3 Chapter 6 General discussion and summary Biomaterial implants are frequently used in modern medicine to restore function and form of human anatomy. The introduction of dental implants presents a major technological advancement in modern dentistry. Implants provide a strong foundation and comfort for patients after tooth loss. Dental implants are placed at a rate of about 100, ,000 per year, according to data provided by the American Association of Oral and Maxillofacial Surgeons. Worldwide, the number of implants is still increasing (chapter 1). However, the presence of implanted biomaterials dramatically elevates infection risk (chapter 2). Paradoxically dental implants placed in a bacteria-laden milieu, experience moderate failure rates due to infection ( %) similar to the ones of joint arthroplasties placed in a near-sterile environment ( %). Transcutaneous bone fixation pins breach the immune-barrier of the epidermis exposing underlying sterile tissue to an unsterile, external environment. In contrast to dental implants, also placed in a highly unsterile environment, these pins give rise to relatively high infection-associated failure-rates of up to 23.0%. In chapter 2 we identify causes as to why dental implants so often succeed where others fail. The major part of all implants considered are metalmade, with similar surface-finishes. Material choice was therefore discarded as underlying the paradox. Antimicrobial activity of saliva has also been suggested as a cause for the success of dental implants, but was discarded because saliva is the implant site fluid from which viable bacteria adhere. Crevicular fluid was discarded as it is largely analogous to serum. Instead, we attribute the relative success of dental implants to ability of oral tissues to heal fast in the continuous presence of commensal bacteria and opportunistic pathogens, and tolerance of the oral immune-system. Inability of local tissue to adhere, spread and grow in presence of bacteria and an intolerant immunesystem are identified as the main causes explaining the susceptibility of other implants to infection-associated failure. Although in this chapter, material choice was discarded as a cause for the infection-paradox, at the same time we do not rule out that amongst the many new materials and coatings under pre-clinical investigation, developments are ongoing that will eventually improve the performance of dental implants even further. Pre-clinical research has been hampered however, by lack of appropriate in vitro models to study the complicated race for the surface between bacteria and tissue cells, as occurring in many implant scenarios including the dental one. Therefore the aim of this thesis was (chapter 1) to carry out the race for different dental implant surfaces in a perioperative model, using supra-gingival bacterial strains versus human gingival fibroblast cells, and in a late post-operative model involving periodontopathogens in their struggle with osteoblast cells. 96

4 General discussion and summary Implant materials and surface properties will be identified that influence the outcome of the race for the implant surface between oral bacteria and tissue cells. Titanium has been widely and successfully used for dental implants, mainly due to its superior mechanical properties, corrosion resistance and biocompatibility, but it does not meet all the clinical requirement in spite of these advantages. In order to reduce the incidence of infection-associated implant failure, improve tissue integration and decrease biofilm formation on dental implants, different modifications have been applied to modify the chemical composition, hydrophobicity and roughness of titanium implants [1-3]. Also materials variants have been employed, including titanium-zirconium alloys and zirconia in the different surface variants, as studied in this thesis. Dental implants anchor in bone through a tight fit and osseointegratable properties of the implant surfaces, while a protective soft tissue seal around the implants neck is needed to prevent bacterial invasion to and destruction of the bone-implant interface. This tissue seal needs to form in the unsterile, oral environment. In chapter 3, we aim to identify surface properties of dental implant materials (titanium, titaniumzirconium alloy and zirconium-oxides) that determine the outcome of this race-for-thesurface between human gingival fibroblasts and different supra-gingival bacterial strains. Biofilms of three streptococcal species and Staphylococcus aureus strain were grown in mono-cultures on the different implant materials in a parallel plate flow chamber and their thickness evaluated using confocal scanning laser microscopy. Similarly, adhesion, spreading and growth of human gingival fibroblasts were evaluated. Co-culture experiments with bacteria and human gingival fibroblasts were carried out to evaluate tissue interaction with bacterially contaminated implant surfaces. Implant surfaces were characterized by their hydrophobicity, roughness and elemental composition. Biofilm formation occurred on all implant materials, and neither roughness nor hydrophobicity had a decisive influence on biofilm formation. Zirconium-oxide attracted most biofilm. All implant materials were covered by human gingival fibroblasts for 80-90% of their surface areas. Human gingival fibroblasts lost the race for the surface against all bacterial strains on nearly all implant materials, except on the smoothest titanium variants. Smooth titanium implant surfaces provide the best opportunities for a soft tissue seal to form on the neck of bacterially contaminated implant surfaces. This conclusion could only be reached in co-culture studies and coincides with the results from the few clinical studies carried out to this end, but does not necessarily need to be valid for the osseointegratable part of dental implants. Sub-gingival anaerobic pathogens can colonize an implant surface to compromise osseointegration of dental implants once the soft tissue seal around the neck of an implant is broken. In vitro evaluations of implant materials are usually done in mono-culture studies involving either tissue integration or bacterial colonization. Coculture models, in which tissue cells and bacteria battle simultaneously for estate on an 97

5 Chapter 6 implant surface, have been demonstrated to provide a better in vitro mimic of the clinical situation. In chapter 4, we compare the surface coverage by U2OS-osteoblasts cells prior to and after challenge by two anaerobic sub-gingival pathogens in a co-culture model on differently modified titanium, titanium-zirconium alloys and zirconia surfaces. Monocultures studies with either U2OS-osteoblasts or bacteria were also carried out and indicated significant differences in biofilm formation between the implant materials, but interactions with U2OS-osteoblasts were favourable on all materials. Adhering U2OSosteoblasts cells however, were significantly more displaced from differently modified titanium surfaces by challenging sub-gingival pathogens than from titanium-zirconium alloys and zirconia variants. Combined with the results from chapter 3, employing a coculture model consisting of human gingival fibroblasts and supra-gingival oral bacteria, results point to a different material choice to stimulate the formation of a soft tissue seal as compared to preservation of osseointegration under the unsterile conditions of the oral cavity. Many studies have shown physico-chemical characteristics of the implant materials, such as surface roughness, hydrophobicity and chemical composition of the implant surface, to be influential for tissue integration and biofilm formation on dental implants surfaces [1-3], but hitherto never in co-culture studies as done in this thesis. In order to identify physical-chemical surface properties that define the outcome of monoversus co-culture studies on dental implant materials, we aim in chapter 5 to identify the physico-chemical characteristics of dental implant materials that can serve as predictive factors with respect to the above processes through multiple backward linear regression analysis. This analysis yields the unfortunate conclusion that in vitro biofilm studies, in vitro studies into adhesion, spreading and growth of tissue cells on the different implant surfaces or in vitro co-culture studies on the competition between bacteria and tissue cells on dental implant materials, are unable to identify physico-chemical implant surface characteristics that predict the above processes. Possible reasons why we were unable to identify such surface characteristics can only be speculated upon. Linear regression analysis may not be appropriate, and possibly other types of regression analyses should have been attempted, although this is unlikely considering the varying outcomes of the linear regression analysis. The surface characteristics evaluated in this thesis are all macroscopic characteristics, averaging characteristics over a relatively large area. Therewith influences of minor surface heterogeneities are neglected, while they are influential on, for instance, micron-sized particle deposition [4]. This is of particular importance in the race for the surface, since bacteria and tissue cells respond differently to surface features with different length-scale [5, 6]. Whereas on the one hand, we were unable to identify physico-chemical characteristics that control the race for the surface on dental implant materials, materials 98

6 General discussion and summary on the other hand yielding the most favorable biological responses could be identified. Moreover, our conclusions on preferred materials with respect to dental implant infection-associated failure from co-culture studies are in line with clinical findings and provide more relevant information than traditional in vitro mono-culture studies addressing solely bacterial or tissue response to biomaterials. This is a major achievement in the current thesis, especially since also the relevance of animal experiments can be doubted as animal microflora is very different from the human one [7]. Moreover, it is almost impossible to evaluate new materials in clinical trials with sufficient statistical power. To conclude, innovative methods to evaluate the infection-resistance of dental implant materials have pointed out that different requirements should be posed to the materials constituting the neck of the implant versus those making up the osseointegratable parts. This is a new conclusion that warrants clinical evaluation, despite statistical limitations of such clinical studies. Inclusion of only a limited number of materials variants, carried out in high-risk patient groups consisting of smokers or patients with a predisposition to peri-implantitis, may reduce the number of patients that need to be included in a clinical evaluation to feasible levels. 99

Deze implantaten worden veelvuldig gebruikt in de moderne geneeskunde om functie en vorm van de humane anatomie te herstellen. De introductie van tandheelkundige implantaten heeft een grote technologische vooruitgang betekend voor de moderne tandheelkunde. Tandheelkundige implantaten voorzien patiënten van veel comfort na verlies van tanden, en worden ongeveer tot keer per jaar geplaatst, volgens de gegevens van de Amerikaanse vereniging van Orale en Maxillofaciale chirurgen. Wereldwijd neemt het aantal geplaatste tandheelkundige implantaten nog steeds toe (hoofdstuk 1). Echter, de aanwezigheid van geïmplanteerde biomaterialen zorgt voor een verhoogde kans op infecties rondom deze implantaten (hoofdstuk 2). Vreemd genoeg ondervinden tandheelkundige implantaten die geplaatst worden in een bacterie-rijke omgeving lage uitvalspercentages ( %), vergelijkbaar met de uitvalpercentages van gewrichtsprothesen die geplaatst worden in een nagenoeg steriele omgeving ( %). Externe bot fixatie pinnen gaan door de huid en verbreken daardoor de natuurlijke barrière waardoor steriel weefsel wordt blootgesteld aan een niet-steriele omgeving. In tegenstelling tot tandheelkundige implantaten, die ook geplaatst worden in een nietsteriele omgeving, komt bij deze pinnen een relatief hoog uitvalspercentage voor, welke kan oplopen tot 23%. In hoofdstuk 2 identificeren we mogelijke oorzaken waarom tandheelkundige implantaten vaker succesvol zijn dan andere implantaten. Het overgrote deel van de beschouwde implantaten zijn van metaal gemaakt, met een vergelijkbare oppervlakteafwerking. Materiaalkeuze werd daardoor verworpen als oorzaak van deze paradox. De anti-microbiële werking van speeksel is ook overwogen als een reden voor het succes van tandheelkundige implantaten, maar ook deze reden werd verworpen omdat het implantaat omringd wordt door speeksel, terwijl de bacteriën die gaan hechten aan een implantaat juist uit het speeksel komen. De aanwezigheid van creviculaire vloeistof werd verworpen als reden voor de paradox, aangezien het vergelijkbaar is met serum wat gewrichtsprothesen omringt. Uiteindelijk kennen we het succes van tandheelkundige implantaten toe aan het vermogen van mondweefsel om snel te genezen in de aanwezigheid van commensale bacteriën en opportunistische pathogenen, en de evolutionaire tolerantie van het orale immuunsysteem. De belangrijkste oorzaken waarom andere implantaten meer gevoelig zijn voor infecties, is het onvermogen van lokaal weefsel om te hechten en te groeien aan het implantaat in de aanwezigheid van bacteriën, en een intolerant, overactief immuunsysteem dat vervolgens gefrustreerd raakt. Hoewel we in dit hoofdstuk materiaalkeuze uitsluiten als een reden voor de infectie-paradox, sluiten we niet uit dat nieuwe materialen en coatings die momenteel 101

9 Hoofdstuk 6 preklinisch worden onderzocht, het succes van tandheelkundige implantaten nog verder kunnen verbeteren. Preklinisch onderzoek wordt echter belemmerd door gebrek aan geschikte in vitro modellen om de ingewikkelde race naar het oppervlak tussen bacteriën en weefselcellen te bestuderen, zoals dit gebeurt tijdens het plaatsen van een implantaat. Daarom is het doel van dit proefschrift (hoofdstuk 1) om de race naar verschillende tandheelkundige implantaten te onderzoeken tussen supragingivale bacteriestammen en humane fibroblasten geisoleerd uit tandvlees, in een perioperatief model, en de race tussen paropathogenen en osteoblastcellen, in een laat postoperatief model. Implantaat materialen en oppervlakte eigenschappen worden geïdentificeerd, die de race naar het oppervlak van het implantaat tussen orale bacteriën en weefselcellen mogelijk beïnvloeden. Titanium wordt met succes op grote schaal gebruikt als materiaal voor tandheelkundige implantaten, vooral vanwege superieure mechanische eigenschappen, corrosie-bestendigheid, en bio-compatibiliteit, maar voldoet desondanks niet aan alle klinische eisen. Om de kans op falen van een implantaat door een implantaat gerelateerde infecties te verminderen, de weefselintegratie te verbeteren, en biofilm vorming te verminderen, zijn er verschillende modificaties aangebracht om de chemische samenstelling, hydrofobiciteit en de ruwheid van titanium implantaten te optimaliseren. Ook veranderingen in de materiaalkeuze zelf worden klinisch toegepast, namelijk het gebruik van titanium-zirkonium legeringen en zirkonia in de verschillende oppervlakte varianten. Tandheelkundige implantaten zijn verankerd in het bot door een strakke pasvorm en goede bot integratie eigenschappen van het implantaat, terwijl rondom de hals van het implantaat een afsluiting door zacht weefsel nodig is om een bacteriële invasie en vernietiging van het bot implantaat interface te voorkomen. Deze weefselafsluiting moet gevormd worden in de niet-steriele orale omgeving. In hoofdstuk 3, hebben we de oppervlakte eigenschappen van tandheelkundige implantaten (titanium, titanium-zirkonium legering en zirkonium-oxiden) geïdentificeerd, die mogelijk de uitkomst bepalen van de race naar het oppervlak tussen humane fibroblasten en verschillende supragingivale bacteriestammen. Biofilms van drie streptokokken stammen en een Staphylococcus aureus stam werden gekweekt in een mono-cultuur op de verschillende materialen in een parallelle plaat stroomkamer en de dikte van de biofilm werd geëvalueerd met behulp van confocale laser scanning microscopie. Tevens werd de hechting, spreiding en groei van humane fibroblasten geëvalueerd in de parallelle plaat stroomkamer. Co-cultuur experimenten met bacteriën en humane fibroblasten werden uitgevoerd om de interactie tussen weefsel en gecontamineerde implantaat oppervlakken te evalueren. Implantaat oppervlakken werden gekarakteriseerd aan de hand van hydrofobiciteit, ruwheid, en elementaire samenstelling. Biofilm formatie vond 102

10 Algemene discussie en samenvatting plaatst op alle implantaat materialen, en noch ruwheid noch hydrofobiciteit hadden enige invloed op de biofilm groei. Op zirconium-oxide groeide de meeste biofilm. Alle implantaat materialen werden voor 80-90% van het oppervlak bedekt met fibroblasten. Op alle materialen, behalve op de gladste titanium varianten, verloren humane fibroblasten de race naar het oppervlak van alle bacteriestammen. Gladde titanium oppervlakken bieden daarom de beste mogelijkheden voor zachte weefsel om een effectieve afsluiting rondom de hals van het implantaat te vormen. Deze conclusie kon alleen worden getrokken met behulp van co-cultuur studies en komt overeen met de resultaten van klinische studies, maar hoeft niet per se waar te zijn voor het bot geintegreerde deelte van een tandheelkundige implantaat. Sub-gingivale anaerobe pathogenen kunnen het implantaat oppervlak koloniseren en bot integratie van tandheelkundige implantaten in gevaar brengen wanneer de zachte weefsel afsluiting rond de hals van het implantaat verbroken is. Meestal worden in vitro beoordelingen van implantaat materialen uitgevoerd in een mono-cultuur studie, waarbij wordt gefocust op weefsel integratie of bacterie kolonisatie. Co-cultuur modellen, waarin weefsel cellen en bacteriën tegelijk strijden om het implantaat oppervlak, tonen aan dat dit in vitro model de klinische situatie beter nabootst. In hoofdstuk 4, vergelijken we de oppervlakte bedekking van U2OS-osteoblast cellen voor en na het toevoegen van twee anaerobe sub-gingivale pathogenen in een cocultuur model op verschillende bewerkte titanium, titanium-zirkonium legeringen en zirkonium oppervlakken. Mono-cultuur studies met alleen U2OS-osteoblasten of bacteriën zijn ook uitgevoerd, en lieten significante verschillen zien in biofilm formatie op verschillende implantaat oppervlakten, maar interacties met U2OS-osteoblasten waren gunstig op alle materialen. Hechtende U2OS-osteoblasten werden echter significant meer vervangen door sub-gingivale pathogenen op verschillende gemodificeerde titanium oppervlakken dan op titanium-zirkonium en zirkonium varianten. Samen met de resultaten van hoofdstuk 3, waarin een co-cultuur model bestaande uit humane fibroblasten en supragingivale orale bacteriën werd bestudeerd, wijzen de resultaten op een andere materiaal keuze voor stimulatie van fibroblasten om de hals van het implantaat af te sluiten dan het materiaal voor bot integratie. Vele studies hebben aangetoond dat de fysisch-chemische eigenschappen van implantaat materialen, zoals oppervlakte ruwheid, hydrofobiciteit en chemische samenstelling, invloed hebben op weefsel integratie en biofilm formatie op tandheelkundige implantaten, maar tot nu zijn er nooit co-cultuur studies uitgevoerd zoals in dit proefschrift is gedaan. Om de fysisch-chemische oppervlak eigenschappen te identificeren, die de uitkomst van mono- versus co-cultuur studies op tandheelkundige implantaten bepalen, identificeren we in hoofdstuk 5 de fysisch-chemische eigenschappen van tandheelkundige implantaat materialen die als voorspellende 103

11 Hoofdstuk 6 factoren kunnen dienen in de uitkomst van bovengenoemde methoden met behulp van meervoudige achterwaartse lineaire regressie analyse. Deze analyse leidt helaas tot de conclusie dat de uitkomsten van in vitro biofilm studies, in vitro studies naar hechting, spreiding en groei van weefsel cellen op verschillende implantaat oppervlakken, of in vitro co-cultuur studies gericht op de competitie tussen bacteriën en weefsel cellen op tandheelkundige implantaten, niet voorspeld kunnen worden door de gemeten fysischchemisch eigenschappen. Wij kunnen alleen maar speculeren naar de redenen waarom we niet in staat zijn om voorspellende oppervlakte eigenschappen te identificeren. Lineaire regressie analyse is misschien niet het meest geschikt, en mogelijk hadden andere typen regressie analyses kunnen worden toegepast, hoewel dit onwaarschijnlijk is als we de uitkomst van de lineaire regressie analyse beschouwen. De oppervlakte eigenschappen bestudeerd in dit proefschrift zijn allemaal macroscopische eigenschappen, die een gemiddelde zijn over een relatief groot oppervlak. Hierdoor worden kleine oppervlakte heterogeniteiten verwaarloosd, terwijl deze van invloed kunnen zijn op de hechting van micron-deeltjes [4]. Dit is van groot belang in de race naar het oppervlak, omdat bacteriën en weefselcellen verschillend reageren op oppervlak heterogeniteiten [5, 6]. Terwijl we de fysisch-chemische eigenschappen van tandheelkundige implantaat materialen die de race naar het oppervlak bepalen, niet konden identificeren, konden we wel materialen die voor de meest gunstige biologische respons zorgen identificeren. Verder zijn onze conclusies met betrekking tot tandheelkundige implantaat gerelateerde infecties vanuit de co-cultuur studies in lijn met klinische resultaten en geven deze meer relevante informatie dan traditionele mono-cultuur studies die zich of alleen op de bacterierespons of alleen op de weefselrespons richten. Dit is een belangrijke uitkomst van dit proefschrift, vooral omdat aan de relevantie van dierproeven kan worden getwijfeld, aangezien de dierlijke microflora heel anders is dan de menselijke microflora. Bovendien is het bijna onmogelijk om nieuwe materialen te evalueren in klinische studies met voldoende statistische power. Concluderend, innovatieve methoden om de infectie weerstand van tandheelkundige implantaat materialen te evalueren hebben aangetoond dat er verschillende eisen moeten worden gesteld aan het materiaal voor de hals van een implantaat en voor het bot ge-integreerde deel. Dit is een nieuwe conclusie die verdere klinische evaluatie nodig heeft, ondanks de statistische beperkingen van klinische studies. Het includeren van een beperkt aantal materiaal-varianten in risicovolle patiënten groepen, zoals rokers en patiënten met een peri-implantitis verleden, kan het aantal statistisch-noodzakelijke patiënten in klinische studies verminderen tot haalbare aantallen. 104

12 讨论与总结 讨论与总结 生物材料种植体在现代医学中常用于修复人体解剖功能和形态 其中口腔种植体的是现代口腔医学的一项巨大技术成果 种植体为失去本体牙的患者提供了实用的功能修复以及口腔舒适性 根据美国口腔颌面外科医师协会的数据, 每年口腔种植牙的植入量高达十万至三十万颗 在世界范围内, 种植体的使用数量每年仍在上升 ( 第一章节 ) 然而随着生物材料的植入, 其所致体内感染的风险也显而易见 ( 第二章节 ) 矛盾的是尽管口腔种植体是植入于一个充满细菌的环境中, 但是只有相对少量的种植体失败是由于感染导致 ( %), 感染率和在几乎无菌环境下所实施的关节置换手术相近 穿皮骨钉破坏了真皮组织的免疫屏障, 使其同时处于无菌的内部组织环境和有菌的外部环境中 令人费解的是, 尽管口腔种植体也被植入在一个未消毒的环境中, 由于感染引致的骨钉失败率却高达 23.0% 在第二章节中我们探讨了为什么口腔种植体临床成功率高而其他的种植体却常常失败的原因, 也即本章提出的 感染悖论 考虑到所有种植体最主要的部分都是由金属制成, 具有相似的表面光洁度, 因此材料的因素并不成立 唾液的抗菌性也被认为是种植牙高成功率的一个原因, 但是由于其流经种植体时会粘附细菌所以这个因素也不成立 龈沟液也不太可能是其中的原因是由于它在很大程度上成分类似于血清 相反, 口腔种植体的广泛成功是由于口腔组织即使和共生菌, 条件致病菌共存的条件下仍能够快速愈合, 另外口腔免疫系统具有相对更强的耐受性 在细菌存在的环境下, 局部组织缺乏粘附, 扩散和生长的能力以及缺乏耐受性的免疫系统被认为是其他植入物易感的主要原因 尽管在本章中材料的选择并不被认为是感染的主要因素, 我们并不排除临床前期对新型材料和材料表面涂层的研究以及该领域目前的快速发展将最终进一步提高了口腔种植体的性能 细菌和组织细胞之间复杂的 表面争夺战 存在于许多生物种植体 由于缺乏适当的体外模型, 其临床前的研究目前受到了诸多限制 因此, 本文的主要目的 ( 第一章节 ) 是分别探讨了围术期体外模型中龈上菌群与牙龈成纤维细胞在不同种植体材料表面之间进行的 争夺战 以及在术后期体外模型中牙周致病菌与成骨细胞在不用种植体材料表面的竞争机制 105

13 第六章 种植体材料和表面特性被认为是影响口腔细菌和组织细胞在种植体材料表面竞争的主要因素 尽管钛金属由于其优越的机械性能, 耐腐蚀性和组织相容性已被广泛且成功地用于口腔种植体, 但是仍不能完全满足所有的临床要求 为了降低由于感染所导致的种植体失败的发生率, 不同的表面改性方法被应用于钛种植体, 例如材料表面化学元素成分的调整, 疏水性以及粗糙度的改性, 以提高组织整合和减少生物膜在口腔种植体表面的形成 本文也研究了对不同的材料包括钛化锆合金和二氧化锆材料表面的改性和修饰 口腔种植体通过其紧密贴合和植入体表面的骨整合而锚在牙槽骨内, 而种植体颈部形成的软组织密封带是防止细菌侵入和损坏骨与种植体表面接触的重要防御 这一软组织密封带需要形成在未经消毒的口腔环境中 在第三章节中, 我们的研究目的是确定种植体材料 ( 钛, 钛化锆合金和二氧化锆 ) 表面的特性是如何影响不同龈上菌群与牙龈成纤维细胞在不同种植体材料表面进行 争夺战 的结果 本研究中我们采用了对 3 种不同链球菌株和金黄色葡萄球菌在装有不同种植体材料的平行板流动室内进行单一生物膜的培养, 之后使用激光共聚焦显微镜来对生物膜的厚度进行测量, 同时对牙龈成纤维细胞的粘附, 扩散以及生长也进行相应的测量 随后在共培养试验中, 进一步的评估了牙龈成纤维细胞与已受细菌污染的种植体表面的相互作用 本实验中我们对种植体材料的表面特性也进行了表征鉴定, 主要包括疏水性, 粗糙度以及化学成分 但是无论是材料表面粗糙度或是疏水性都并不会对种植体上形成的生物膜产生显著的影响 氧化锆材料表面上出现了相对最多的生物膜 牙龈成纤维细胞在所有的种植体材料表面的覆盖率高达 80-90% 除了在最光滑的钛改性材料表面上, 牙龈成纤维细胞在其他的种植体材料表面上与对不同的菌株的 争夺战 中均以失败告终 相对光滑的钛表面最有利于软组织密封带在已被污染种植体表面的形成 这一由只有通过共培养方法所能获得的结论与一些临床研究的结果相吻合, 虽然其并不一定适用于口腔种植体中的骨结合部分 一旦种植体颈部的软组织密封带遭到破坏, 龈下厌氧致病菌便会群集在种植体表面从而危害到种植体的骨整合 通常体外评价种植体材料的实验选用细胞组织在种植体表面培养或是细菌在种植体表面培养这种单一的培养模型 显而易见, 共培养模型的优势在于其可以在同一时间研究材料表面上细菌与组织细胞的争夺战, 106

14 讨论与总结并且这一特点也被证明是相比单一培养模型而言更好地模拟了临床的实际情况 在第四章节中, 我们选用了共培养模型研究不同表面改性的钛, 钛化锆和二氧化锆的材料其表面成骨细胞在龈下致病菌侵入前后的覆盖面积 对成骨细胞或细菌在不同的材料表面进行单一培养显示生物膜的形成存在显著性地差异, 我们发现所有的材料都有利于成骨细胞的生长 然而相比钛化锆合金和二氧化锆, 在改性的钛材料表面上粘附的成骨细胞更容易因为龈下致病菌的侵入而被细菌所替代 结合第三章节采用由牙龈成纤维细胞和龈上口腔细菌组成的共培养模型的结果, 本论文综合表明有利于龈上软组织密封带的形成和有菌环境下保持龈下骨整合分别要求不同的材料特性 许多研究表明种植体的物理化学特性会影响与种植体表面的组织结合和生物膜生成, 例如材料表面的粗糙度, 疏水度和化学成分, 但是至今文献中尚未报道本论文所使用的共培养模型 为了明确证明材料表面的物理化学性质如何影响单一培养和共培养的结果, 在第五章节中我们将口腔种植体材料的物理化学性质作为预测因子对单一培养和共培养的实验结果进行多元性反向线性回归分析 遗憾的是这个分析方法得到的结果无法指向明确的结论 : 无论是体外研究的单一生物膜的培养实验, 组织细胞实验以及细菌与细胞在材料表面上相互竞争的共培养, 材料的表面性质都无法作为主导预测因子而影响最终的实验结果 由此我们只能猜测无法鉴别出材料表面性质影响的原因 一种可能是线性回归分析对此类实验并不适用 虽然还可以尝试其他类型的回归分析, 考虑所有不同的线性回归分析的结果并不现实 另外, 本文评估的表面特性都是宏观特性, 在一个相对较大的区域里对特性进行了平均化 因此一些不大显著的表面差异就被忽略了, 但是这些表面特性很有可能会造成实质性的影响, 比如微米级的颗粒的沉积 [4] 这些因素可能会对种植体表面的 争夺战 中产生重要的影响, 特别是因为细菌与组织细胞对材料表面不同尺度的特征有着不同的反应 [5, 6] 从另一个角度而言, 虽然我们无法证实材料的物理化学特性是如何影响种植体材料表面的 争夺战 的结果, 但是本文证实了能对材料产生有利的生物反应 考虑到动物实验中微生物群落与人有很大的不同 [7], 我们研究感染所致种植牙失败所采用的体外共培养试验中相比传统的体外单一培养试验中所得到的结果更加贴近 107

15 第六章临床研究结果, 这是本论文的重要成果, 此外也很难基于少量的病例数量而进行临床评估新的生物材料 最后, 使用创新的方法去评估抗感染口腔种植体材料也应根据不同的要求以及不同的种植体组成材料而不尽相同 这个新的结论也得到了临床试验的肯定, 尽管临床研究的统计数据有一定的局限性 由于只有有限的改性材料最终用于临床试验, 并且高危患者群中包含了吸烟患者, 其更容易患有种植体周围炎症, 所以通过研究高危人群从而减少患者数量而去提高临床评价看起来是更为可行的研究方式 108
