1 Management of inadequate restorative space - 咬合位置的科學基礎講員 : 陳韻之醫師記錄 : 陳亦琛醫師 Anterior repositioning splint 有可能會改變的 remodeling Liu et al., 2012 在老鼠的動物實驗中, 老鼠大腦內的神經會因為 VD 的增加而發生神經可塑性的變化 (neuroplastic change) Kato et al., 2012 提高VD 的目的主要是重建一個令人滿意美觀的假牙 VD 增加的越少, 療程的複雜性也會降低 VD 增加超過 5mm 是不常見的對於一個沒有 TMD 的病人, 使用咬合板來增加 VD 反而會產生一些症狀 Abduo et al., 2012 結論於全口重建時可能無法找出最佳與唯一的顎間關係 ( 垂直與水平 ), 因此咬合面的設計須儘量給予 freedom in centric 長時間的 trial phase 有可能造成某些不可逆的生理變化, 非必要不要改變咬合位置 Free-way space 的大小可能與決定 muscle tone 中的 contractile activity 有關 Free-way space 足夠的患者直接提高VD 的風險較低頁面 1 / 2

3 Management of inadequate restorative space - 有限咬合空間的保守贋復處理講員 : 彭炯熾醫師記錄 : 陳昱廷醫師前言 : 一位年輕女性的 46 在近心舌側牙齒結構嚴重缺損, 在傳統治療想法下會先 intentional endodontic treatment, 以 post 和 core 重建, 但是在盡量保留牙齒的想法下, 直接以 resin 重建 core, 並以 porcelain onlay 贋復處理 Treatment philosophy: 成熟 enamel 重量 96%~97% 為無機物, 其餘的為有機物和水, 而成熟 dentine 重量的 70% 為無機物, 有機物為 20%, 水為 10% 在物理性質比較下,porcelain 和 enamel 是較為接近的, 而resin 和 dentine 是類似的, 因此希望在接近自然結構下來贋復的話, 以 resin 來做為 core,porcelain 取代 enamel 要達成這樣贋復目標, 須達成 immediate dentin sealing, 達成的方法是在印模之前, 在剛切削過的 dentin, 塗上 dentinbonding agent 並 polymerization, 這麼做的理由有 : 1. 剛切削過的 dentin 有理想的結構用來 dentin bonding, 有研究說明經過 provisional cements 汙染過的表面,bonding strength 會降低 2. 在放入 composite resin 之前先 curing dentinbonding agent, 比起兩者同時 curing, 會增加 bonding strength 頁面 1 / 2

4 3. Immediate dentin sealing 會讓 dentin bond 在無受壓的情況下 (indirect restoration) 有時間增加強度, 有研究顯示在一周之後 bond strength 會增加 4. Immediate dentin sealing 可保護 dentin 在臨時贋復的期間免於 bacterial leakage and sensitivity 而在垂直咬合贋復空間不夠的情況下 : 1. 在 anteriors 和 premolars 要有 3mm OC dimension, 在 molars 要有 4mm OC dimension 2. 而molar intrusion 的量在研究中平均可達 2mm 左右, 但並不會增加 attachment level 的量牙齒嚴重磨耗的病人, 重建的 three-step technique 頁面 2 / 2

5 Management of inadequate restorative space - 過與不及的痛 : 前牙近遠心之間的考量講員 : 蕭仰嶔醫師記錄 : 許丹菁醫師 Excessive spacing or crowding of the anterior dentition 造成前牙贋復空間不理想的原因 : 1. 發育上的異常 : 先天性缺牙 peg lateral incisor microdontia 咬合不正 ( spacing, crowding, and malposition) 2. 牙齒喪失了一段時間, 造成鄰近牙齒的位移 3. 牙齒外傷 (Traumatic injuries) 4. 其他原因 : 不適當的填補物齟齒齒列磨耗或酸蝕牙齒斷裂或是因牙周問題造成的牙齒移動 (migration) Tooth analysis 分析前牙美觀的因素大致上可以分成以下幾類 :1, 2 1. Type, Color, Texture 2. Form and Contour 3. Size and Proportion 4. Incisal margin and Buccal profile! 而其中使上顎前牙看起來美觀及和諧, 影響最大因素是上顎的正中門齒因此在決定前牙美觀區的考量時, 最應該從上顎正中門牙開始評估起 (Pascal Magne et al. J Prosthet Dent 2003;89: ) Maxillary Central Incisal Dimension3 1. 上顎的正中門齒長度約為 mm, 寬度約為 8.3~9.3mm 2. 在統計資料中顯示 : 頁面 1 / 3

6 1. 前牙的大小及比例, 會依年紀性別種族牙齒磨耗程度有無 parafunction 飲食習慣和環境因素而有所不同 4, 5 2. 男性比女性的牙齒大小平均較高 3. 但女性前牙牙齒的長寬比, 會略高於男性 ( 也就是說看起來較寬一些 ) 4. 前牙平均的長寬比例 5. [Man] Central: lateral: canine= 0.85:0.76: [Woman] Central: lateral: canine=0.86: 0.79: 前牙的黃金比例 6: 指的是從正面觀看到的前牙寬度比, 理想上正中門牙 : 側門牙 : 犬齒 = 1.618: 1: 以上統計顯示, 平均的牙齒長度並不等於理想中的黃金比例, 即使將人的牙齒大小調整到黃金比例也未必是比較好看的因此, 使前牙看起來美觀和諧的原因有很多, 大小並不是一個絕對的因素, 黃金比例只是一個參考值, 更重要的是加上其他因素的整體考量 (Sterett JD et al, J Clin Periodontal 1999; 26: ) Procedure of alteration 所以有一些方法可以改變前牙近遠心空間分配不理想的問題 7 1. Prosthetic: crown contour modification, illusory perception 8 可以改變假牙的外觀如 incisal edge 長度 line angle cervical/ buccal contour, 或是經由牙齒顏色的改變造成視覺上牙齒較大或較小的錯覺 2. Prosthetically compromised restorations (establish realistic treatment objective) 要確定病人的 motivation, finance, 及對前牙美觀的期待, 一起考量或討論出一個較理想的治療目標, 然後擬定一個完善適合的治療計畫 3. Surgery: Crown lengthening, APF, root coverage 若是因為軟組織造成不美觀的原因, 很難經由一般假牙贗復來處理, 通常都要經由手術方式來獲得改善 4. Orthodontic: Intrusion, Extrusion, Axis correction, space redistribution 若是合併有口內齒列不整的問題, 經由矯正治療後, 不但可以改善美觀問題, 包括咀嚼功能和牙周健康也可以同時改善 5. Alternative option: Snap on 其他的像是之前有上市的產品 ( 微笑貝齒 ), 但有它產品使用的 indication, 並不是所有的 case 都適用頁面 2 / 3

Bruxism: 可分為生理性與病理性病理性又可分為環境因素 : 如游泳選手因長期處在含氯的游泳池, 可能會造成牙齒酸蝕嚼食菸草或檳榔也會牙齒磨耗酸性食物如柑橘類水果與汽水也會對牙齒造成酸蝕 3. 不理想的補綴物 : 當側方或前突運動出現干擾時, 牙齒磨損位置會改變, 容易造成前牙磨損如上顎前牙顎側面應為凹面, 但牙技師常做為平面或凸面造成下顎前牙過度磨耗又或者假牙沒有仔細 polish 使咬合面過於粗糙, 以會造成對咬牙的磨損 4 種牙齒磨耗型態 Attrition: 牙齒與牙齒的磨耗, 無論是正常咬合或是 parafunction Abration: 物質與牙齒之間的磨耗, 如含有顆粒的牙膏或牙刷 Erosion: 因化學性物質造成牙齒酸蝕 Abfraction: 因咬合問題形成牙齒磨損, 如咬合力過大或喜歡咬硬食物等有時牙齒磨耗對病人外觀會產生影響, 如在嚴重磨耗病人身上可看到臉型輪廓改變較薄的嘴唇與較窄的 vermillion border 等牙齒磨耗在口內也會產生變化磨耗型態為 mechanical wear 時, 則牙齒與 amalgam 或 composite resin 的磨耗速率為趨於一致, 此外牙齒上可以看到許多頁面 1 / 4

9 中華民國贋復牙科學會第 70 次牙醫師繼續教育邊緣銳利的 wear facets 由於是屬於長期慢性磨耗, 病人並不會有敏感的症狀磨耗型態如果為 chemical wear 時, 則 amalgam 或 composite resin 因為不會被酸蝕, 所以會較牙齒突出而形成 amalgam island, 此外 enamel 磨穿後因 dentin 較軟所以會呈現杯狀凹陷, 此類患者牙齒通常有 hypersentive 的症狀磨耗型式雖然有許多種, 但在病人口內可能同時會有不同型式磨耗同時發生, 但是藉由了解各種磨耗型式產生的牙齒改變會有助於牙醫師擬定治療計畫例如 : 1. 上顎前牙鄂側有斜面式的磨耗時, 通常是因為 loss of posterior support 2. 若後牙與前牙都有全面性磨耗, 則患者大都有磨牙的習慣, 而且在做前突或側方運動可以找到相對應的磨耗面 3. 磨耗面出現在單面時, 特別容易出現在小臼齒與犬齒通常是因為刷牙習慣不良或使用顆粒型牙粉有關, 又或者有咬鉛筆髮夾或菸斗等習慣 4. 發生在上顎前牙舌側靠近 cervical area 可能是因為病人有嘔吐的習慣, 如酗酒或厭食症造成催吐, 又或有胃食道逆流引起胃酸逆流這兩者對牙齒會有酸蝕性磨耗 5. 發生在上顎前牙唇側, 通常病人有習慣性吃柑橘類等酸性物質病例分享 C.C: 我想要全口重建, 因為全口牙齒已經磨耗 O.E: 微笑時上顎前牙目前為 reverse smile line, 微笑時不美觀希望可以採取! 較保守性治療, 磨越少牙齒越好! 病人有胃食道逆流病史, 上顎前牙顎側明顯看到有 surface loss, 而後牙! 區是咬合面出現磨耗現象造成咬合高度減少, 下顎前牙外觀上並無明顯! 改變 X 光片 : 牙周健康尚可, 並無明顯牙周問題病因 : 單純磨耗造成牙齒外觀不佳治療計畫 : 上顎前牙以 crown 改善牙齒長度與外觀, 小臼齒區以 bonded! restoration 建立高度, 下顎臼齒區考慮咬合力建議以 gold crown 復型頁面 2 / 4

10 前牙美觀分析 : Gingival line: 標準 high-low-high, 因此參考 lip line 即可決定牙! 齒長度中華民國贋復牙科學會第 70 次牙醫師繼續教育步驟 : 1. Mock-up for the smile design: 把 diagnostic wax-up transferred 到病人口內, 有助於讓病人了解補綴物的結果, 也便於醫師與患者做溝通 2. Provisionals: 讓患者可以適應咬合與發音等 3. X-arch mounting allows the information to be transferred -- 當在臨床上病人已經適應 provisional restoration 的 VDO anterior guidance 與 estheitc 時, 可利用X-arch mounting technique 把這些資料盡可能轉移讓技工可以做出與 provisional restoration 一樣的 final restoration -- 作法 : 須取 4 個咬合紀錄, 第一個咬合紀錄是病人配戴上下顎 provisional restoration 時, 第二個咬合紀錄是上顎為 master cast 下顎為配戴 provisional restoration, 第三個咬合紀錄是上下顎皆為 master cast, 第四個咬合記錄上顎配戴 provisional restoration 下顎為 mater cast 製作順序如下: 第一步 : 將帶有 provisional restoration 的上顎模型以 face-bow transfer 到咬合器上, 再以第一個咬合紀錄 mounting 下顎 provisional restoration cast 第二步 : 以第二個咬合紀錄 mounting 上顎 master cast 與下顎 provisional restoration cast 第三步 : 以第三個咬合紀錄 mounting 上下顎 master cast 第四步 : 以第四個咬合紀錄檢查之前所取得的咬合紀錄與 cast 所 mounting 的位置正確與否如此一來 4 個 casts 可以在咬合器上互換當製作下顎補綴物時, 可以有上顎 provision restoration cast 當作 index 可參考製作上顎補綴物時, 可以有下顎 provision restoration cast 當作 index 供參考以這種方式製作可以讓原本的 provisional restoration 盡可能忠實轉移到 final restoration, 讓 final restoration 頁面 3 / 4

11 中華民國贋復牙科學會第 70 次牙醫師繼續教育與原本設計好的 provisionals restoration 在牙齒大小美觀排列與 anterior guidance 等忠實複製進行全口重建時... 在進行全口重建時, 牙齒磨耗過大時可能會有 VDO 有 loss 的情況, 所以治療時須適時增加 VDO 一般增加 VDO 的通常是 arbitrary, 因為 VDO 為一範圍並非一個固定值, 所以可參考美觀 freeway space 等在假牙醫師的立場, 在美觀與功能許可下增加量越少越好要提高VDO 要先了解決定 VDO 的條件是甚麼? 一派的說法是 masseter m. 的長度是決定 VDO 的關鍵臨床病人可以提高高度而沒有顳顎關節或肌肉疼痛的問題的原因通常有兩個 : 1. 前牙提高3mm 時, 臼齒區通常只有提高1mm, 對 masseter m. 的長度改變範圍不大 2. 提高VDO 時, 下顎位置通常是以 CR 為起始點, 這時 condyle 在 glenoid fossa 的最上最前位置, 所以 masseter m 的長度是縮短的基於以上兩點, 雖然有增加 VDO 但是因為範圍不大, 肌肉改變長度不多, 所以患者大都無症狀在增加 VDO 時, 患者牙齒的 horizontal overlap 會加大所以如果患者原本的 horizontal overlap 就是偏大可能較不適宜增加 VDO 全口重建的補綴物咬合調整注意事項有三 : 1. Bilateral even occlusal contact 2. 前突與側方運動時前牙接觸但後牙要分開 3. 前突運動時不要影響病人原有的 envelop movement 做全口重建的時建議使用半調節咬合器, 可以模擬補綴物在口內的前突與側方運動路徑, 而且補綴物的高度也與口內所設定的 VDO 較為接近最後, 當補綴物完成後要製作咬合板, 保護病人關節並防止補綴物的磨耗頁面 4 / 4

12 Management of inadequate restorative space - 針對咬合空間不足及垂直高度喪失病人的全口贋復重建講員 : 劉閔結醫師記錄 : 黃士哲醫師 1.Turner's tooth wear classification Category 1: loss VD, excessive wear removable splint, 提高VD 6~8 周, 注意有無 muscle pain, 再給予 provisional2~3 個月再進行final 治療 Category 2: tooth wearing, without loss VD combine crown lengthening, Fixed prosthesis Category 3: tooth wearing, loss VD but no space, 無法提高VD, combine ortho., or surgical crown lengthening 2.mandibular plane angle FMA < 28.9 度 FMA > 28.9 度 Low angle, high biting force, 通常需要合併 relax muscle high angle, low biting force, 通常沒有 canine guidance 3.Chewing pattern Grinder: Class II division I, Class III 習慣咬在一個比較前突的位置, 取 bite 時要特別注意 bite 位置 ( 病人習慣向前咬 ). 病人不只想要一 freedom eccentric, 需要 freedom everywhere 頁面 1 / 3

13 Chopper : Class III, Class II division II, deep bite p't 比較沒有 freedom, 力量在後牙通常很大, 咬力大 4.Tooth elongation treatment: mm 以內 : enamel plasty mm 以上 : endo +crown 3. 5mm 以上 : ortho. tx. +crown 5. 遇到 FMH 建議使用 Cadiax Axiograph--> tracing condyle guidance 6.Facebow transfer: 基本上只是拿來參考咬合面水平的位置 1. 如果 pt 耳朵歪的, 可以自己轉到一個將來想要成型的位置 2. Occlusal plane use arcon type: 可以透過咬合器偷偷調整升 VD use non arcon type: 在口內咬一個提高的 bite 在轉移到咬合器上 ( 直接 mount 高的位置!!) 7. Treatment course 1. Mounting cast at CR position, 升高VD 後 2. 決定前牙incisal edge 3. anterior guidance 發音 esthetic.. 頁面 2 / 3

14 決定 occlusal plane: Broadrick s occlusal plane analyser 4. Occlusal indices 一組 diagnostic wax up, 一組原先的 cast 先在前牙做一個 increase VD index, 然後 prepare 後牙, 就有一個 guide, 不會 over prepare 後牙prepare 好後,take bite, 再轉到前牙換前牙prepare 後牙temp --> 前牙temp 之後開始 follow 有無 wearing 或是 muscle pain, biting force 等等 5. Final impression technique 一定在前一個 appointment 做好相當好的 preparation, 當次真的只做 impression 前一次要做到相當好的 temp fitting, 印模當次絕對不要碰到珍貴的牙齦 6. 倒兩個 master cast, 一個照樣切 die, 另一個在 margin 上面放蠟, 再灌一個, 可以拿來測 proximal contact 8. Case low angle --> heavy bite force 升高的 VD 要怎樣讓上下分配? 1. 理想的 occlusal plane 在哪裡決定怎麼分配因為在這個 case, upper lingual side cusp 比較磨耗多, 所以 upper 給 2mm 下面給 1mm 在一般的 case 中通常都平均分配 2. Study model 先 mounting 在一個提高後 VD 的位置決定 incisal edge, 下顎前牙先有個高的 bite, prepare 後牙, 咬後牙index, prepare 前牙, reline 前牙index--> 全套 temp 印模 cross mounting 頁面 3 / 3
