1 1. 目的 Purpose : 為使 Focaltech 產品可符合環境危害之禁用物質相關國際法令規定及客戶要求, 特制定本辦法管制 要求外包廠和原物料供應商之材料零組件及包裝材料中危害物質限制使用含量 To ensure Focaltech products could be met International directives and requirements of customers. Suppliers should follow this management regulation to control the hazardous substance which contained in the components, used in manufacturing process andpacking materials of Focaltech products. 2. 範圍 Scope: 本公司及外包廠 原物料供應商有關於環境危害之禁用物質管理皆適用 Targets are the parts, materials and other components purchased by Focaltech. 3. 名詞定義 Terms and Definitions: RoHS: 歐盟對電子與電氣設備中限制使用某些有害物質禁用的指令 Restriction on Hazardous Substance WEEE: 歐盟對電子與電氣設備廢棄物回收的指令 Waste of Electrical and Electronic Equipment BOM: 材料清單 Bill of Material 原材料 : 構成產品主體的材料, 亦即產品生產所需, 經加工或製程後轉化為產品或產品的一部份 Raw Material 包裝材料 : 產品回貨包裝時所需的物品, 亦即回貨時除產品本身以外的所有物品皆屬之 Packing Material Lead free ( Pb free ) product: 管制中的產品, 針對鉛的含量依歐盟 RoHS 規定需小於 1,000 ppm According to Rohs, Lead(Pb) should be limited under 1,000 ppm. 2 4. 權責 Responsibility: 4.1. 敦泰業務部門 : 確認客戶端的相關需求並轉達品保處審核 Focaltech Sales Dept.: Confirming requirement from customers 敦泰品保處 : 定期進行環境有害物質規範鑑別暨符合度審查與審核廠商提供的相關文件 ( 包含無鉛產品的可靠度驗證 ), 以確保產品符合國際相關法令與客戶之要求 Focaltech QC Dept.: Review International hazardous substance directives regularly. Clarifying products could conform to International directives and regulations of customers with verifying relative documents from suppliers 敦泰之製程外包廠及原物料供應商 : 提供符合本規範要求之產品與相關文件 報告 Suppliers: Providing reports and other relative documents to promise all applications and process could comply with Focaltech s regulation. 5. 作業內容 Content: 5.1 外包廠製程管理 Supplier management: 針對新增外包廠, 由敦泰品保處提供 環境有害物質不使用保證書 ( 附件三 ), 供應商認證證書調查表 ( 附件四 ) 及 材料物質調查表 予新增外包商填寫 To declare not using banned substances contained in parts, components, and packaging materials, or during manufacturing process, supplier should sign Annex Ⅲ Annex Ⅳ 針對特定客戶需求, 外包廠須依客戶格式及需求資料回覆相關問卷後提交敦泰品保處審核 Suppliers should providing reports and other relative documents to Focaltech for specific customers 本公司各相關之外包製程廠商之適用範圍參照下表 scope : 產品有害物質 Hazard substance in product 包材有害物質 Hazard substance in packing material IC Fab Bumping Dice saw / COG Assembly ILB for TCP / COF product V V V V FT ( 包含 T&R) V V V V ISO 文件 : 版本 :K 2 此文件為敦泰電子專有之財產, 未經許可,

3 5.2 文件要求 Document Requirement: 製程外包廠於提供 供應商認證證書調查表 時需附上相關證書 ( 以電子檔方式儲存 ) Supplier should provide certifications of ISO 9001 / ISO / OHSAS18001 / TS16949 / QC08000 / SA 製程外包廠於提供 材料物質調查表 時需提供測試的材料分析報告, 以證明所供應貨品符合本公司及客戶要求 Supplier should provide ICP reports and SDS(MSDS) with Material & Substance Check List to promise all applications and process could comply with Focaltech s regulation 製程外包廠需於歐盟正式公告最新版高關注度物質清單後 3 個月內, 提供最新版 EU REACH ICP 測試報告 / 聲明書予本公司, 以確保所供應之貨品符合最新 EU REACH 規範之要求 EU REACH 監控清單可參閱附件二 Suppliers should provide the latest EU REACH ICP test report / Declaration letter to Focaltech in three months after EU published the latest Candidate List of of Very High Concern to ensure all products and processes could comply with EU REACH regulation. EU REACH-SVHC List can refer to attachmentⅡ. 5.3 材料測試分析化驗報告的相關要求 Test Report Requirement: 測試報告只接受合格的第三方公正單位, 通過 ISO 認證, 並取得認可證書, 且核發證書的認證機構須為 ILAC MRA( 國際實驗室認證聯盟相互承認辦法 ) 簽屬會員國的實驗室認證機構的檢測報告, 不接受原廠自有的實驗室化驗的分析測試報告 Only accept reports from the 3 rd party laboratory. The laboratory is required to pass the ISO certification and acquire the approval certificate. Moreover the certificate-issuing institute must be the laboratory certification institute in one of ILAC MRA s signatory member states (mutual recognition regulations for the International Laboratory Accreditation Cooperation) 測試報告有效期為一年, 供應商需每年提供最新檢測報告, 測試報告僅接受英文版 ( 或中英文版 ) 內容, 其餘版本文件將被退件並要求更新後重新提交 The acceptable valid period of third party test report is one year from the report issued date. Supplier should renew report yearly. Only English(or Chinese and Engilsh) content will be acceppted for the chemical analysis report. All other document versions eill be retuened and requested to be re-submitted after being updated 化驗報告中至少需記載下列事項 ICP report should include: a. 報告編號 Serial number. b. 產出報告時間 Date. 5 5.3.8 當外包商檢測出有害物質超過標準時, 必須在 24 小時內回報敦泰 若材料不良品已交貨至敦泰, 亦須立即回報敦泰並提供緊急召回與防堵改善措施 When hazardous substances detect and out of control limit, subcontracts should inform FocalTech immediately within 24 hours If products already ship to FocalTech, subcontracts should inform FocalTech and provide call back action and containment/ corrective action plan. 5.4 材料物質調查表 & 物質安全資料表 Material & Substance Check List & SDS(MSDS): 供應商需針對每一產品填寫 材料物質調查表, 內容需附上 SDS(MSDS) 及材料測試分析化驗報告 Supplier should provide Material & Substance Check List by device. The contents have to correspond with SDS(MSDS), and SDS(MSDS) & ICP report should be attached 包材部分 : 切割廠 封裝廠 ( 包含 T&R) 對產品包裝時所需的物品填寫 Packing material: Die Saw & Assembly house (include T&R) should provide Material & Substance Check List for packing material 物質安全資料表有效期限為三年 The valid period of Material safety data sheets is three years from the sheet issued date. 5.5 環境管理有害物質限值 Restricted Standard: 管制對象及其限值要求請依照附件一國際指令及敦泰客戶規範為依據 Restricted substances and limit concentrations are specified in Restricted List as attachment Ι 提供予敦泰化驗報告至少需含鉛 (Pb) 鎘(Cd) 汞(Hg) 六價鉻(Cr6+) 多溴聯苯 (PBBs) 多溴聯苯醚(PBDEs) 溴(Bromine) 氯(Chlorine) 及鄰苯二甲酸鹽類物質 (Phthalate) ICP report must include test result of Pb Cd Hg Cr6+ PBBs PBDEs Bromine Chlorine and Phthalate(DEHP BBP DBP DIBP). 5.6 如敦泰客戶及其終端客戶有特殊的管控物質及其化驗需求, 經雙方 ( 敦泰電子與外包廠 ) 達成協議, 外包廠需完全配合其規範並提供相關測試報告 If end-customer have specific request for hazardous substance, suppliers have to follow the request and provide relative reports after agreement of both side. 6 5.7 客戶要求相關證書與問卷的處理 Procedures for customer survey: 敦泰需定期進行環境有害物質規範鑑別暨符合度審查, 並將結果登錄於鑑別查核表, 當法規新增 修訂或廢止時, 須通知供應商進行調查產品及製程之符合性, 相關紀錄統一由敦泰品保處處理並建檔儲存 Focaltech should identify and record International hazardous substance directives regularly. If there is any variation, Focaltech should issue suppliers to make sure the conformation 敦泰品保處於收到客戶需求, 彙整後轉交相關單位或權責外包廠回覆, 一般件供應商需於 5 個工作天內完成回覆 Supplier should reply survey result within 5 working day for normal case 敦泰品保處於收到客戶規範後, 需將規範建立於系統 文件 / 資料查詢調閱作業, 統一由敦泰技術資料室將其文件公佈予各供應商 Focaltech QA Dept. have to upload customer s regulations to DCC system. Focaltech DCC will release the document to each supplier 依據 RoHS(2002/95/EC and 2011/65/EU) 要求, 應保存相關文件 / 檔案超過 10 年以上 According to RoHS (2002/95/EC and 2011/65/EU) requirement, the related hazardous material documents/files should be saved more than ten years. 5.8 Lead free 及 green 產品關於 BOM 或包材的驗證及選擇 BOM and packing material for Lead free product & green product. 客戶有特殊限定之指定材料或包材, 本公司將予以轉達權責外包廠以確定外包廠商可配合客戶需求執行 若客戶無特殊限定之指定材料或包材, 任何 BOM 或包材的選擇原則上由外包廠依其製程特性選定後經其內部驗證或檢測合格後交本公司進行審核, 一旦經本公司核可後, 任何變更需經本公司同意後始可執行, 外包廠皆不得擅自變更, 任何變更需依循 PC 管理辦法執行 Focaltech will confirm with suppliers if end-customer have specific request for fabrication. If end-customer do not assign for specific material, suppliers have option for purchasing suitable objects, and then provide internal verification or 3rd part test report to Focaltech. After approving by Focaltech, any change should be implemented follow PC management procedure 5.9 衝突礦產 Conflict metals 衝突礦產是指鉭 (Ta) 錫 (Sn) 鎢 (W) 金 (Au) 及其他礦產, 其來源來自剛果人民共和國或鄰近國家之非法開採地區 Conflict metals means the Tantalum(Ta) Tin(Sn) Tungsten(W) Gold(Au) or other minerals mined in the illegal area in Democratic Republic of the Congo or an adjoining country. 7 5.9.1 敦泰聲明 Statement of Focaltech a. 敦泰重申敦泰電子及其供應商會遵守電子產業行為準則, 只從對環境和社會負責的供應商採購材料 Focaltech expects our suppliers to comply with the EICC and to only source materials from environmentally and socially responsible suppliers. b. 敦泰不接受產品所含有之鉭 (Ta) 錫(Sn) 鎢(W) 金(Au), 其礦產來源來自剛 果人民共和國或鄰近國家之非法開採地區 Focaltech does not accept the Tantalum(Ta) Tin(Sn) Tungsten(W) Gold(Au) or other minerals mined in the illegal area in Democratic Republic of the Congo or an adjoining country. c. 敦泰將告知及要求所有供應商履行前述聲明 Focaltech will inform and ask its suppliers to perform the above statement 供應商應盡之職責 Responsibility of suppliers a. 供應商交料與敦泰之材料成分資訊若含有鉭 (Ta) 錫(Sn) 鎢(W) 金(Au) 時, 需額外提供此材料之冶煉廠資訊予敦泰確認 Suppliers shall provide smelter information to Focaltech if the composition of materials provided to Focaltech include Tantalum(Ta) Tin(Sn) Tungsten(W) and Gold(Au),and Focaltech will confirm these information. b. 冶煉廠資訊須包含冶煉廠名稱 所在國家 / 地址 聯絡人資訊及礦源所在國家等 Smelter information need include smelter name, country/address and contact Information and country of mine(s). c. 供應商需於當年度 12/31 完成提交 CFSI 調查表 若敦泰有其他需求, 供應商得從敦泰之要求 Suppliers shall be provided CFSI file before 12/31 in this year. And if Focaltech has other request, suppliers shall follow Focaltech s request EICC CFSP 合格熔煉廠導入 EICC CFSP introduced a. 供應商需使用 EICC CFSP 認證之合格熔煉廠 Suppliers shall use the qualified smelter from EICC CFSP. b. 供應商若使用尚未通過 CFSP 認證之熔煉廠, 供應商需於敦泰要求之期限內完成合格熔煉廠之導入 If suppliers used non-qualified smelter from EICC CFSP, suppliers shall finish the introduction of qualified smelter from EICC CFSP according to the deadline of Focaltech s request. 8 5.9.4 衝突礦產管理流程建立 Establishment of conflict minerals management flow a. 供應商需建立衝突礦產管理流程, 並將管理流程文件化 Suppliers shall establish conflict minerals management flow, and document the related management flow. b. 供應商需接受敦泰之衝突礦產稽核, 並透過稽核活動提供管理流程之相關文件及證明 Suppliers shall accept the Focaltech conflict minerals audit, and provide related advance document via the audit activities. 6. 相關參考文件及表單 : 6.1.
