1 SRPO-( ) 投標須知 Notice to Tender(Second) 第二次 Re: Our Tender No. : SRPO-( ) 以下各項招標規定內容, 由本中心填寫, 投標廠商不得填寫或塗改 各項內含選項者, 由本中心擇符合本採購案者勾填 (National Synchrotron Radiation Research Center (hereinafter referred to as the NSRRC ), is a National Science Laboratory which is fully subsidized by the government of Republic of China ( ROC ) on Taiwan, herewith solicits your tender in according to the following selected items: )(The following items are selected by the NSRRC only, The tenderer(s) are not allowed to select, erase or change the wording.) 一 本採購適用政府採購法 ( 以下簡稱採購法 ) 及其主管機關 ( 行政院公共工程委員會 ) 所訂定之規定 ( This procurement is apply to the Government Procurement Act of the Republic of China on Taiwan (hereinafter referred to as the Government Procurement )and all related regulations are set forth by the responsible entity, the term responsible entity referred to in the above-mentioned Act means the Procurement and Public Construction Commission of Executive Yuan) 二 本標案名稱 (Designation of tender): 微波導波管元件一批 (waveguide components of WR1500 and WR1800) 三 採購標的為 (This objective of tender is): (1) 工程 (Construction Work ) (2) 財物 (Property) ; 其性質為 (the attribution of property is): 購買 (Purchasing); 租賃 (Lease); 定製 (make to order); 兼具兩種以上性質者 ( 請勾選 )( the objective of tender belongs to both two attributions, please select simultaneously ) (3) 勞務 (Service) 四 本採購屬 (The value of tender is): (1) 公告金額十分之一以下之採購 (less than one tenth of the threshold for publication) (2) 逾公告金額十分之一未達公告金額之採購 (more than one tenth of the threshold for publication but not reaching the threshold for publication) (3) 公告金額以上未達查核金額之採購 (more than or equal to the 1

12 Bid Bond, Guarantee Bond and Other Guarantee ) 五十二 廠商有下列情形之一者, 其所繳納之押標金, 不予發還, 其已發還者, 並予追繳 (The bid bond deposited shall not be refunded or returned to the tenderer and the refunded or returned bid bond shall be recovered if there is any of the following circumstances::( 無押標金者免列 ) (Please discard where there is no need to require bid bond) ( 一 ) 以偽造 變造之文件投標 (1.The tenderer used forged or altered documents to tender) ( 二 ) 投標廠商另行借用他人名義或證件投標 (2.The tenderer borrowed the name or certificate of another to tender) ( 三 ) 冒用他人名義或證件投標 (3.The tenderer assumed the name or certificate of another to tender) ( 四 ) 在報價有效期間內撤回其報價 (4.The tenderer withdraw its bid before expiration of the tender) ( 五 ) 開標後應得標者不接受決標或拒不簽約 (5.The tenderer refused to be awarded or refused to execute a contract after opening of tenders) ( 六 ) 得標後未於規定期限內, 繳足履約保證金或提供擔保 (6.The tenderer failed to deposite sufficient guarantee bond or to provide other guarantee sufficiently within a prescribed time-limit after being awarded) ( 七 ) 押標金轉換為履約保證金 (7.A bid bond has been served as a performance bond) ( 八 ) 其他經主管機關認定有影響採購公正之違反法令行為者 (8.The responsible entity found that there is a violation of laws and regulations which affects the fairness of the procurement) 五十三 廠商依 押標金保證金暨其他擔保作業辦法 規定減收押標金, 其有不發還押標金之情形者, 應就不發還金額中屬減收之金額補繳之 (Subject to Regualtions for Bid Bond, Guarantee Bond and Other Guarantee in relating to the reduced bid bond, if there is any circumstances that the bid bond deposited shall not be refund or returned to the tenderer, and the amount of the reduced bid bond shall be recovered by the suppliers) 五十四 本採購 (This procurement is ): (1) 訂底價, 但不公告底價 (Where a government estimate is set but shall be kept confidential) 12
