1 Funded by The Community Investment & Inclusion Fund Organized by Tsing Yi Integrated Service Centre Duration September 2013 to August 2016 Service Target Women with childbearing and child rearing experiences, expectant parents and new parents Project Goals and Highlights There were many low income and new arrival new born babies families who had weak problem-solving abilities and social networks. The aims of this project are: 1. The community empowerment We recruited the women with childbearing and child rearing experiences to participate in the Aunties trainings, empower them in order to help the new parents. They can share their experiences, problem-solving skills, parenting tips and also the communication skills with family members in order to build up the community capacity. 2. The promotion of the spirit of mutual help and caring Women who completed the Aunties trainings will be paired with the new parents as a companion. The aunties will help the new parents to solve the problems arising due to childcare, establish interpersonal and emotional support network for them to tide over the change in their lives. 3. The conversion of recipients into helpers After the new parents received the services for helping them to spend their pregnancy and postpartum adjustment period, we encouraged them turn into helpers to share their experiences to another new parents. As a result, the mutual helps and the care can keep on growing to help the families in need in the community. Project Content and Progress 1. Aunties trainings Invited the women with childbearing and child rearing experiences to participate the trainings from different professional parties, e.g. prenatal and postnatal care, baby care, communication skills within families, in order to help the new parents. The women also set up different groups for sharing experiences and building up their support network. 78

Prenatal and postnatal support services The women would act like a caring ambassador to support the postpartum mothers. Through the telephone contact, home visits and other cares or supports to the mothers, the mutual help network of community was established. 4. Other volunteer support network Small group founded by the women according to their different expertise, such as sewing team, crocheted team, diet group and also photographic group. The volunteers can develop their talents in order to show their care to the participated families. 5. Events Different gatherings would be regularly held, such as birthday parties, family photography activities, hundred day s banquets. The new parents can share their parenting experiences and the difficulties with each other. "Angel Aunties" participated in different training or support group, e.g. crocheted team, for supporting the new born baby parents. "Angel Aunties" and project colleague visited the new born baby family and supported their needs. New born baby parents can learn more about the knowledge of prenatal and postnatal care, and the skills of baby care. 79

3 Funded by the Henderson Land Group Organized by Industrial and Commercial Social Service Unit Duration June to August 2014 Service Target Youths aged between 15 and 29 (Secondary 4 or above students or those in the early stages of their career) Project Goals and Highlights The objective of the scheme was to provide workshop and training for youth as enrichment by arranging suitable participants to work in actual working environment. Our Enterprises partner - Henderson Land Group, arranged senior staff to introduce property management, including company culture, the entry requirement and career path. Also, internship vacancy was provided for youth gaining precious experience through working in the actual working environment. Project Content and Progress The content of the scheme was soft skill training, practical skill training, corporate visit and internships, assisting the teenagers to establish a sense of occupation orientation, as well as to enhance their competiveness. 1. Practical and soft skills training: Through the soft skills training, participants learned how to adapt to the working environment. The Security and Guarding Services Industry Authority Quality Assurance System Compliance and Fire Safety Ambassador Appoint Certifi cate was for enhancing their familiarity with the property management knowledge. Attainment of the certifi cates further equiped themselves for their career path. 2. Corporate visit: visit The Mira Hong Kong, Miramar Shopping Centre and The Hong Kong Shipyard Ltd in Tsing Yi. Participants were concentrating on the briefi ng regarding the daily operation. 80

Early Detection and Intervention 1. Finding out the hidden elders with mild cognitive impairment (MCI) or early stage dementia as well as their caregivers in the community. 2. Providing counselling and support service to elders and caregivers like medical follow up and continuous cognitive training. 3. Delivering home-based living guidance service according to different cognitive levels of elders, improving their quality of life and relieving caregivers pressure. Medical-Welfare-Family-Social Collaboration 1. Exhibiting the cooperation network between Kwai Chung Hospital (KCH) Psychogeriatric Team, elderly centres and home care services subsidized by Social Welfare Department. 2. Developing neighborhood network to monitor the condition of elders. 3. Encouraging collaboration between medical and social professionals as well as families to exchange their information about the elders. Flexible and Down-to-earth Training Well-trained volunteers provide cognitive training and home-based living guidance according to different cognitive levels, personality, living habit and home environment of elders. 82

6 Project Content and Progress Forget Me Not demonstrates the power of collaboration between DECC and KCH in digging out those hidden elders with cognitive impairment problem, as well as to identify and support their caregivers by means of providing them guidance to face their problems due to cognitive deterioration, delivering early intervention, relieving pressure of both elders and caregivers. The Home-based Living Guidance Training Kit developed by the project makes professional cognitive training easyto-understand and easy-to-use. Trained middle-aged volunteers can use the kits, supporting by professionals at behind, to help elders with cognitive problem to improve their quality of life. Our project believes in Understand Cures, Realizing Knowledge by Action, Multiple outcome by Multiple- Disciplinary Collaboration and People facing problem have their solution. So we use the following work methods: 1. Screening and differentiating elders with different needs - We launch public education programs in the community and use common cognitive assessment tools to identify elders with cognitive problem to receive further medical follow up service from KCH. 2. Using different mode of services regarding to different conditions - We have both centre-based and homebased cognitive training programs (e.g. play groups, volunteer groups, etc.) that are suitable for elders with different cognitive levels and different preferences. For those who have no cognitive problem yet are invited to become our volunteers. 3. Home-based living guidance service and volunteers - Adopting foreign experiences and concept of Montessori Teaching Method, our project develops with KCH a Home-based living guidance training kit, which is tailor made for elders in Hong Kong. We trained about 60 volunteers by 3 phrases to use this kit to execute cognitive training with elders. Home Living instructors learned how to choose suitable cognitive training tools to elderly with different cognitive impairment levels. The project focused more on early identifi cation and thus preventive education and simple assessment activities were held in the community periodically. 83

7 4. Caregivers participation and learning - The project team communicates well with elders and their caregivers to know their daily life changes and arrange relevant follow ups. During home visits by volunteers, caregivers can also learn the techniques of cognitive training, so that they can train their elders by themselves in the future. Community screening identified early dementia elders and referred them to KCH for immediate follow up, seizing the golden phase for intervention to dementia. The project made cognitive training methods used by medical professionals down-to-earth and user-friendly. Volunteers and caregivers can systematically and continuously use them to train the elders in their familiar home environment, improving their cognitive level. This intervention model reduced the reliance of caregivers to others and relieving their sense of helplessness. Statistics collected by the project revealed that about 70% of elders have improvement in cognitive level and 90% of elders have improvement in ADL and IADL after receiving our services. Elderly had cognitive training at home. Our staffs, volunteer and strategic partners reported the outstanding achievements of the project to Community Chest. 84

8 Stroke patients were receiving rehabilitation training in the community under the supervision of our health care worker of stroke rehab specialist. Organized by Dr. Lam Chik Suen District Elderly Community Centre Duration All year round Service Target Stroke elderly and their caregivers Project Goals and Highlights Uncover, Discover, Recover, a holistic rehabilitation process, was the theoretical framework leading the project development of Flyway. To assist those stroke elderly with different rehabilitation stages to take advantage of the golden period of rehabilitation, 3 strategic interventions namely strength-oriented, community network, barrier-free community rehabilitation was employed to exert the strengths of stroke elderly, caregivers, neighbor volunteers, medical and social work professionals. The Flyway focused more on psychological rehabilitation to encourage stroke participants to rebuild their social support network and share their unique rehabilitation processes at the nearby mutual support platform. Project Content and Progress Community-oriented was the working model of Project Flyway which lined up different stakeholders and professionals by integrating formal and informal supports to assist those stroke elderly and their caregivers. These home-based and community-based rehabilitation programs were included as follow: 1. Take advantage of golden rehabilitation period by integrating the home care and rehabilitation services A promotional channel and referral system such as periodical talks at the Day hospital were already built-in in this project. Those stroke elderly can prepare themselves to join the seamless self-fi nanced community rehabilitation program named Stroke 60-Day: Home Rehabilitation Program before the completion of hospital treatment. Health care workers of stroke rehab specialist were trained to execute this self-financed program. To take advantages of golden rehabilitation, those patients whom without rehabilitation training in the hospital were assisted to keep rehabilitation training at home. Clinical data evidenced that patients had confidence to live at home continuously because the improvement of the functional mobility, self-care ability and balance. Ling Hong Exercise instructor trainings were held periodically to support the operations of the 7 localized platforms. 85

9 86 Publics were keen in participating the Orange Ribbon Action every year to encourage those stroke patients to have a positive rehabilitation. 2. Localized stroke rehabilitation training platform of Ling Hong Exercise Ling Hong Exercise, a flagship program of Project Flyway, was a rehabilitation exercise that can strengthen muscle of stroke elderly and be executed by trained volunteer instructors. Opportunities of receiving services and social life were being restricted under the problems of poor mobility and transportation difficulties. This project, therefore, developed 7 localized Ling Hong Exercise training platforms in Tsuen Wan and Kwai Tsing districts to facilitate those stroke elderly received weekly gatherings and rehabilitation trainings at their nearby residence. 3. Localized mutual support organization and social network of stroke elderly Ling Hong Exercise was executed in a group setting. Stroke elderly were not only merely trained up muscle but also had mutual support among patients and volunteers at the platform. This community network and mutual support platform became another important intervention element of this project. Volunteers not merely took up a role of information givers and emotion supporters but also linked up stroke rehabilitators of different stages and their caregivers to encourage them to have a role change to care with other newly stroke elderly. The Nursing Department of Hong Kong University conducted a focus group for the caregivers of these stroke elderly. It was found that the caring stresses of those caregivers were released under this designated service design and peer supports. These community networks and mutual support platforms advocated those caregivers and volunteers to form and registered as 4 independent organizations named Flyway mutual support organization to prepare for further development of stroke. 4. Developed a package to breakthrough barriers of geographical restrictions and communications A web page ( for stroke rehabilitation was set up. To achieve the goal of barriers-free for stroke rehabilitation, Stroke 5 Chinese Input method, was promoted to those stroke elderly whom were suitable to use computer and smart phone so as to facilitate them to equip their knowledge of rehabilitation and community resources without any barriers of geographical restrictions. 5. A territory-wide mobilization: The World Stroke Day (Hong Kong) Orange Ribbon Action To arouse public concerns on the rehabilitation needs of stroke elderly, the DECC organized the 1st World Stroke Day(Hong Kong) Orange Ribbon Action in From the period of 2013 to 2015, enterprises and volunteer organizations were participated different community programs such as car parade and kite fl ying in the Orange Ribbon Action. Millions of citizens were wearing the Orange Ribbon to show their concerns on stroke diseases on the World Stroke Day during the past two years.

Lady MacLehose Community Pharmacy Duration Since June 2014 till now Service Target People from the Community Project Goals and Highlights Atrial fibrillation is one of the most common diseases in Arrhythmia. Getting older, high blood pressure, diabetes and cardiovascular disease are the high risk factors causing atrial fibrillation. Around one-fourth of the populations over 40-years-old may suffer from such disease before death. The occurence of atrial fi brillation raises up to 8.6% when we are aging 65. The chance of getting stroke for those who are suffering from atrial fi brillation is of 5-7 times higher than the normal person, and resulting in more serious harmful effects than other types of stroke. This leads to irreversible consequence and creates long-term burdens to the patients, their family and the society. Project Content and Progress With the support of business sector, we had launched a press conference in May 2014 so as to promote this project to the community. Having widely reported by the mass media, the project started up successfully. By using the mobile monitoring devices, only thirty seconds is needed for completing a single-lead electrocardiogram which would then be sent to our project consultant, a cardiology specialist, Dr. Chan Ngai Yin for further medical diagnosis. Those who were diagnosed as suffering from atrial fibrillation, referral letters would be given to them at the same time so as to encourage him for medical follow up and better control of their health condition. The design of the mobile monitoring device is similar to a smart phone case. Participants simply put their fi ngers on the sensor at the back of the case, then the rhythm of the heartbeat will be recorded by the mobile app of the tablet or smart phone with an accuracy of 99%. When compared with the traditional ECG, this new technology is more convenient and faster which is more suitable for community health promotion programs. Our project consultant, a cardiology specialist, Dr. Chan Ngai Yin, was holding a health talk related to prevention of atrial fi brillation in a elderly centre. 87

11 Atrial Fibrillation Community ECG Screening Program (ACEP) keeps on implementing in Tsuen Wan, Kwai Chung and Tsing Yi. Apart from processing the singlelead electrocardiogram, we also provide other body check services, including height measurement, weight measurement, blood pressure measurement, body fat analysis, and etc. Our registered pharmacist was stand-by for medication counseling and answering questions arised so that the participants can have more understanding towards their health condition. Besides, different forms of community health education programs were launched, like educational talks, health information exhibitions and educational leaflets distribution, so as to enhance the knowledge of general public on cardiovascular disease, especially atrial fibrillation; at the same time, identify the high-risk patients and make suitable referral for medical follow-up. Deprived ones would then be referred for the Drug Subsidy Program organised by H.K.S.K.H. Lady MacLehose Community Pharmacy to buy drugs at a lower price, in turn reducing the threats of stroke caused by atrial fi brillation. Over the past ten months, we had held the program in 8 public estates and malls and cooperated with 14 district organizations. Over 2,600 elderly and community residents were served in which 51 participants were diagnosed as suffering atrial fibrillation. They were then referred to governmental healthcare system for followup. In the future, we will continue putting efforts on connecting more social service units so as to raise the public awareness towards cardiovascular disease in Tsuen Wan and Kwai Tsing. It is hoped that more potential patients were identifi ed, in a long run, reducing the risk of stroke. Having widely reported by the mass media, the project initiated the public awareness of the risk of Atrial Fibrillation and Cerebral Vascular Accident. By using mobile monitoring device, the single-lead electrocardiogram(ecg) test can detect the rhythm of the heartbeat from participants in 30 seconds. The accuracy of the result reaches to 99%. The participant was conducting body check tests, evaluating the risk factors of having cardiovascular disease. 88

12 Funded by Mr. Frankin Wai, Mrs. Betty Wai and Miss Christinnie Yan Organized by Gospel Service Unit Duration September 2014 Service Target Grassroots families (residents from sub-divided units) Project Goals and Highlights This activity has used the approach of Outreaching Supportive Community Gospel Cooperative Work. It is to integrate the religious values into Holistic Care, Holistic Health social service approach toward cooperation with the community, church and individual Christian. To assist elderly, adult and young people in local families towards positive life values and relationship via the aspects of physical, mental, social and spiritual health development. Gospel Service Unit has successfully contacted Mrs. Betty Wai, a passionate Christian. By targeting the sectors of grassroots families as the aim of the activity, our unit has held an event called "Big & Small Footprints" LinkAges Community Gospel Activity to respond to community ministry. Hoping that through the tour, visit, dining and sharing in this event, it could encourage and inspire the motivation of grassroots students and their families. Furthermore, they would have a memorable moment as well as a chance to listen the gospel. Project Content and Progress Families were scheduled to visits Art Museum of the Chinese University of Hong Kong and the University Hall on that day first and left "small footprints". Via the exhibits and sharing by the tour guides, parents had an opportunity to encourage their children to study hard under warm and comfortable scenes. After that, all participants were transferred to a hotel in Tsuen Wan and had a lunch gathering. In addition to enjoy the meal together, the activity was also supported by The Rev. Wong Kin Sun from "Sheng Kung Hui Church of the Divine Love " and Christian artist Hiromi Wada. They shared the biblical messages and gospel to the participants. This activity provided a resting place for the stressed parents who always focus on caring their children - "big footprints" living in grassroots to relax. Also, the whole activity was encouraging and cheerful. Children and their families could experience the Love and Care of God. Extemporaneous songs from our guest Hiromi Wada and lucky-draw put the event to its ending. 89

Proverbs 16:9 The event brought joyfulness to grassroots family members and delivered the gospel to them as well as God's Love and Care with lunch gathering in "Big &Small Footprints" LinkAges Community Gospel Activity. The whole activity was encouraging and cheerful and children and their families could experience the Love and Care of God with the extemporaneous songs from our guest Hiromi Wada. In "Big &Small Footprints" LinkAges Community Gospel Activity,families visited the Chinese University of Hong Kong and the University Hall. 90
