5 Eligibility Requirements Those who meet one of the following qualifications are eligible to apply for Doctoral Course. (1) Have graduated or will graduate from a university program in medicine, dentistry, pharmaceutical sciences (limited to those whose minimum duration of study is six years), or veterinary medicine by March 31, (2) Have completed or will complete 18 years of formal education in a foreign country by March 31, (3) Have completed or will complete 18 years of formal education of a foreign country in Japan by taking courses offered by a school of said foreign country by distance education by March 31, (4) Have completed or will complete a course of study in Japan at an educational institution positioned under the schooling system of a foreign country as offering university programs (limited to those where persons who have completed the program are treated as having completed 18 years of schooling of said foreign country) and designated separately by the Minister of Education, Culture, Sports, Science and Technology by March 31, (5) Have been conferred or will confer a degree equivalent to a bachelor's degree through attending an overseas university or other overseas school (limited to those whose education and research activities have been evaluated by persons who have been certified by the relevant country's government or a related institution, or have been separately designated by the Ministry of Education, Culture, Sports, Science and Technology as being equivalent to such) and graduated from a program that requires 5 or more years to complete (Includes graduating from a program implemented by the relevant overseas school while living in Japan through distance learning, as well as graduating from a program implemented by an educational facility established with the relevant overseas country's school education system and has received the designation mentioned above) by March 31, (6) Have designated by the Minister of Education, Culture, Sports, Science and Technology (as prescribed by The Ministry of Education, Science, Sports and Culture Public Notice No. 39 of 1955). Those who have completed an undergraduate program in either medicine or dentistry at a university in accordance with the former University Decree (1918, Imperial Edict No. 388). Those who have graduated from National Defense College in accordance with the Ministry of Defense Establishment Law (1954, Law No. 164). Those who have completed a master's program or a professional graduate school program which stated in accordance with the School Education Act (1947, Law No. 26) Article 99, Item 2 / Those qualified to receive a master's degree, or those who have been enrolled for two years or more in a consistent five-year doctoral program which is not divided into two-year master's program and three-year doctoral program, have obtained more than 30 credits there and completed required research work by March 31, Those who have two or more subsequent years of a research experience at a university or research institution after graduated from a university (except for a veterinary medicine program at a graduate school or specialty school, or programs at a graduate school or non-degree program in medicine, dentistry or pharmacy founded in order to cultivate practical clinical ability), or after completed 16 years of education at a foreign school, and those who also have been recognized by their research achievement to have academic ability equivalent or superior to that of those who have graduated from a university in medical program, dentistry or pharmacy program designed in order to cultivate practical clinical ability. (7) Have been enrolled in a program in medicine, dentistry, pharmaceutical sciences (limited to those whose minimum duration of study is six years), or veterinary medicine at a university for at least four years or persons who have completed 16 years of schooling in a foreign country or a course of study in Japan at an educational institution positioned under the schooling system of a foreign country as offering university programs (limited to those where persons who have completed the program are 3

6 treated as having completed 16 years of schooling of said foreign country) by March 31, 2020 and designated separately by the Minister of Education, Culture, Sports, Science and Technology, who are found to have acquired the designated credits at a graduate school of the University with an excellent academic record. (8) Have been recognized by Graduate School of Medicine based on individual screening of qualifications as having equal academic abilities as a university graduate, and of an age not less than 24 by March 31, Notice 1: Those who apply by the above requirement item (6), (7) or (8) must refer to the page Preliminary Screening of Eligibility. 4

21 Application Documents The following documents should be submitted to the Admission Office of Graduate School of Medicine. Please fill out in Japanese or English and send the original forms provided in this application guidebook. (1) Application Form for Admission and Curriculum Vitae (both side printing / prescribed form / 1 copy): Applicants must decide their specialized area for research in advance. Approval by the professor or associate professor of the field is required. (2) Purpose of Application (prescribed form / 1 copy) (3) Certificate of graduation / expected graduation from the most recent graduating university (1 copy): Graduates from Nagoya University, Graduate School of Medicine do not have to submit this certificate. (4) Transcript (1 copy): It must be officially sealed by the most recent graduating university or college. (5) Admission Ticket for the examphotograph Card (prescribed form) (6) Photograph (2 copies) :Attach each photograph (4cm3cm, upper body, bareheaded, front face, taken within the last 3 months) to the Application form for Admission and Curriculum Vitae (1) and the Photograph Card (5). (7) Envelope for Reply (3 pieces): a. One to send the Admission Ticket for the Examination and One to send pass-fail notification: prescribed envelopes (small size) with clearly self-addressed, signed and stamped equivalent with 362 yen (as an express mail charge) b. One for notification of entrance procedure: prescribed envelope (33cm24cm) with clearly self-addressed, signed and stamped equivalent with 530 yen (as an express mail charge) (8) A Copy of Passport: Applicants without Japanese nationality must submit a photocopy the page of their valid passport showing the passport number and the applicant's photograph. A Copy of Residence Card: Those applicants who are currently residing in Japan must also submit a photocopy of their Residence Card, both sides. A Copy of Resident Record: This is for those applicants currently residing in Japan; This certificate must include your visa status, and is available at your local ward/city office. If you are unable to obtain Residence Card, please contact the Admission Office of Graduate School of Medicine. (9) Application Fee: 30,000 Japanese Yen is required (non-refundable). Please use a Japanese Postal Money Order with no entry in section to be paid to or. Applicants with Japanese Government Scholarships are requested to submit the certificates of receiving scholarships instead of application fee. (10) Receipt (for applicant)receipt (for office use only) (prescribed form / 1 copy): Submit with all the required items filled out. 19

23 Preliminary Screening of Eligibility 1. Candidates applying under Eligibility Requirement (6), (7), (8) must meet both of the following conditions: (1) At the time of the application, the applicant must have either graduated from a Japanese university / college or completed a course of 16 years of regular curricular studies in another country, followed by research for at least 2 years at a university or research institute. (2) The applicant needs to have published research papers or books or made research presentations recognized as the equivalent with or superior to a master s thesis. 2. Those who apply under the category of Eligibility Requirement (6), (7), (8) should ask about the eligibility at the Administration Office of Graduate School of Medicine and submit in person or send together with required documents(described bellow)by May 24, 2019(Fri)(The first time), October 25, 2019(Fri)(The second time). In case of mailing, please send by registered mail written in red ink, Application for Preliminary Screening on the front of the envelope. During the process of preliminary screening, it is possible that the applicant may be requested to interview. Result of the Preliminary Screening will be sent to the applicants by June 6,2019(Thu)(The first time), November 7, 2019(Thu)(The second time). 3.
