Our mission is to train professionals of nutrition consultation, diet planning and food preparation, nutrition treatment, HCCP safety quality inspection, food biotech and development, and food hygiene and safety laboratory techniques. We offer degree program to train beauty and healthcare, fashion design, cosmetic technology professionals by providing training and practice at a high-end SPA workshop including body and facial care, makeup, hair design, nail care, image design, and aromatherapy cosmetics production laboratories. Additionally, we offer dental assistant degree program for professional dental assistant education. Our primary mission is to educate students on the complete fundamentals of medical laboratory technology including serology, physiology, biochemistry, and microorganism. To allow our students to obtain advanced laboratory techniques, we put a further emphasis on molecular genomic analysis, prevention medicine, and tumor biology. 4 壹 申請資格 /Qualifications 一 入學身分 ( 含轉學生 )/Identity( transfer students included) 符合以下身分者, 得以外國學生身分申請入學 International students who meet one of the following identity regulations can apply for admission. ( 一 ) 具外國國籍且未曾具有中華民國國籍, 於申請時並不具僑生資格者, 得依規定申請入學 An applicant of foreign nationality, who has never held the nationality of the Republic of China (R.O.C.), does not possess overseas Chinese student status at the time of application. 有下列各款情形之一者, 屬中華民國國籍 : 1 出生時父或母為中華民國國民 2 出生於父或母死亡後, 其父或母死亡時為中華民國國民 3 出生於中華民國領域內, 父母均無可考, 或均無國籍者 4 歸化者 前項第一款及第二款之規定, 於國籍法修正公布時之未成年人, 亦適用之 An applicant who meets one of the following conditions acquires the nationality of the Republic of China: 1 His/Her father or mother was a national of the Republic of China when he/she was born. 2 He/She was born after the death of his/her father or mother, and his/her father or mother was a national of the Republic of China at the time of death. 3 He/She was born in the territory of the Republic of China, and his/her parents can't be ascertained or both were stateless persons. 4 He/She has undergone the nationalization process. Preceding subparagraph 1 and subparagraph 2 shall also apply to the persons who were minors at the time of the revision and promulgation of the Nationality Act. ( 二 ) 具外國國籍, 於申請時已連續居留海外六年以上 ( 計算至 2018 年 8 月 1 日止且每年在臺停留時間不超過 120 日 ) 並符合下列規定之一者 : Applicants with foreign nationality, who have stayed overseas continuously for more than 6 years (up to August 1,2018), stayed in Taiwan for no more than a total of 120 days per calendar year, and with one of the following conditions. 1 申請時兼具中華民國國籍者, 應自始未曾在臺設有戶籍 Applicants holding both foreign and R.O.C. nationalities but have NEVER established any household registration in Taiwan. 2 申請前曾兼具中華民國國籍, 於申請時已不具中華民國國籍者, 應自內政部許可喪失中華民國國籍之日起至申請時已滿八年 Applicants holding foreign nationality who once held R.O.C. nationality must have abdicated their R.O.C. nationality for more than 8 years before application. (As determined by the Ministry of Interior up to August 1,2018) 3 前二款均未曾以僑生身分在臺就學, 且未於當學年度接受海外聯合招生委員會分發 Applicants in the preceding subparagraph 1 and subparagraph 2 should have NEVER held overseas Chinese student status, also can NOT apply to the University Entrance Committee for Overseas Chinese Students for admission in the enrollment year ( 2

5 ( 三 ) 具外國國籍, 兼具香港或澳門永久居留資格, 且未曾在臺設有戶籍, 申請時於香港 澳門或海外連續居留滿六年以上者 Applicants of foreign nationality, concurrently holding a permanent residence status in Hong Kong or Macao, having no history of a household registration record in Taiwan and, at the time of application, have resided in Hong Kong, Macao, or another foreign country for no less than 6 years. ( 四 ) 曾為大陸地區人民具外國國籍且未曾在臺設有戶籍, 申請時已連續居留海外六年以上者 Applicants being a former citizen of Mainland China and hold foreign nationality, having no history of household registration record in Taiwan, and at the time of application, have resided overseas continuously for no less than 6 years. 依教育合作協議, 由外國政府 機構或學校遴薦來臺就學之外國國民, 其自始未曾在臺設有戶籍者, 經主管教育行政機關核准, 得不受前列規定之限制 前列所定六年, 以擬入學當學期起始日期 ( 二月一日或八月一日 ) 為終日計算之 According to the Cultural Exchange Agreement, a foreign national who was sent by a foreign government, organization, school, or educational/cultural group to study in Taiwan, is not subject to the limitation as prescribed in the preceding paragraph. The six year period as prescribed in the first paragraph shall be calculated from the date of when the semester begins (Feb. 1 or Aug. 1). ( 五 ) 本簡章所稱海外, 指大陸地區 香港及澳門以外之國家或地區 ; 所稱連續居留, 指外國學生每曆年在國內停留期間不得逾一百二十日 連續居留海外採計期間之起迄年度非屬完整曆年者, 以各該年度之採計期間內在國內停留期間未逾一百二十日予以認定 但符合下列情形之一且具相關證明文件者, 不在此限 ; 其在國內停留期間, 不併入海外居留期間計算 : The term overseas as prescribed above is limited to countries or regions other than Mainland China, Hong Kong and Macau; the term reside overseas continuously means that an individual may stay in Taiwan for no more than a total of 120 days per calendar year. The only exceptions to this method of calculation are for those who fulfill one of the following requirements with written supported proof: 1 就讀僑務主管機關舉辦之海外青年技術訓練班或教育部認定之技術訓練專班 Attended overseas youth training courses organized by the Overseas Compatriot Affairs Council or technique training classes accredited by the Ministry of Education ; 2 就讀教育部核准得招收外國學生之各大專校院華語文中心, 合計未滿二年 Attended a Mandarin Chinese language center at a university/college of which foreign student recruitment is approved by the Ministry of Education, and to which the total length of stay is less than 2 years. 3 交換學生, 其交換期間合計未滿二年 Exchange students, whose length of total exchange is less than 2 years. 3

6 4 經中央目的事業主管機關許可來臺實習, 實習期間合計未滿二年 An Internship in Taiwan which has been approved by an authorized central government agency, to which the total length of stay is less than 2 years. 具外國國籍並兼具中華民國國籍, 且於外國學生來臺就學辦法中華民國一百年二月一日修正施行前已提出申請喪失中華民國國籍者, 得依原規定申請入學, 不受前述規定之限制 An individual, who has both foreign and R.O.C. nationalities and has applied for an annulment of their R.O.C. nationality before February 1, 2011, the effective date of this amendment, will then be qualified to apply for admission as an international student under the amendment effective before February 1, 2011 and will not be subject to the limitation as prescribed above 年度申請至本校秋季班就讀之外國學生, 其應備文件及相關規定應以中華民國教育部網站公布之最新 外國學生來臺就學辦法 之規定為準, 請申請者隨時查閱教育部網站 ( 網址 及本校網站 All application required documents and regulations are based on MOE Regulations Regarding International Students Undertaking Studies in Taiwan. 7 二 學歷資格 /Academic Qualification Requirements ( 一 ) 外國學生具以下學歷資格者得申請入學本校日間部碩士, 學士及副學士學位課程 申請人的學歷及相關學歷證明文件應經台灣駐外代表處或入學申請授權的機構驗證 International students with corresponding academic qualification are eligible for master, bachelor and associate degree programs in Day Division. Qualification certificates for application should be verified by Taiwan s overseas representative offices or authorized agencies. 1 具有等同國內大學院校學歷者, 可申請碩士班 Graduate Institute (Master) Program: Applicants should be graduates of public, private or foreign universities, colleges or equivalent. 2 具有等同國內高中職學歷者, 可申請四年制學士班 Four-Year Bachelor Program: Applicants should be graduates of public, private or foreign vocational high school, high school or equivalent. 3 具有等同國內專科之學歷者, 可申請二年制學士班 Two-Year Bachelor Program: Applicants should be graduates of public, private or foreign junior colleges or equivalent. 4 具有等同國內國中之學歷者, 可申請五年制專科部 Five-Year Junior College Program: Applicants should be graduates of public, private or foreign junior high school or equivalent. ( 二 ) 以上外國學生最高學歷必須符合本國 入學大學同等學力認定標準 相關規定 ( 請參見教育部 畢業年級相當於國內高級中等學校二年級之國外或香港 澳門同級同類學校畢業生 ( 如英制高中中學五年或 11 級 ), 得申請本校學士班 除原課程外, 另需增修至少 12 個畢業學分, 其外加科目由各系另行訂定之 畢業 2 年 ( 含 ) 以上者, 逕以高中同等學力採認之, 不另增修畢業學分 The recognition of applicants best educational qualification shall be in accordance with the Standards for Recognition of Equivalent Qualifications for University Entrance Examination (see the Ministry of Education, R.O.C at ( Applicants, having completed study at an equivalent foreign or Hong Kong/Macau high school (Form 5 or Year11), equivalent to 2 nd year of senior high school in Taiwan, may apply for an undergraduate program with a condition of taking extra 12 credits beside graduation credit criteria and courses designated by intended departments. Applicants having graduated from senior high school for no less than 2 years are considered as senior high school graduates with equivalent certificate, exempt from the required condition. 5

11 肆 申請應繳交資料 /Required Documents 一 繳交資料記錄表 /Application checklist 二 入學申請表 Application form for international student admission. A two-inch recent photo (about 3.5cm 4.5cm) must be attached to the application form. 三 護照影本或其他國籍證明文件 Copy of passport or other equivalent verification of nationality. 四 經我國駐外使領館 代表處 辦事處或其他經外交部授權機構 ( 以下簡稱駐外館處 ) 驗證之外國學校最高學歷或同等學力證明文件及成績單 ( 中 英文以外之語文, 應附中文或英文譯本 ) One copy of the highest academic diploma and the transcript of the highest educational degree authenticated by Taiwan, R.O.C. embassies, representative, consulates, or other mission authorized by the Taiwan Ministry of Foreign Affairs. (hereinafter referred to as Taiwan Overseas Mission) If the original document is not in English or Chinese, the document must be notarized after being translated into English or Chinese. 五 由金融機構提出 ( 於報名日期內, 密封逕寄本校 ) 足夠在臺就學之財力證明 Financial Statement verified / sealed by the financial institution with proof of sufficient funds for the applicant s entire stay for study in Taiwan. 若財力證明非申請者本人, 需同時附上資助者財力證明書 If the financial statement is not in the name of the applicant, a financial statement is also required from the sponsor. 六 入學申請切結書 / Declaration 七 留學計畫書 ( 中 英文以外之語文, 應附已公證之中文或英文譯本 ) Study plan in Chinese or English. (A Chinese or English translation which is notarized by local authority is required if the study plan is not in either of the languages aforementioned.) 八 申請入學五專課程之外國學生應另檢附 (1) 在臺監護人資格證明文件 (2) 經駐外館處驗證之父母或其他法定代理人委託在臺監護人之委託書 (3) 經我國公證人公證之在臺監護人同意書 所稱在臺監護人, 應為在臺設有戶籍之中華民國國民, 並提出警察機關出具之無犯罪紀錄證明 Applicants applying for the Junior College Program have to submit theapplicants applying for the Junior College Program have to submit (1) Documents of the eligibility of a legal guardian in Taiwan (2) Authorization Letter of one s parents or any other legal representative attorney power to the guardian in Taiwan with verification of overseas agencies. (3) Notarized Letter of Agreement from one s legal guardian in Taiwan. The legal guardian in Taiwan referred in the preceding Article shall mean a Taiwan, R.O.C. citizen who has passed a police criminal record check. 9

(For more information about TOCFL, please refer to the website at 十 申請人於報名時所提供之資料, 僅作為本校招生使用, 且除提供申請人個人 本校招生相關單位 申請系所 移民署 教育部及其委託之學歷查證單位使用外, 其餘均依照 個人資料保護法 相關規定處理 All applied materials are only for school admissions use, and in addition to providing the individual applicant, admission units, the application program, National Immigration Agency, the Ministry of Education and the units of academic credential examination, but all the other uses have to be in accordance with the "Personal data Protection Act". 10

13 伍 其他申請注意事項 /Other Application Related Information 一 本校教學以華語授課為主, 部分課程採英語授課 Classes are instructed mainly in Chinese and partially in English. 二 申請人至多可同時申請三個系所, 所有申請資料皆需要重複繳交並填寫志願序 各系所申請資料分別裝袋並個別黏貼 報名信封封面, 但申請資料可合併寄送 Applicants can apply for a maximum of three degree programs one time. Please prepare DUPLICATE application materials for each program and specify your priority order. Place the application materials for each program in separate envelopes with respective application cover sheet attached. However, the application envelops can put in one single package together. Applicants have to fulfill all application requirements for each degree program for admission. 三 申請人不曾以僑生身分在中華民國就學 Applicants have never studied in the Republic of China with Overseas Chinese student identities. 四 報名時所繳之外國學校最高學歷或同等學力證明文件, 其為外國學校核發者, 除海外臺灣學校所發者外, 應依 大學辦理國外學歷採認辦法 相關規定辦理 The diploma and transcript submitted for application review shall first be authenticated in accordance with the rules promulgated by the Taiwan, R.O.C Ministry of Education, except for diplomas and transcripts issued by Overseas Taiwanese or Overseas Chinese schools. 五 凡曾遭國內大專校院退學者或觸犯刑法者, 不得再向本校申請入學 入學後經查證屬實者, 撤銷其入學資格 Those who have been expelled or withdrawn from any university or collage in Taiwan due to their misconducts, failure of their academic grade, or found guilty of criminal cases are NOT eligible to apply. Applicants who violate this regulation will have their admission nullified or status as a Fooyin University student revoked. 六 如有其他未盡事宜, 悉依教育部 外國學生來臺就學辦法 本校學則暨相關規定辦理 For all other related information, please refer to MOE Regulations regarding International Student Studies in Taiwan, the International Student Recruiting Policy of Fooyin University, or other related guidelines. 七 本招生簡章中文與英文若有文字敘述衝突之處, 以中文敘述為依據 If any conflict between the text description of Chinese and English in this admission brochure is presented, it will base on the basis of the Chinese narrative. 11

14 陸 獎學金 /Scholarships 一 輔英科技大學優秀外國學生獎 / Fooyin University Rewards Guidelines for Excellent Foreign Students 本校優秀外國學生獎依本校獎學金要點辦理 Excellent international student awards will be issued according to our scholarship regulation. 二 獲獎同學, 需於就學期間參與若干時數之服務學習 Students who wins rewards will have to serve for certain hours as requirement before completion of study. 三 依本校 優秀外國學生奬勵要點 規定, 如入學前辦理休學或保留學籍 原核定獎助學金之資格即予取消 獲獎學生如有中途轉學或休退學情事者, 則追繳已領取之當學年獎勵 According to Fooyin University Rewards Guidelines for Excellent Foreign Students, if qualified recipients reserve their admissions status or take a leave of absence before registration, they shall lose their scholarship. 16 玖 審查及通知錄取 / Review and Notification for Admission 一 本校外國學生之入學申請, 由國際暨兩岸事務處受理後進行初審 ; 初審合格者, 即送交各系 ( 所 ) 複審 ; 各系 ( 所 ) 應依其訂定之入學標準, 在規定名額內, 確定錄取名冊並送交國際暨兩岸事務處 錄取名冊呈報校長核准後, 由國際暨兩岸事務處發給錄取生錄取通知 Application received by the Office of International and Cross-Strait Affairs will be proceeded to qualification review. Qualified ones will be sent to corresponding academic department for admission review. 18 拾壹 報到與註冊入學 /Registration and Admission 一 外國學生接到入學許可後, 應依規定程序辦理報到及註冊手續, 逾期未辦理報到者, 即以自動放棄入學資格論, 但事前以書面提出延期申請並經許可者, 不在此限 Students receiving Admission Certificate, enrollment procedures should be taken in accordance with the provisions of the procedure. Students fail to enroll on time will be deemed to forfeit their rights of admission, except a written petition is submitted for extension approval. 二 外國學生經入學後, 如發現有申請文件資格不符 偽造 假借 塗改等情事, 一經查明即開除學籍, 亦不發給任何學歷證件 ; 如於畢業後始發覺者, 除勒令繳銷其畢業證書外, 並公告取消其畢業資格 Admission shall be denied and / or the student shall be dismissed from Fooyin University after admission if there are any violations of the general academic honor codes and / or in the authenticity of the applicant s documents. Those who have graduated shall have their diploma revoked. 三 外國學生接獲本校入學許可後, 須提出接獲錄取通知後 3 個月內之健康檢查記錄 ( 如附件七 - 健康檢查證明應檢查項目表乙表 ) Upon receiving Admission Certificate, each student need to provide a Health Examine Form with certificate dated within the past 3 months (See Appendix VII-Items Required for Health Certificate, Form B). 四 外國學生來臺註冊後, 仍需配合參加本校新生體檢 Each student also needs to have a physical examination in school after registration. 五 經本項招生錄取之外國學生於報到後, 應依規定辦理註冊入學手續, 否則取消入學資格 New students shall arrive at Fooyin University campus for registration prior to the date specified on the admission document. 16

The above mentioned written proof of insurance issued in a foreign country shall be verified by Overseas Agencies. 2. 外國學生在臺領獲居留證後並連續居住滿六個月, 始可加入全民健保 Until international students are eligible to enroll the National Health Insurance Program, they have to get Alien Residence Certificate and then live in Taiwan continuously for six months. 18

21 拾伍 其他注意事項 /Other Important Information 一 報名時繳交之 畢業證書, 除海外臺灣學校所發者外, 應依教育部 國外學歷查證認定作業要點 之規定辦理 The diploma submitted for application review shall first be authenticated in accordance with the rules promulgated by the Ministry of Education, except for diplomas issued by Overseas Taiwanese schools. 二 入學許可並不保證簽證核給 ; 簽證核發需檢付相關文件至我國駐外館處申請 ( 請參閱外交部領事局申請居留簽證說明 The Admission Certificate does not guarantee visa issuance. Visas are approved by the Ministry of Foreign Affairs and must submitted the application documents.
