頻繁, 修復期就越短, 可能傷害牙齒和骨組織 延長回診間隔時間, 可預防和減少上述損傷的發生 臨床上固定矯正器一般間隔 4-6 周加力一次 ; 活動矯正器 2-3 周加力一次為宜 卷 5 期

8 5. 動態治療中如發現牙根吸收, 首先回顧術前資料, 分析發生原因, 然後重新評估最終治療目標 必要時應中止治療或妥協 停止矯正力量或以咬合板緩衝咬合力 6. 停止咬指甲或吐舌頭等不良習性, 有文獻指出這種矯正患者的牙根吸收更為嚴重 7. 所有型態的牙齒移動都會造成牙根吸收 下壓內縮 (Intrusion) 似乎是最嚴重 8. 咬合創傷可能對牙根有害, 應盡量去除 治療完成時盡量位於穩定的咬合 9. 常態下的矯正治療有其解剖學上與生理學上的極限, 認知這一點很重要 如果治療計劃超過這些極限, 可能需要配合外科處理 10. 牙根吸收的牙齒如要作為牙橋的支台齒使用, 則牙根長度需超過臨床牙冠長度 11. 早期治療階段的骨骼移動 (orthopedic effect) 和後期治療階段的牙齒移動 (dentoalveolar effect) 相比, 早期治療比較不會破壞牙根 12. 選擇治療裝置時, 除了裝置的效果與患者的需求之外, 應考量牙根吸收的風險 13. 受傷的牙齒在矯正治療時, 應盡量謹慎, 因為這些牙齒可能更容易發生牙根吸收 14. 發生嚴重牙根吸收時, 醫學檢查與家族傾向可能具有一定意義 15. 動態治療結束後, 應持續注意外傷性咬合與維持裝置的力量仍然可能是造成牙根吸收的因素 固位期間, 如果牙根繼續吸收, 建議做根管治療並以氫氧化鈣充填, 只有確定牙根吸收已停止, 才可以馬來膠 (gutta-percha) 充填 16. 如果牙根吸收後需要重新矯正治療, Kokich VG 5 (2008) 認為 : 必須限制牙齒移動量 限制治療時間與避免下壓力量 (to limit the amount of tooth movement, limit the length of orthodontic treatment, and avoid intrusive tooth movements ) 17. 個案本身存在危險因素時, 治療計畫中應包含迴避相關危險因素的治療法, 即使該危險因素尚未被證明 18. 治療機制的設計, 應確認牙冠與牙根的移動方向 牙齒移動方向與治療目標相反時, 牙 根尖的 jiggling 會造成其治療期間以及牙根尖應力之作用期間的長期化, 應盡量避免 19. 對於牙根嚴重吸收時的應變,Levander 等人建議, 不要讓牙齒受到矯正力量, 在被動狀態下保持金屬線在溝內, 經過 2-3 個月的觀察, 再按原定治療計畫, 繼續動態治療, 則可以抑制牙根吸收的進行 這個應變方法的有效性雖然還未被證實, 但卻是一個值得嘗試的方法 Krishnan(2009) 建議 (Whenever root resorption is observed, a temporary halt in treatment for 4-6 months is advised, which helps in preventing further resorption. Some repair processes can also occur, such as smoothing and remodeling of cemental surfaces with restoration of the normal width of the PDL.) 20.Makedonas 15 (2008) 的報告中, 發現牙根中度吸收後的處置方法有 : (1)alter treatment plan, (2)use lower forces, (3) use longer intervals, (4)use resting periods, (5) decrease total duration, (6)no treatment, (7)use of removable appliances. 其中以 (2), (4), (5) 最常被使用 取自 :Makedonas 15 Treatment modalities when moderate root resorption (from 2 mm up to 1/3 of the root length) is diagnosed during treatment 卷 5 期 47

9 柒 個案檢討 : 黃 小姐 (34 歲 ) 因上顎門牙搖晃來門診, 希望拔牙做進一步治療 據患者表示,21 歲時做矯正, 四年後結束 (25 歲 ), 裝置拆掉後發現上顎四顆門牙已搖晃, 就將四顆牙齒做陶瓷牙冠 splinting 口內 X 光檢查發現,12 已不在口內,11 21 牙根嚴重吸收周圍無齒槽骨,22 牙根中度吸收周圍只剩少許齒槽骨 換言之, 這四顆門牙矯正後, 只存在大約 9 年 捌 結論 : 關於矯正造成的牙根吸收, 有些專家將其稱為醫源性 (iatrogenic), 雖然這種牙根吸收是不做矯正就不會發生, 冠上 醫源性 三個字似乎就變成是醫師的錯, 容易造成誤解, 其實這個現象除了醫師要小心預防之外, 和患者個人體質 ( 基因 ) 有關, 可以說是矯正過程中無法避免的病理結果 (an unavoidable pathologic consequence of orthodontic tooth movement. 4 ) 臨床矯正上看到的牙根吸收, 通常是局限於牙根表層或牙根尖的小吸收, 最後由 cementoblast 修復發生吸收的部位, 因此臨床上不會有問題 然而偶爾臨床矯正上會發生廣泛的或嚴重的牙根吸收, 造成臨床上的安全意外與困擾 既然 OIIRR 和發炎反應有關, 矯正中除了服用抗發炎的藥物之外, 我認為上嘴唇冰敷或口內含冰塊, 或許多少有些幫助 這一點雖然我並無證據 矯正造成的牙根吸收有點類似地震, 雖然對於個別患者其發生率 (incidence) 與嚴重性 (extent) 很難預測, 但我們仍然在治療前就應告知風險, 治療中則應定期監控 ( 照 X 光檢查 ), 治療後也要預防外傷性咬合 (Relapse, Jiggling) 造成再次吸收 參考文獻 : 下野正基 前田健康 溝口到編 Clinical Biomechanics on Tooth Movement--- Dynamism of Bone and Periodontal Ligament --- ( 原文日文 : 歯の移動の臨床バイオメカニクス 骨と歯根膜のダイナミズム ) 医歯薬出版株式会社 2006 于世鳳主編口腔組織病理學人民衛生出版社 2001 Heithersay GS Management of tooth resorption J Australian Dent Endo Supple 2007;52:1 Brezniak N, Wasserstein A, Orthodontically Induced Inflammatory Root Resorption.
