7 中文摘要摘要背景及目的肿瘤干细胞 (CSCs 或 T-ICs) 具有耐受化疗药物, 自我更新无限增殖, 及肿瘤起始的潜能肿瘤干细胞是肿瘤化疗失败的根源, 因此找到有效的对抗肿瘤干细胞的药物成为抗肿瘤治疗的新策略二甲双胍 (Met) 作为胰岛素增敏剂, 于 20 世纪 50 年代即已应用于 2 型糖尿病的治疗, 近年多个研究提示二甲双胍具有抑制乳腺癌胃癌干细胞生长的作用 CD133 分子作为分离肿瘤干细胞的常用标记, 已在大量实体瘤中被证实 CD133 + 肝癌细胞已被证实具有肝癌干细胞的生物学特性二甲双胍能否抑制肝癌干细胞的生长增殖, 目前尚无研究报道我们主要探讨二甲双胍能否抑制肝癌干细胞 (CD133 + 肝癌干细胞 ) 及初步分析其调节肝癌干细胞的分子机制方法流式细胞术 (FCM) 检测 Huh-7 PLC/PRF/5 细胞中 CD133 + 肿瘤干细胞表达情况 ;Western Blot(WB) 和流式细胞术检测二甲双胍对肝癌细胞中 CD133 + 肿瘤干细胞表达量的影响免疫磁珠分选技术 (MACS) 纯化肝癌 CD133 + 肿瘤干细胞 ; 无血清悬浮培养肿瘤干细胞球, 观察其在二甲双胍处理后的生长增殖情况 ; 平板克隆形成实验观察经免疫磁珠分选出的 CD133 + 肿瘤干细胞在 Met 处理后的生长增殖情况实时荧光定量 PCR(qRT-PCR) 检测二甲双胍对肿瘤干细胞相关基因的影响流式细胞仪检测裸鼠移植瘤实验瘤体内的 CD133 + 肿瘤干细胞表达量结果 WB 和 FCM 结果显示二甲双胍抑制肝癌细胞 Huh-7 和 PLC/PRF/5 中 CD133 + 肿瘤干细胞的表达,25mM 二甲双胍处理 Huh-7 细胞后,CD133 表达量由原来的 73.9±2.2% 下降到 23.83±2.85%;PLC/PRF/5 中的 CD133 + 肿瘤干细胞由原来的 45.26±3.28% 下降到 3.71±3.55% 同时 Met 下调干细胞相关基因 (β-catenin Nanog Notch1 Oct4 Sox2 CD133) 的表达 ; 无血清悬浮培养肿瘤干细胞球, 在二甲双胍处理天后相差显微镜下观察药物处理组肿瘤干细胞球显著少于对照组 ;Huh-7 和 PLC/PRF/5 细胞经免疫磁珠分选技术 (MACS) 分选出 CD133 + 肿瘤干细胞, 进行克隆形成实验, 显示在二甲双胍处 I

8 二甲双胍在体内外靶向抑制肝癌 CD133 + 肿瘤干细胞理天后, 吉姆萨染液 (Giemsa) 染色观察到药物处理组克隆形成数显著少于对照组 ; 二甲双胍对 CD133 + 肿瘤干细胞的抑制作用是具有靶向性的, 在 25mM 二甲双胍处理 Huh-7 和 PLC/PRF/5 细胞 48h 后, CD133 p-akt s473 p-mtor 蛋白表达水平及 Bcl-2/Bax 比值均较对照组下降在 PI3K/Akt/mTOR 信号通路抑制剂 LY 处理 Huh-7 和 PLC/PRF/5 细胞 48h 后,CD133 p-akt s473 p-mtor 蛋白表达水平均下降, 然而当联合应用 LY 和 Met 处理细胞时,CD133 p-akt s473 p-mtor 蛋白表达水平高于单用 Met 时的表达水平, 说明低活性的 PI3K/Akt/mTOR 信号通路对 Met 作用不敏感在加入 PI3K/Akt/mTOR 信号通路激活剂 EGF 后,CD133 p-akt s473 p-mtor 表达均上调, 然而在激活的 PI3K/Akt/mTOR 信号通路中同时应用二甲双胍处理细胞 (EGF+Met),CD133 p-akt s473 p-mtor 蛋白表达较单用 Met 处理后下调更显著, 说明高活性的 PI3K/Akt/mTOR 信号通路对 Met 作用更敏感 ; 裸鼠移植瘤实验显示, 与对照组相比, 二甲双胍组顺铂组, 及联合用药组肿瘤体积均显著减小, 其中二甲双胍组的抑瘤率为 46.37%, 顺铂组抑瘤率为 51.90% 二甲以胍联合顺铂组抑瘤率为 74.60% 结论二甲双胍在体内体外均具有靶向抑制肝癌 CD133 + 肿瘤干细胞表达的作用关键词 : Metformin;CD133;PI3/AKT/mTOR II

9 英文摘要 Abstract Background&Aims Cancer stem cells (CSCs) are thought to be inherently resistant to the cytotoxic effects of chemotherapy, CSCs can self-renewal proliferate following a prolonged period of quiescence, and maintain tumor-initiating potential. Therefore, effective anticancer research strategies should target the unique properties of CSCs. But few of drugs were reported to target CSCs at present. The aim of this study was to inverstigate whether metformin, a first-line drug of choice for the treatment of type 2 diabetes, can target CSCs in Hepatocellular carcinoma (HCC). Methods Cells were cultured as spheres under stem-cell conditions. Proliferation Assay was used to detect the proliferation ability of CSCs.Flow cytometry was used to detect expression of CD133 antigens. Magnetic cell sorting was applied to enrich and purify CD133 populations. qrt-pcr was used to validate expression levels of stemness marker genes in Hepatocellular carcinoma cell lines. Hepatocellular carcinoma mouse xenografts were examined for their sensitivity to metformin. Results Metformin inhibited the formation of hepatospheres and epithelial-specific antigen-positive (ESA, CD133 + ) cell colonies by hepatocellular carcinoma (HCC) cell lines. Metformin also downregulated the expression of several CSCs-related genes which are involved in the signaling pathways governing the self-renewal, proliferation and differentiation of CSCs. Furthermore, metformin selectively target cancer stem cells both in vivo and in vitro. The PI3K/Akt/mTOR inhibitor LY abolished the inhibitory effect of metformin on CD133 + cells, and the PI3K/Akt/mTOR stimulator EGF promoted the inhibitory effect of metformin on CD133 + cells, this experiment indicated that PI3K/Akt/mTOR signal pathway with high activities in cells were more sensitive to metformin and PI3K/Akt/mTOR signal pathway with low activities in cells were III

10 二甲双胍在体内外靶向抑制肝癌 CD133 + 肿瘤干细胞 less sensitive to metformin. Metformin also dramatically decreased the tumor volume and number of CD133 expressing tumor cells in a xenograft mouse model. Metformin exerted a synergistic effect with cisplatin to target both CSCs and non-cscs, and result in the smallest tumor volume and lowest number of CD133 expressing tumor cells. Conclusions We found that metformin selectively targets liver cancer stem cells both in vivo and vitro. This study indicates that the antidiabetic drug metformin could potentially be used in combination therapy with chemotherapeutic agents to improve the treatment of liver cancer.
