Once this credit facility has been successfully arranged, we will settle your eligible medical expenses directly with the hospital or clinic upon your discharge or treatment. This gives you complete peace of mind and frees you from the hassle of settling the medical bills and making a claim for reimbursement. Medical networks are provided by our appointed service provider and its authorised agents, and subject to the availability of the services at the requested location. It is not a contractual service. We have the right to change the appointed service provider, and such service is subject to termination at any time without prior notice. 2. Age of enrolment for spouse and dependent children: spouse aged between 18 and 70 years of age and dependent children aged between 15 days and 17 years of age (or under 23 if unmarried and full-time students). 3. Your policy may be renewed annually during your lifetime subject to the continual availability of this plan and the payment of the renewal premium based on attained age and the applicable premium table upon renewal. Such premium table is not guaranteed and may be adjusted. However, we will neither charge extra premium nor impose additional exclusions on your policy based on your health status and claims history at the time of renewal. We reserve the right to revise the benefit structure and/or any other product features on any policy anniversary by giving at least 30 days prior written notice. 4. The Priority Cashless Arrangement or credit facility arrangement is not a contractual service but an administrative arrangement offered in our absolute discretion with respect to the eligible medical expenses under this arrangement and subject to the availability of such services. If the incurred amount has exceeded the eligible claim amount, the shortfall will be borne by the policyholder, and we have the right to recover any shortfall amount from the policyholder. We have the right to change the appointed service provider, and such service is subject to termination at any time. 5. Prior authorisation from our appointed service provider is required on some benefit items. Your benefit amount payable will be reduced by 50% if the hospitalisation or day case surgery is rendered in the USA or Western Europe without obtaining prior authorisation from our appointed service provider, unless it was due directly to an accident or an emergency. Western Europe includes Austria, Belgium, Denmark, Finland, France, Germany, Greece, Iceland, Ireland, Italy, Luxembourg, Monaco, Netherlands, Norway, Portugal, Spain, Sweden, Switzerland, United Kingdom and Vatican City. 6. Subject to the annual deductible for Core benefits only (except Wellness benefits) and/or coinsurance for Out-patient Benefits, if selected. 7. Permanent residents of the USA residing in the USA cannot apply for Worldwide Coverage Area. Your benefit amount payable will be reduced by 50% if you have resided in the USA for 183 days or more in the past 12 months at the time of seeking medical treatment in the USA. We reserve the right to change the coverage area from Worldwide to Worldwide excluding the USA or Asia if you have resided in the USA for 183 days or more in the past 12 months. 8. Worldwide excluding the USA: worldwide, not including the United States of America. 9. Asia includes Afghanistan, Bangladesh, Bhutan, Brunei, Cambodia, China including (Hong Kong and Macau), India, Indonesia, Japan, Kazakhstan, Kyrgyzstan, Laos, Malaysia, Maldives, Mongolia, Myanmar, Nepal, North Korea, Pakistan, Philippines, Singapore, South Korea, Sri Lanka, Taiwan, Thailand, Timor-Leste, Turkmenistan, Uzbekistan and Vietnam. 10. The annual deductible is an amount that you or each person on your policy pays towards the cost of your own medical expenses. We will deduct this amount from any benefit amount payable to Core benefits only (except Wellness benefits). 11. If optional Out-patient Benefits is selected, a 10% co-payment applies to the optional Out-patient Benefits if the HK$10,000 annual deductible is selected for Core benefits; and a 20% co-payment applies to the optional Outpatient Benefits if the HK$25,000 annual deductible is selected for Core benefits. 12. All benefits (include optional benefits) are subject to the annual and lifetime limits, including those benefits which indicate. All benefits are payable for an eligible medical condition that incur usual, customary and reasonable charges. 15. Lifetime limit refers to the maximum total amount of benefits we will pay under all medical insurance policies issued by us, whether or not still in force, which have a specific lifetime limit in line with their terms and conditions. 16. Annual limit refers to the maximum total amount of benefits (include optional benefits), we will pay under all benefits of this policy for covered person. 17. Benefits payable will be adjusted if confined to a hospital room of any class above Standard Private Room: if confined to a room of the immediate class above or two classes above, the benefit amount payable will be reduced to 50% and 20% respectively. 18. Examples of covered Hospital Services include drugs and medicine prescribed by the attending physician, dressing, ordinary splints and plaster casts used in the hospital, laboratory examinations, X-rays, ambulance service to or from the hospital for hospital confinement. 19. Policy year refers to each twelve-month period starting on the date on which the policy takes effect. 20. Specified Items of Medical Appliances include a) pace maker, b) coronary stents, c) intraocular lens, d) artificial cardiac valve, e) metallic or artificial joints, f) prosthetic ligaments for replacement or implantation between bone, and g) prosthetic intervertebral disc. 21. Other Items of Medical Appliances include prosthetic devices other than stated in Specified Items. 22. Immediate family member refers to the insured person s spouse, children, parents, parents-in-law, siblings, grandparents, grandchildren and legal guardians/wards. 23. Cover for Hospice and Palliative Care benefit commences after the policy has been continuously in-force for three hundred and sixty-five (365) consecutive days. 24. Maternity and Newborn Treatment benefit is applicable to females aged 18 and above only. This benefit is only effective and payable after a waiting period of three hundred and sixty-five (365) consecutive days. 25. Covered pregnancy complications include a) ectopic pregnancy, b) molar pregnancy, c) disseminated intravascular coagulopathy, d) pre-eclampsia, e) threatened abortion, f) foetal death, g) postpartum haemorrhage requiring hysterectomy, and h) eclampsia, amniotic fluid embolism and pulmonary embolism of pregnancy. 26. Covered newborn treatments include a) esophageal atresia and pyloric stenosis, b) biliary obstruction, c) intestinal obstruction inclusive of anal atresia, d) thoracotomy or laparotomy, e) endoscopy surgery, catheter surgery or any such proven new surgical procedures, up to a limit of four times. 27. The Worldwide Emergency Assistance Services are provided by our appointed service provider and its authorised agents, and subject to the availability of the services at the requested location. Generali, Generali HK Branch, the Company, we, us or our herein refers to Assicurazioni Generali S.p.A., Hong Kong Branch. 2. This brochure is for general information purpose only. It is not a contract of insurance, and it does not form any part of the policy contract. Details are subject to the terms and conditions of the policy. In case of discrepancies between the English and Chinese versions of this brochure, the English version shall prevail. 3. Usual, customary and reasonable refers to (a) in relation to a fee, charge or an expense, means a charge for medical care that does not exceed the prevailing charges for a service or supply in the same geographic area, by those of similar professional standing where the charge is incurred for similar treatment, services or supplies to individuals of same gender and age, for a similar medical condition (excluding charges that would not have made other than for insurance purpose). If usual and prevailing charges for a service or supply cannot be determined because of the unusual nature of the service or supply, we shall determine to what extent the charge is reasonable, taking into account the complexity involved, degree of professional skill required and other pertinent factors such as schedule of fee indicated by government or relevant authority; (b) in relation to a hospital confinement, means confinement in hospital for a covered illness or covered injury which is medically necessary, where the admission, length of confinement, and medical services and treatments received during confinement are all in accordance with standards of good medical practice; and do not exceed the usual standard for the treatment of a similar medical condition at the location where such confinement takes place. For the avoidance of doubt, a Confinement is not usual, reasonable and customary if it is in respect of a medical procedure or treatment which, having regard to standards of good medical practice: (a) is routinely performed on other patients on an out-patient basis; and (b) could reasonably have been performed on the insured person as an out-patient. 4. Medically Necessary refers to the need of the insured person to receive medical service, procedure or supply, which in our opinion (a) is consistent with generally accepted professional standards of medical practice; and (b) is required to establish a diagnosis and to provide medical treatment consistent and necessary for medical treatment of a covered illness or covered injury and; (c) is not for the convenience of the insured person nor the physician or specialist; and (d) is not experimental in nature, screening nor preventive service; and (e) in the case of in-patient care, cannot be provided safely on an out-patient basis. 5.
