1 澳 洲 " 瑞 樂 沙 旋 達 碟 衛 署 藥 輸 字 第 號 RELENZA ROTADISKS (Zanamivir 5mg) Australia 定 性 與 定 量 組 成 每 一 片 RELENZA ROTADISK 含 有 4 個 規 則 間 隔 的 雙 面 錫 箔 泡 囊, 每 一 個 泡 囊 中 都 含 有 由 微 粒 化 之 zanamivir (5 毫 克 ) 及 乳 糖 (20 毫 克 ) 混 合 而 成 的 白 色 至 灰 白 色 粉 劑 劑 型 吸 入 用 粉 劑 臨 床 特 性 適 應 症 本 藥 須 由 醫 師 處 方 使 用 治 療 流 行 性 感 冒 RELENZA 適 用 於 治 療 及 預 防 成 人 及 兒 童 ( 5 歲 ) 之 A 型 及 B 型 流 行 性 感 冒 劑 量 與 用 法 RELENZA 僅 可 使 用 隨 藥 提 供 的 碟 型 吸 入 器 (DISKHALER ) 以 經 口 吸 入 的 方 式 投 入 呼 吸 道 對 準 備 將 其 他 吸 入 用 藥 ( 如 速 效 性 支 氣 管 擴 張 劑 ) 和 RELENZA 同 時 使 用 的 患 者, 應 建 議 其 先 使 用 該 藥 再 使 用 RELENZA 成 人 治 療 流 行 性 感 冒 RELENZA 的 建 議 劑 量 為 連 續 5 天, 每 天 兩 次, 每 次 吸 入 兩 劑 (2 5 毫 克 ), 每 天 的 總 吸 入 劑 量 為 20 毫 克 為 獲 得 最 大 效 益, 症 狀 出 現 後 應 盡 快 開 始 治 療 ( 最 好 在 兩 天 之 內 ) 預 防 性 治 療 RELENZA 的 建 議 劑 量 為 連 續 10 天, 每 天 一 次 吸 入 兩 劑 (2 5 毫 克 ), 每 天 的 總 吸 入 劑 量 為 10 毫 克 如 果 接 觸 風 險 的 時 間 超 過 10 天, 可 將 此 療 程 延 長, 最 多 可 延 長 至 1 個 月 應 依 處 方 完 成 整 個 預 防 性 治 療 的 療 程 兒 童 不 須 調 整 劑 量 ( 參 見 藥 物 動 力 學 ) 老 年 人 不 須 調 整 劑 量 ( 參 見 藥 物 動 力 學 ) 腎 功 能 損 害 者 不 須 調 整 劑 量 ( 參 見 藥 物 動 力 學 )

2 肝 功 能 損 害 者 不 須 調 整 劑 量 ( 參 見 藥 物 動 力 學 ) 禁 忌 症 對 本 製 劑 中 之 任 何 成 分 過 敏 者 ( 參 見 賦 形 劑 ) 警 語 及 注 意 事 項 感 染 流 行 性 感 冒 可 能 會 使 氣 道 的 過 敏 反 應 性 升 高 在 因 流 行 性 感 冒 而 接 受 治 療 的 患 者 中, 曾 有 在 使 用 zanamivir 後 出 現 支 氣 管 痙 攣 及 ( 或 ) 呼 吸 功 能 降 低 之 現 象 的 報 告, 但 極 為 罕 見, 其 中 部 份 病 患 過 去 並 無 任 何 呼 吸 道 疾 病 病 史 任 何 出 現 此 類 反 應 的 病 患 都 應 停 用 zanamivir, 並 就 醫 檢 查 併 有 呼 吸 道 疾 病 的 患 者 在 使 用 zanamivir 時, 應 隨 時 備 妥 速 效 性 支 氣 管 擴 張 劑 ( 參 見 劑 量 與 用 法 ) 類 似 過 敏 反 應, 包 括 口 咽 部 水 腫 嚴 重 皮 膚 疹, 曾 在 RELENZA 上 市 後 使 用 經 驗 中 報 告 過 若 發 生 過 敏 反 應 或 懷 疑 是 過 敏 反 應, 應 停 用 RELENZA 並 給 予 適 當 處 置 雖 然 流 行 性 感 冒 也 可 能 引 起 神 經 系 統 與 行 為 方 面 的 症 狀 ; 但 許 多 上 市 後 報 告 ( 大 部 分 來 自 日 本 及 兒 童 受 試 者 ) 提 到 流 行 性 感 冒 患 者 在 接 受 包 含 RELENZA 在 內 的 神 經 胺 酸 酶 抑 制 劑 (neuraminidase inhibitor) 治 療 時, 有 癲 癇 譫 妄 幻 覺 與 異 常 行 為 等 表 現 這 些 症 狀 主 要 發 生 於 疾 病 初 期, 往 往 突 然 出 現 之 後 又 迅 速 消 退 目 前 尚 未 證 實 RELENZA 與 這 些 症 狀 的 關 聯 性 如 果 出 現 神 經 精 神 方 面 的 症 狀, 應 個 別 評 估 每 位 患 者 繼 續 接 受 治 療 的 風 險 與 效 益 藥 物 交 互 作 用 Zanamivir 並 不 會 和 蛋 白 質 結 合, 也 不 會 在 肝 臟 代 謝 或 改 變 結 構 不 太 可 能 會 發 生 具 臨 床 意 義 的 藥 物 交 互 作 用 懷 孕 與 授 乳 於 妊 娠 期 間 使 用 RELENZA 的 安 全 性 尚 未 獲 得 確 立 針 對 大 鼠 (rat) 與 兔 子 所 進 行 的 生 殖 研 究 顯 示,zanamivir 會 透 過 胎 盤 轉 移 在 針 對 大 鼠 所 進 行 的 研 究 中, 並 未 發 現 任 何 證 據 顯 示 投 予 zanamivir 會 造 成 畸 形 生 殖 力 損 害 或 是 對 子 代 的 出 生 前 發 育 或 出 生 後 發 育 造 成 具 臨 床 意 義 的 損 害 不 過, 目 前 並 無 任 何 關 於 zanamivir 是 否 會 透 過 人 類 胎 盤 轉 移 的 資 料 懷 孕 期 間 不 可 使 用 RELENZA, 尤 其 是 在 第 一 孕 期 ( 妊 娠 的 最 初 三 個 月 ), 除 非 醫 師 認 為 其 對 患 者 的 潛 在 效 益 超 越 胎 兒 可 能 面 臨 的 任 何 風 險 大 鼠 的 研 究 顯 示,zanamivir 會 分 泌 進 入 乳 汁 不 過, 目 前 並 無 任 何 關 於 zanamivir 是 否 會 分 泌 進 入 人 類 乳 汁 的 資 料 由 於 經 驗 有 限, 因 此, 只 有 在 醫 師 認 為 其 對 授 乳 母 親 之 潛 在 效 益 超 越 嬰 兒 可 能 面 臨 之 任 何 風 險 的 情 況 下, 才 可 考 慮 對 授 乳 母 親 使 用 RELENZA 對 駕 駛 及 機 械 操 作 能 力 的 影 響 無 任 何 已 知 影 響

3 不 良 反 應 本 品 尚 未 證 明 用 於 有 潛 在 高 風 險 病 人 的 療 效 及 安 全 性 也 沒 有 在 足 以 認 定 病 況 嚴 重 或 不 穩 定 而 即 將 需 要 住 院 的 病 人, 使 用 本 品 治 療 流 行 性 感 冒 的 資 料 臨 床 試 驗 資 料 RELENZA 經 口 吸 入 投 予 後 的 耐 受 性 極 佳 在 臨 床 試 驗 中, 包 括 那 些 針 對 高 危 險 患 者 ( 老 年 人, 以 及 併 有 某 些 慢 性 疾 病 的 患 者 ) 所 進 行 的 研 究,RELENZA 組 中 所 發 生 的 不 良 反 應 都 和 安 慰 劑 組 相 當 上 市 後 的 資 料 極 常 見 1/10 常 見 1/100 但 <1/10 不 常 見 1/1000 但 <1/100 罕 見 1/10,000 但 <1/1000 極 罕 見 <1/10,000 RELENZA 被 核 准 用 於 治 療 流 行 性 感 冒 之 後 曾 發 生 下 列 事 件 免 疫 系 統 疾 患 : 極 罕 見 : 過 敏 性 反 應, 包 括 顏 面 水 腫 及 口 咽 水 腫 呼 吸 道 胸 部 及 縱 膈 疾 患 : 極 罕 見 : 支 氣 管 痙 攣 呼 吸 困 難 皮 膚 及 皮 下 組 織 疾 患 : 極 罕 見 : 極 罕 見 : 皮 疹 蕁 麻 疹 嚴 重 皮 膚 反 應, 包 括 多 形 性 紅 斑 史 蒂 芬 強 生 症 候 群 (Stevens- Johnson syndrome) 及 毒 性 表 皮 壞 死 溶 解 症 過 量 由 於 本 品 有 物 理 及 投 予 途 徑 上 的 限 制, 而 且 zanamivir 的 口 服 生 體 可 用 率 很 差 (2 至 3%), 因 此 不 太 可 能 發 生 意 外 過 量 的 問 題 曾 以 經 口 吸 入 的 方 式 投 予 劑 量 最 高 達 64 mg/day ( 約 為 每 日 最 高 建 議 劑 量 的 3 倍 ) 的 zanamivir, 結 果 並 未 發 生 不 良 反 應 此 外, 也 曾 連 續 5 天 以 靜 脈 注 射 的 方 式 全 身 性 投 予 最 高 達 1200 mg/day 的 劑 量, 結 果 也 未 發 生 任 何 不 良 反 應 藥 理 學 特 性 藥 效 學 ATC 代 碼 :J05AH01 Zanamivir 是 一 種 強 效 且 具 高 度 選 擇 性 神 經 胺 酸 酶 (neuraminidase; 流 行 性 感 冒 病 毒 表 面 酵 素 ) 抑 制 劑 病 毒 的 神 經 胺 酸 酶 可 幫 助 新 形 成 的 病 毒 粒 子 自 被 感 染 的 細 胞 中 釋 放 出 來, 也 會 使 病 毒 更 容 易 通 過 黏 膜 抵 達 上 皮 細 胞 表 面, 從 而 使 病 毒 得 以 感 染 其

5 高 齡 病 患 : 在 每 日 20 毫 克 的 治 療 劑 量 下, 其 生 體 可 用 率 極 低 (10 至 20%), 因 此 患 者 並 不 會 出 現 任 何 有 意 義 的 zanamivir 全 身 曝 藥 量 藥 物 動 力 學 隨 年 齡 而 發 生 令 任 何 改 變 皆 不 太 可 能 具 有 臨 床 上 的 重 要 性, 因 此 並 不 建 議 調 整 劑 量 腎 功 能 損 害 患 者 : 在 每 日 20 毫 克 的 治 療 劑 量 下, 其 生 體 可 用 率 極 低 (10 至 20%), 因 此 患 者 並 不 會 出 現 任 何 有 意 義 的 zanamivir 全 身 曝 藥 量 由 於 zanamivir 的 安 全 範 圍 極 廣, 嚴 重 腎 衰 竭 患 者 雖 然 可 能 出 現 曝 藥 量 升 高 的 現 象, 但 一 般 並 不 認 為 會 造 成 問 題, 也 不 須 調 整 劑 量 肝 功 能 損 害 患 者 : Zanamivir 在 人 體 內 並 不 會 代 謝, 因 此, 對 肝 功 能 損 害 患 者 並 不 須 調 整 劑 量 臨 床 研 究 當 依 照 建 議 用 於 治 療 健 康 或 高 危 險 群 患 者 的 流 行 性 感 冒 時,zanamivir 不 僅 可 減 輕 症 狀, 亦 可 使 病 期 縮 短 一 項 針 對 主 要 第 III 期 治 療 研 究 (NAIB3001 NAIA3002 NAIB3002 及 NAI30008) 所 進 行 的 綜 合 分 析 顯 示, 在 使 用 zanamivir 的 患 者 中, 流 行 性 感 冒 症 狀 獲 得 緩 解 的 中 位 時 間 可 較 使 用 安 慰 劑 者 縮 短 1.5 天 (p 值 小 於 0.001) 在 使 用 安 慰 劑 的 患 者 中, 併 發 症 的 發 生 率 為 208/711 (29%), 在 使 用 zanamivir 的 患 者 中 則 降 低 至 171/769 (22%) ( 相 對 風 險 :0.77;95% CI:0.65 至 0.92;p=0.004) 在 使 用 安 慰 劑 的 患 者 中, 使 用 抗 生 素 治 療 併 發 症 的 比 例 為 136/711 (19%), 在 使 用 zanamivir 的 患 者 中 則 降 低 至 110/769 (14%) ( 相 對 風 險 :0.76;95% CI:0.60 至 0.95;p=0.021) 研 究 顯 示, 如 果 在 症 狀 出 現 後 盡 快 開 始 治 療,zanamivir 可 發 揮 最 佳 的 療 效 使 用 zanamivir 進 行 預 防 性 治 療 已 證 實 可 有 效 預 防 成 人 與 兒 童 (5 歲 [ 含 ] 以 上 ) 感 染 流 行 性 感 冒 從 出 現 症 狀 且 經 實 驗 室 確 認 的 流 行 性 感 冒 來 看, 在 流 行 性 感 冒 預 防 性 治 療 的 建 議 劑 量 下, 和 安 慰 劑 相 比 較,zanamivir 可 提 供 67 至 79% 的 保 護 效 果, 和 活 性 對 照 藥 物 相 比 較, 亦 可 提 供 56 至 61% 的 保 護 效 果 臨 床 前 安 全 性 資 料 在 動 物 毒 性 研 究 中, 投 予 zanamavir 並 不 會 引 發 任 何 臨 床 相 關 影 響 Zanamivir 並 不 具 基 因 毒 性, 在 針 對 大 鼠 及 小 鼠 所 進 行 的 長 期 致 癌 性 研 究 中, 也 未 發 現 任 何 顯 示 其 具 有 致 癌 性 的 證 據 藥 劑 學 特 性 賦 形 劑 乳 糖 ( 含 乳 蛋 白 ) 不 相 容 性 無 任 何 已 知 的 不 相 容 性

6 有 效 期 限 有 效 期 限 標 示 於 包 裝 上 貯 存 注 意 事 項 RELENZA ROTADISKS 不 可 貯 存 於 溫 度 超 過 30 以 上 的 處 所 容 器 之 性 質 與 內 容 物 RELENZA ROTADISKS 為 一 含 有 4 個 規 則 分 佈 之 泡 囊 的 環 型 錫 箔 碟 片 ( 旋 達 碟 ), 每 一 個 泡 囊 中 都 含 有 5 毫 克 的 zanamivir 及 20 毫 克 的 乳 糖 包 裝 外 盒 中 附 有 一 個 用 以 投 藥 的 碟 型 吸 入 器 (DISKHALER) 使 用 及 操 作 說 明 此 粉 末 製 劑 須 經 口 吸 入 至 肺 臟 碟 型 吸 入 器 (DISKHALER) 中 裝 有 一 片 碟 片, 藥 物 即 盛 裝 於 此 碟 片 的 各 個 泡 囊 中, 這 些 泡 囊 會 在 操 作 吸 入 器 的 過 程 中 打 開 病 患 須 知 說 明 書 使 用 指 示 : 初 次 使 用 前, 請 先 仔 細 閱 讀 使 用 步 驟 教 學 如 果 您 對 DISKHALER 的 使 用 方 式 仍 有 疑 問, 請 要 求 藥 師 為 您 完 整 解 說 一 遍 DISKHALER 可 分 為 三 個 部 分 : 在 您 研 讀 使 用 步 驟 教 學 前, 切 勿 拆 開 這 項 裝 置 吸 嘴 ( 兩 側 各 有 一 個 氣 孔 ) 轉 盤 Rotadisk 可 裝 在 這 個 部 分 上 方 指 握 部 分 刺 針 這 能 刺 破 Rotadisk 上 的 泡 殼, 備 妥 準 備 吸 入 的 藥 物 上 蓋 ROTADISK 可 裝 進 DISKHALER 內

7 ROTADISK 可 裝 在 DISKHALER 的 轉 盤 上 方 ROTADISK 的 四 個 泡 殼 內 均 含 有 RELENZA 的 單 次 劑 量 重 要 : 在 將 ROTADISK 裝 上 DISKHALER 前, 不 可 刺 破 任 何 一 個 泡 殼 未 使 用 時 可 將 ROTADISK 留 在 DISKHALER 上 ; 不 過, 請 等 到 準 備 吸 入 藥 物 前 再 刺 破 泡 殼 請 保 持 DISKHALER 的 清 潔 使 用 後 請 以 紙 巾 擦 拭 吸 嘴, 並 在 未 使 用 時 將 藍 色 外 蓋 裝 回 原 位 使 用 步 驟 教 學 : 如 何 使 用 您 的 RELENZA DISKHALER 將 ROTADISK 裝 入 DISKHALER: 1. 移 除 藍 色 外 蓋 檢 查 吸 嘴 內 外 是 否 清 潔 2. 如 圖 所 示 抓 住 滑 盤 向 外 拉 出, 直 到 停 下 為 止 3. 輕 壓 白 色 滑 盤 兩 側 的 指 握 部 分 將 滑 盤 從 裝 置 主 體 取 出 白 色 滑 盤 應 可 輕 易 取 出

8 4. 將 一 組 新 的 RELENZA ROTADISK 放 上 轉 盤 確 認 印 刷 面 在 上, 泡 殼 朝 下 將 泡 殼 裝 進 轉 盤 的 圓 孔 5. 將 白 色 滑 盤 推 回 裝 置 主 體 內 部 若 您 尚 未 準 備 立 即 吸 入 RELENZA 藥 物, 請 將 藍 色 外 蓋 裝 回 原 位 準 備 您 要 吸 入 的 藥 物 : 請 等 到 準 備 吸 入 藥 物 前, 再 操 作 以 下 步 驟 6. 將 DISKHALER 固 定 於 水 平 狀 態 使 DISKHALER 保 持 在 水 平 狀 態 盡 量 掀 開 上 蓋 上 蓋 必 須 完 全 垂 直, 以 確 保 能 徹 底 刺 破 泡 殼 將 上 蓋 推 回 原 位 現 在 您 的 DISKHALER 已 備 妥 可 供 使 用 在 吸 入 藥 物 前, 請 保 持 裝 置 水 平

9 吸 入 藥 物 : 7. 請 您 暫 時 還 不 要 將 DISKHALER 放 入 口 中 將 DISKHALER 從 您 的 嘴 邊 移 開, 在 舒 適 範 圍 內 盡 可 能 地 吐 氣 請 不 要 對 著 DISKHALER 吹 氣, 這 樣 會 將 藥 物 粉 末 吹 出 ROTADISK 使 DISKHALER 保 持 在 水 平 狀 態 將 吸 嘴 置 入 齒 間, 緊 閉 雙 唇 包 住 吸 嘴 ; 請 勿 啃 咬 吸 嘴 不 要 阻 塞 吸 嘴 側 邊 的 氣 孔 從 吸 嘴 快 速 地 深 吸 一 口 氣 ; 然 後 憋 住 這 口 氣 約 幾 秒 鐘 將 DISKHALER 從 口 中 移 開 繼 續 屏 息 幾 秒 鐘, 或 在 舒 適 範 圍 內 盡 可 能 延 長 這 段 時 間 準 備 下 個 泡 殼 ( 劑 量 的 第 二 部 分 ): 8. 盡 量 向 外 拉 出 白 色 滑 盤 ( 請 勿 將 其 完 全 取 出 ), 然 後 將 它 推 回 原 位 這 個 動 作 會 轉 動 轉 盤, 使 下 個 泡 殼 出 現 必 要 時 重 複 這 個 動 作, 直 到 完 整 的 泡 殼 出 現 在 刺 針 下 方 重 複 步 驟 6 與 步 驟 7 以 吸 入 藥 物

11 RELENZA Zanamivir QUALITATIVE AND QUANTITATIVE COMPOSITION Each RELENZA ROTADISK consists of four regularly spaced double foil blisters each containing a white to off-white micronised powder mixture of zanamivir (5 mg) and lactose (20 mg). PHARMACEUTICAL FORM Inhalation powder. CLINICAL PARTICULARS Indications Treatment of Influenza: RELENZA is indicated for treatment of both influenza A and B in adults and children ( 5 years). Prophylaxis: RELENZA is indicated for prophylaxis of both influenza A and B in adults and children ( 5 years). Dosage and Administration RELENZA is for administration to the respiratory tract by oral inhalation only, using the DISKHALER device provided. Patients scheduled to take inhaled drugs, e.g. fast acting bronchodilators, at the same time as RELENZA should be advised to administer that drug prior to administration of RELENZA. Adults Treatment of Influenza The recommended dose of RELENZA is two inhalations (2 x 5 mg) twice daily for five days, providing a total daily inhaled dose of 20 mg. For maximum benefit, treatment should begin as soon as possible (preferably within two days) after onset of symptoms.

12 Prophylaxis The recommended dose of RELENZA is two inhalations (2 x 5 mg) once daily for 10 days, providing a total daily inhaled dose of 10mg. This may be increased up to one month if the period of exposure risk extends beyond 10 days. The full course of prophylaxis therapy should be completed as prescribed. Children No dose modification is required (see Pharmacokinetics). Elderly No dose modification is required (see Pharmacokinetics). Renal impairment No dose modification is required (see Pharmacokinetics). Hepatic impairment No dose modification is required (see Pharmacokinetics). Contraindications Hypersensitivity to any ingredient of the preparation (see Pharmaceutical Particulars List of Excipients). Warnings and Precautions Influenza infection can be associated with increased airways hyper-responsiveness. There have been very rare reports of patients being treated for influenza who have experienced bronchospasm and/or decline in respiratory function after the use of RELENZA, some of whom did not have any previous history of respiratory disease. Any such patients should discontinue RELENZA and seek medical evaluation. Patients with underlying respiratory disease should have a fast acting bronchodilator available when taking RELENZA (see Dosage and Administration). Allergic-like reactions, including oropharyngeal edema, serious skin rashes have been reported in postmarketing experience with RELENZA. RELENZA should be stopped and appropriate treatment instituted if an allergic reaction occurs or is suspected. Influenza can be associated with a variety of neurological and behavioural symptoms. There have been postmarketing reports (mostly from Japan and in paediatric subjects) of seizures, delirium, hallucination and abnormal behaviour in patients with influenza who were receiving neuraminidase inhibitors, including RELENZA. The events were observed mainly early in the illness and often had an abrupt onset and rapid resolution. The

13 contribution of RELENZA to these events has not been established. If neuropsychiatric symptoms occur, the risks and benefits of continuing treatment should be evaluated for each patient. Interactions Zanamivir is not protein bound and not hepatically metabolised or modified. Clinically significant drug interactions are unlikely. Pregnancy and Lactation The safe use of RELENZA during pregnancy has not been established. Reproductive studies performed in rats and rabbits indicated that placental transfer of zanamivir occurs. Studies in rats did not show any evidence of teratogenicity, impairment of fertility or clinically significant impairment of peri or post-natal development of offspring following administration of zanamivir. However, there is no information on placental transfer in humans. RELENZA should not be used in pregnancy, especially during the first trimester, unless the possible benefit to the patient is thought to outweigh any possible risk to the foetus. In rats zanamivir has been shown to be secreted into milk. However, there is no information on secretion into breast milk in humans. As experience is limited, the use of RELENZA in nursing mothers should be considered only if the possible benefit to the mother is thought to outweigh any possible risk to the infant. Effects on Ability to Drive and Use Machines None known. Adverse Reactions Safety and efficacy have not been demonstrated in patients with high-risk underlying medical conditions. No information is available regarding treatment of influenza in patients with any medical condition sufficiently severe or unstable to be considered at imminent risk of requiring inpatient management. Clinical trial data RELENZA is well tolerated by the oral inhaled route of administration. In clinical studies, including those studies with high risk patients (the elderly, and patients with certain chronic medical conditions), the adverse events reported were similar in the RELENZA and placebo groups.

Immune Systems Disorders Very rare: Allergic-type reaction, including facial and oropharyngeal oedema. Respiratory, thoracic and mediastinal disorders Very rare: Bronchospasm, dyspnoea. Skin and subcutaneous tissue disorders Very rare: Very rare: Rash, urticaria. Severe skin reactions including Erythema Multiforme, Stevens-Johnson syndrome, Toxic epidermal necrolysis Overdose Accidental overdose is unlikely due to the physical limitations of the presentation, the route of administration and the poor oral bioavailability (2 to 3%) of zanamivir. Doses of zanamivir up to 64 mg/day (approximately 3 times the maximum daily recommended dose) have been administered by oral inhalation (by nebuliser) without adverse effects. Additionally, systemic exposure by intravenous administration of up to 1200 mg/day for five days showed no adverse effect. PHARMACOLOGICAL PROPERTIES Pharmacodynamics ATC code: J05AH01. Zanamivir is a potent and highly selective inhibitor of neuraminidase, the influenza virus surface enzyme. Viral neuraminidase aids the release of newly formed virus particles from infected cells, and may facilitate access of virus through mucus to epithelial cell surfaces, to allow viral infection of other cells. The inhibition of this enzyme is reflected

15 in both in vitro and in vivo activity against influenza A and B virus replication, and encompasses all of the known neuraminidase subtypes of influenza A viruses. The activity of zanamivir is extracellular. It reduces the propagation of both influenza A and B viruses by inhibiting the release of infectious influenza virions from the epithelial cells of the respiratory tract. Influenza viral replication is confined to the superficial epithelium of the respiratory tract. The efficacy of topical administration of zanamivir to this site has been confirmed in clinical studies. Clinical trial data have shown that treatment of acute influenza infections with RELENZA produces reductions in virus shedding from the respiratory tract compared to placebo without any detectable emergence of virus with reduced susceptibility to zanamivir. Pharmacokinetics Absorption Pharmacokinetic studies in humans have shown that the absolute oral bioavailability of the drug is low (mean 2%). Similar studies of orally inhaled zanamivir indicate that approximately 10 to 20% of the dose is systemically absorbed, with serum concentrations generally peaking within 1-2 hours. The poor absorption of the drug results in low systemic concentrations and therefore there is no significant systemic exposure to zanamivir after oral inhalation. There is no evidence of modification in the kinetics after repeated dosing with oral inhaled administration. Distribution After oral inhalation, zanamivir is widely deposited at high concentrations throughout the respiratory tract, thus delivering the drug to the site of influenza infection. Following a single 10 mg dose the concentrations of zanamivir were measured at the epithelial layer of the airways, the major sites of influenza viral replication. Zanamivir concentrations of approximately 340 and 52 fold above the median viral neuraminidase IC 50 were measured at 12h and 24h, respectively. The high concentrations of zanamivir in the respiratory tract will result in the rapid onset of inhibition of the viral neuraminidase. The two major sites of deposition are the oropharynx and the lungs (mean 77.6% and 13.2%, respectively). Metabolism Zanamivir has been shown to be renally excreted as unchanged drug, and does not undergo metabolism. Elimination The serum half-life of zanamivir following administration by oral inhalation ranges from 2.6 to 5.05 hours. It is entirely excreted unchanged in the urine. Total clearance ranges from 2.5 to 10.9 l/h as approximated by urinary clearance. Renal elimination is completed within 24 hours.

16 Special Patient Populations Children In an open-label single-dose study the pharmacokinetics of zanamivir have been evaluated in 24 paediatric subjects ages 3 months to 12 years using nebulised (10 mg) and dry powder (10 mg) inhalation formulations. The systemic exposure in children was similar to 10 mg of inhaled powder in adults. Elderly At the therapeutic daily dose of 20 mg, bioavailabilty is low (10 to 20%), and as a result there is no significant systemic exposure of patients to zanamivir. Any alteration of pharmacokinetics that may occur with age is unlikely to be of clinical consequence and no dose modification is recommended. Renal Impairment At the therapeutic daily dose of 20 mg, bioavailabilty is low (10 to 20%), and as a result there is no significant systemic exposure of patients to zanamivir. Given the wide safety margin of zanamivir the possible increased exposure in patients with severe renal failure is not considered problematic and no dose adjustment is required. Hepatic Impairment Zanamivir is not metabolised, therefore dose adjustment in patients with hepatic impairment is not required. Clinical Studies RELENZA, when taken as recommended for treatment of influenza in otherwise healthy and high risk patients, alleviates the symptoms and reduces their duration. In a pooled analysis of the principle phase III treatment studies (NAIB3001, NAIA3002, NAIB3002 and NAI30008) the median time to alleviation of influenza symptoms was reduced by 1.5 days for patients taking RELENZA as compared to placebo (p<0.001). Complications were reduced from 208/711 (29%) of placebo patients to 171/769 (22%) of RELENZA patients (relative risk: 0.77; 95% CI: 0.65 to 0.92; p=0.004). Use of antibiotics for treatment of complications was reduced from 136/711 (19%) of placebo patients to 110/769 (14%) of RELENZA patients (relative risk: 0.76; 95% CI: 0.60 to 0.95; p=0.021). The efficacy of RELENZA has been shown to be optimal if treatment is initiated as soon as possible after the onset of symptoms. RELENZA given as prophylaxis has been shown to prevent influenza in adults and children ( 5 years). RELENZA given at the recommended dose for the prophylaxis of influenza provided 67-79% protection compared to placebo, and 56-61% protection compared to an active comparator against symptomatic, laboratory-confirmed influenza.

17 Pre-clinical Safety Data Administration of zanamivir in animal toxicity studies was not associated with any clinically relevant effects. Zanamivir was not genotoxic and showed no evidence of carcinogenic potential in long term carcinogenicity studies in rats and mice. Special Precautions for Storage RELENZA ROTADISKS should not be stored above 30 C. Nature and Contents of Container RELENZA ROTADISKS consist of a circular foil disk (a Rotadisk) with four regularly distributed blisters each containing 5 mg of zanamivir and 20 mg of lactose. A DISKHALER is provided to administer the medication. Instructions for Use/Handling The powdered medicine is inhaled through the mouth into the lungs. The DISKHALER device is loaded with a disk which contains the medicine in individual blisters which are opened as the device is manipulated. If you re still not sure how to use the DISKHALER, ask you pharmacist to go through the instructions with you. The DISKHALER has three parts: Don t take it apart until you have looked at the step-by-step guide.

18 The ROTADISK fits into the DISKHALER The ROTADISK fits onto the wheel of the DISKHALER. Each of the four blisters on the ROTADISK contains a single dose of RELENZA. Important: Don t pierce any of the blisters on the ROTADISK before you load it onto the DISKHALER. You can keep a ROTADISK on the DISKHALER between doses, but don t pierce a blister until just before you inhale your dose. Keep the DISKHALER clean. Wipe the mouthpiece with a tissue after you use it, and replace the blue cover between uses. STEP-BY-STEP GUIDE TO USING YOUR RELENZA DISKHALER To load a ROTADISK into the DISKHALER: 1. Remove the blue cover.

19 Check that the mouthpiece is clean, inside and outside. 2. Hold the white sliding tray as shown and pull it out until it stops. 3. Gently squeeze the finger grips on the sides of the white tray. Remove the tray from the main body. The white tray should come out easily. 4. Place a new RELENZA ROTADISK on the wheel. Make sure the printed side is up, with the blisters facing downwards. The blisters fit into the holes in the wheel. 5. Push the white tray back into the main body. If you re not ready to inhale a dose of RELENZA straight away, replace the blue cover.

20 To get your dose ready to inhale: Don t do this until just before you inhale a dose. 6. Hold the DISKHALER horizontally. Keep the DISKHALER horizontal Flip the lid up as far as it will go. The lid must be fully vertical, to make sure that the blister is pierced completely. Push the lid back down. Your DISKHALER is now ready for use. Keep it horizontal until you have inhaled your dose. To inhale the medication: 7 Don t put the DISKHALER into your mouth yet. Breathe out as far as is comfortable, keeping the DISKHALER away from your mouth. Don t blow into the DISKHALER. If you do, you ll blow the powder out of the ROTADISK. Keep the DISKHALER horizontal Place the mouthpiece between your teeth. Close your lips firmly around the mouthpiece. Don t bite the mouthpiece. Don t block the airholes on the side of the mouthpiece. Take one quick, deep breath in through the mouthpiece. Hold this breath for a few seconds. Remove the DISKHALER from your mouth. Carry on holding your breath for a few more seconds or as long as is comfortable.

21 To prepare the next blister (the second part of your dose): 8. Pull the white tray out as far as it will go (don t remove it completely), then push it back in again. This will turn the wheel so the next blister will appear. Repeat if necessary until a full blister is positioned under the piercing needle. Repeat steps 6 and 7 to inhale the medicine. 9. After you ve inhaled the full dose (normally two blisters): Wipe the mouthpiece with a tissue and replace the blue cover. It s important to keep the DISKHALER clean. To replace the ROTADISK: 10. When all four blisters are empty, remove the ROTADISK from the DISKHALER and insert a new one, using steps 1 to 5. Not all presentations are available in every country.
