There are cancer cell fragments around treatment target margin, cancer cells are completely necrotized in treatment area with distinct boundary. 原 因 为 : 对 肿 瘤 区 域 扩 大 丌 足, 某 些 细 小 分 支 未 包 含 在 靶 区 之 内, 应 通 过 乳 腺 增 强 核 磁 影 像 确 定 肿 瘤 组 织 大 小 和 形 态, 并 进 行 适 当 扩 大 The reason is due to inadequate expansion of the tumor area. We should enhance breast MRI images to confirm the size and shape of tumor tissue and expand it appropriately so that some tiny branches can be involved in the treatment area.

Massive hemorrhage, degeneration and necrosis is observed and scattered in fragment of chronic inflammatory cells as well as necrotic cancer cells. Most cancer cell nucleus has been broken and structure is not clear. In residual cancer cells, most karyon fine structure is not clear with cytoplasm being slightly red. All the above is degeneration changes. There is no obvious change in surrounding normal tissues, and sporadic chronic inflammatory cell infiltration can be observed.

The cancer bed field: part area shows large piece bleeding, degeneration and necrosis in mammary tissue. Lumen expands in local in which foamy tissue cells has gathered, surrounding has duct residual fragments as well as obvious degenerative atypical cells. No explicit cancer tissue is found.
患 者 3 Patient 3 周 围 组 织 : 皮 肤 未 见 癌 组 织 浸 润 累 及, 皮 下 局 部 导 管 上 皮 增 生, 间 质 及 导 管 周 围 散 在 慢 性 炎 细 胞 浸 润 ; 纤 维 血 管 及 脂 肪 组 织, 局 部 间 质 可 见 出 血 退 变 及 慢 性 炎 细 胞 浸 润 ; 乳 腺 增 生 症 伴 腺 病, 局 部 导 管 上 皮 增 生 伴 柱 状 细 胞 病 变, 个 别 扩 张 的 导 管 内 可 见 泡 沫 状 组 织 细 胞 聚 集 ; 部 分 导 管 和 小 叶 周 围 有 少 量 慢 性 炎 细 胞 浸 润 Surrounding tissue: infiltrating involvement of cancer tissue cannot be seen on the skin, subcortical ductal epithelial hyperplasia, chronic inflammatory cells infiltration scattered around mesenchyme and duct; bleeding, degeneration and chronic inflammatory cells infiltration could be seen in local mesenchyme around fibre, blood vessel and adipose tissue; cyclomastopathy with adenopathy, local ductal epithelial hyperplasia with cytopathy of columnar cell, foamy tissue cells has gathered in individual dilated duct; part of ducts and lobules are surrounded by a small amount of chronic inflammatory cell infiltration.

患 者 2 & 患 者 3 Patient 2 & Patient 3 结 论 Conclusion: 第 2 例 和 第 3 例 为 正 确 治 疗 后 的 结 果, 对 治 疗 靶 区 进 行 合 理 扩 大, 并 改 进 了 治 疗 点 排 布 方 案 根 据 正 确 治 疗 后 的 HE 染 色 病 理 结 果 显 示, 未 见 明 确 癌 细 胞, 只 见 少 量 癌 细 胞 残 影, 癌 组 织 被 完 全 杀 死 The second case and the third case are results after proper treatment, which has expanded the treatment area reasonably and increased treatment dose appropriately. The pathology result of HE staining shows cancer cells are ablated completely and only part of cancer cell fragments could be seen.
