2 Registration Information 1. The Enrichment Programme for second half year (Jun Oct 2018) consists of 12 lessons (Pri 6 to Sec 5) or 16 lessons (K1 to Pri 5) unless otherwise stated. 2. Programmes are open to Singapore Citizens and Singapore Permanent Residents. Interested students have to register as members of CDAC@ Tanjong Katong in order to register for the programmes. Please bring along the birth certificate/ NRIC to register as our member. Membership is free. 3. Please ensure that the date and time of the programmes that your child/children intend to enrol in do not clash with any other programmes. Withdrawal from one of the clashed programmes must be done if it is discovered after registration. If not, CDAC reserves the rights to claim the full programme fees if a participant is absent for the programmes due to the clash. CDAC also reserves the rights not to refund in the event of a clash in programmes after registration. 4. Phase Date Criterion Remarks Priority 09/04/ /05/2018 (Mon ) For the class(es) (Lesson 9 to 16) that the students are currently enrolled in Register with Payment Notice Phase 1 20/05/ /05/2018 (- Sat ) For student members who are currently on CDAC Activity Fee Subsidy Scheme Vacancies will be allocated on a firstcome-first served basis Please bring along the CDAC Activity Fee Subsidy successful notification letter Phase 2 From 27/05/2018 For all other student members Remaining vacancies available after Priority and Phase 1 Registrations on a first-come-first served basis 5. Registration closes either after the 3 rd lesson of a class commencement or when the class is full. 6. A class will only start when the minimum class size is met. 7. All CDAC Centres will be closed on 4 June 2018 as centre staff will be attending a full day training. 8. Please bring along your child s Student Ez-Link Card. Parents are only allowed to register for your own child/children. 9. Registration will close at 9.30pm on Monday to Friday and 4.30pm on Saturday and day. 10. Only cash, NETS (minimum payable by NETS is $10) and cheque payment is accepted at the point of registration. Cheque should be made payable to CDAC. 11. Each payment is for 6/ 8 lessons. Collection of payment for Lesson 23 to 28/ 25 to 32 will start on the third or fourth lesson of Module 3. Payment notification slips will be given to existing students to make payment within a timeframe. If payment is not made within the timeframe, it is deemed that the student has decided to withdraw from the programme. Vacancies will be open to new students for applications after the payment deadline. 12. Book and computer loaning services will not be available on the first day of registration for both phases. 13. The programme outline is available at CDAC@ Tanjong Katong. Alternatively, you can download the module outline from the CDAC website ( 14. There are no lessons on 9/8 (National Day) and 22/8 (Hari Raya Haji). 15. CDAC reserves the rights to cancel or amend the programme schedule, venue and teaching staff, if and when necessary. 16. CDAC is not liable to give reminders regarding the commencement of classes. Parents are requested to note down the commencement dates. 17. Topics covered may not follow the school syllabus as every school has its autonomy to decide the sequence of the topics taught.

3 报名须知 1. 除非另外声明, 下学期的益智课程 (2018 年 6 月至 10 月 ) 共有 12 堂课 ( 小六至中五 ) 或 16 堂课 ( 幼稚园一年级至小五 ) 2. 活动只开放给新加坡公民和新加坡永久居民丹戎加东的会员以参加课程请携带学生的出生证 / 居民证副本以免费注册成为会员 3. 请在报名活动前确保孩子所要报名的活动在日期或时间上没有与其他的活动有任何冲突 4. 如果在报名以后发现所报名的不同活动在日期或时间上有所冲突, 则必须通知中心并退出其中一项活动如果因为活动日期和时间有所冲突而缺席任何课程, 华助会将有权追讨课程的全部费用此外, 如果因为活动日期和时间上有所冲突而必须退出, 华助会将不退款报名阶段日期条件备注优先报名 09/04/ /05/2018 ( 星期一至 ) 第一阶段 20/05/ /05/2018 ( 至星期六 ) 5. 课程将在每个学段的第 3 堂课之后或在课程名额已满时停止接受报名 6. 每个班级都需达到指定人数才能开班 7. 由于中心工作人员将参与一天的培训课程, 华助会属下的 12 所中心将在 2018 年 6 月 4 日关闭一天 8. 报名当天, 请记得携带学生证 ( 易通卡 ) 家长只能为自己的孩子报名 9. 华助会中心将在晚上 9 点 30 分 ( 星期一至五 ) 和下午 4 点 30 分 ( 星期六和日 ) 停止接受报名 10. 我们接受现金 NETS 或支票付款请在报名时支付所有的活动费如以支票付款, 请注明收款人为 CDAC 如以 NETS 付款, 最低金额为 10 元 11. 我们将每一次收取 6 或 8 堂课的学费第 23 至 28 堂课或 25 至 32 堂课的学费将在第 3 或 4 堂课后开始收取在上课的学生将会收到付款通知书学费必须在时限内缴交如果没缴交学费, 将视为退出课程剩下的学额将在付款期限后, 开放给其他的学生 12. 在两个报名阶段的第一天, 我们将不会提供借书与租用电脑的服务 13. 丹戎加东询问课程的内容, 或到华助会网址 ( 下载课程的内容 14. 9/8( 国庆日 ) 和 22/8( 哈芝节 ) 无课已报读第 9-16 堂课的学生可优先报名进入原来的班级已成功申请华助会学生活动费津贴的会员 15. 华助会有权删改活动时间, 地点或更换指导员 16. 华助会将不会提醒家长开课日期请家长留意开课日期请携带报名通知书到中心报名限定的名额将以先到先得的方式分配报名时, 请携带成功申请华助会学生活动费津贴通知书第二阶段从 27/05/2018 起其他会员优先报名和第一阶段报名之后的剩余名额将以先到先得的方式分配 17. 由于各所学校可制定针对学生的教学内容, 华助会益智课程的教学纲要或会与学校不同
