4 摘 要 骨缺损修复是长期以来困扰外科医生的一个棘手难题 目前, 多采用自体骨嫁接或异体骨移植, 或者广泛采用金属 陶瓷 高分子材料以及复合材料等各种 人造骨替代材料的方法进行修复 虽然每一种材料及方法各有其特点, 但在生物 方面和力学功能上总是不尽人意 为了客服这些局限, 人们开始运用组织工程的 方法, 研究可用于骨缺损修复的人工支架材料 为了研制既有抗感染能力又有高效成骨活性的新型骨组织工程支架材料, 本研究以明胶和硅氧烷为载体, 复合庆大霉素, 制备了具有局部药物缓释作用的复合支架材料, 用于骨组织工程, 并对其体外生物活性和药物释放特性进行了研究 本文的主要研究内容和结论如下 : 1. 选择天然高分子材料明胶与 3-( 环氧丙烷丙基 ) 三甲氧基硅烷 (GPSM) 和 Ca 2+ 进行混杂, 采用溶胶 - 凝胶和冷冻干燥等方法, 制备了三维 多孔的明胶 - 硅氧烷复合支架材料, 并考查了其生物活性 结果表明 :(1) 复合 Ca 2+ 的明胶 - 硅氧烷复合支架材料具有良好的孔隙结构, 孔径为 100 μm 左右 ;(2) 可以通过预冷冻温度来控制支架材料的孔隙结构 ;(3) 具有较高的生物活性, 可望在骨组织工程中得到有效利用 2. 制备硫酸庆大霉素 - 明胶 - 硅氧烷复合支架材料, 通过对各组分含量的优化, 成功制备出了均相多孔载药支架材料 结果表明 :(1) 体外药物释放 24 h 内释药量大, 其后能维持一定时间的平稳释放 ; 有效浓度维持时间在 1 周左右 ; (2) 硫酸庆大霉素 - 明胶 - 硅氧烷复合支架材料有着良好的生物活性, 硫酸庆 大霉素的加入并没有降低材料的生活活性, 反而使之增强, 具有强大的诱导成骨作用 3. 采用沉淀法利用 GPSM 交联, 硫酸钠作为沉淀剂, 成功制备出硫酸庆大霉素 - 明胶 - 硅氧烷微球溶液, 并研究不同实验条件对其微球大小的影响, 通过透射电子显微镜研究了微球颗粒的粒径分布 形态 结构特征等 结果表明 :1) 微球颗粒粒径大小在 nm 呈规则球形;2) 操作简单方便, 而且微球在溶液中分布均匀 4. 制备复合缓释微球的明胶 - 硅氧烷 - 硫酸庆大霉素支架材料 用复合技术
5 将硫酸庆大霉素负载于微球溶液与聚合物的结合体等药物载体系统上, 对其的理化性能和生物活性进行测试, 研究其体外释放特性 结果表明 :(1) 可起到缓释作用 ; 释药速度降低, 缓释效果良好 ;(2) 具有良好的生物活性 总之, 本研究构建的明胶 - 硅氧烷 - 硫酸庆大霉素复合支架材料兼具抗感 染和成骨的双重作用, 能作为骨组织工程支架材料, 另外, 在开放性骨折骨缺损 的治疗中也具有良好的应用前景 关键词 : 骨组织工程 ; 支架材料 ; 药物缓释 ; 明胶 ; 硅氧烷 ; 硫酸庆大霉素
6 Abstract Bone defect repaired is one of the difficult problems for surgeon. At present, the transplantation of autogenous osteoblastes to regenerate bone tissue or more biomaterials including metal, ceramics material, macromolecule materials and their composites are widely used. But they cannot be satisfied with high mechanical strength and good bioactivity. To overcome the lack, scaffold materials of bone tissue engineering are more and more have been applicated in the defect repaired The aim of this thesis is to develop a novel kind of scaffolds for bone tissue which may not only be biocompatible and bioactive, but also prevent the bone infection. We loaded delivery microparticles systems into porous gelatin-siloxane hybrids, and prepared the scaffolds which are capable of controlled release of drug. The bioactivity and the drug release behaviors in vitro were also investigated. 1. The porous gelatin-siloxane hybrids were prepared by sol-gel method and lyophilization. The bioactivity as bone tissue engineering scaffolds was studied. The results show that the porous scaffolds had good network pore structures. The pore size of the hybrids could be well controlled by the freezing temperature. The gelatin-siloxane hybrids had good bioactivity. Thus, they may construct a promising scaffolds for tissue engineering. 2. Porous gentamicin sulfate-loaded gelatin-siloxane hybrids were successfully synthesized. The drug release of gentamicin sulfate(gs) was with a burst, followed by an almost constant release up to 7 days. In vitro bioactivity studies revealed that both gentamicin loaded and drug-free hybrids have bioactivity. Gentamicin sulfate- incorporation did not affect the bioactivity of the hybrids, but may enhance it instead. 3. The microparticles of gentamicin sulfate - gelatin-siloxane were prepared by chemical deposition methods. The influence of components and preparing conditions on the micromorphology of those microparticles was also investigated. The GSgelatin-siloxane microparticles have spherical shape with a size range of nm. 4. We loaded delivery microparticles systems into porous gelatin-siloxane hybrids. The bioactivity and the drug release behaviors in vitro were also investigated. The hybrid scaffolds were capable of controlled release of drug and had good bioactivity
7 Hence, it is expected that the gentamicin sulfate microparticles-loaded gelatin-siloxane hybrids will an ideal scaffold for bone tissue and as an antibiotic depot in the treatment and prevention of bone infection. 9 第一章文献综述 第一章 文献综述 1. 骨组织介绍 骨由骨组织 骨膜及骨髓等构成 骨组织是体内坚硬而有一定韧性的结缔组织, 在维持基体功能时有不可替代的作用 骨功能主要包括以下几个方面 : 保护体内器官 ; 运动时为肌肉提供附着部位 ; 产生红细胞和白细胞, 白细胞为其他组织提供免疫保护, 而红细胞与氧结合 ; 是钙 磷酸根以及其他重要离子的存储器 1.1 骨组织 (osseous tissue) 的结构骨与其他结缔组织基本相似, 也由细胞 纤维和基质三种成分组成 但骨的最大特点是细胞基质具有大量的钙盐沉积, 成为很坚硬的组织, 构成身体的骨骼系统 钙化的细胞间质称为骨基质 (bone matrix) 骨组织细胞有骨原细胞 成 骨细胞 骨细胞及破骨细胞四种 骨细胞最多, 位于骨基质内, 其余三种细胞均 位于骨组织的边缘 ( 图 1.1) 1
10 第一章文献综述 图 1.1 骨组织和各种细胞 骨基质骨基质即骨的细胞间质, 由有机成分和无机成分构成, 含水极少 有机成 分由成骨细胞分泌形成, 包括大量胶原纤维 ( 占有机成分的 95%) 及少量无定 形基质 无定形基质为凝胶, 内含中性或弱碱性糖胺多糖, 有粘着胶原原纤维的作用 基质中还含有两种钙结合蛋白 : 骨钙蛋白 (osteocalcin) 和骨磷蛋白 (phosphophoryms) 前者有两个与钙亲合力强的部位, 后者则有许多钙结合部位, 但只有一部分骨磷蛋白是可溶性的, 其余均与胶原纤维相结合 钙结合蛋白与钙化及钙的运输有关 无机成分又称骨盐 (bone mineral), 主要为羟磷灰石结晶 (hydroxyapatite crystal,(ca 10 (PO 4 ) 6 ( OH ) 2 ), 属不溶性中性盐, 呈细针状, 长 10-20nm, 沿胶原原纤维长轴规则排列并与之结合 有机成分与无机成分的紧密结合使骨十分坚硬 2
11 第一章文献综述 骨基质结构呈板层状, 称为骨板 (bone lamella), 成层排列的骨板犹如多 层木质胶合板 同一骨板内的纤维相互平行, 相邻骨板的纤维则相互垂直, 这种 结构形式有效地增强了骨的支持力 图 1.2 长骨骨干结构模式图 骨组织的细胞 (1) 骨细胞 (osteocyte): 单个分散于骨板内或骨板间 骨细胞是有许多细 长突起的细胞, 胞体较小, 呈扁椭圆形, 其所在空隙称骨陷窝 (bone lacuna), 突起所在的空隙称骨小管 (bone canaliculi) 相邻骨细胞的突起以缝隙连接相连, 骨小管则彼此连通 骨陷窝和骨小管内含组织液, 可营养骨细胞和输送代谢产物 ( 图 1.3) 骨陷窝周围的薄层骨基质钙化程度较低, 并可不断更新, 在机体需要时, 骨细胞的溶骨作用可溶解此层骨基质, 使 Ca 2+ 释放入骨陷窝的组织液中, 继而进入血液, 对维持血钙的恒态水平有一定作用 3
12 第一章文献综述 图 1.3 骨细胞超微结构模式图 (2) 骨原细胞 (osteogenic cell): 是骨组织中的干细胞, 位于骨外膜及骨内膜贴近骨处 细胞较小, 呈梭形, 核椭圆形, 细胞质少, 弱嗜碱性 当骨组织生长或改建时, 骨原细胞能分裂分化为成骨细胞 ( 图 1.1) (3) 成骨细胞 (osteoblast): 分布在骨组织表面, 成年前较多, 常排成一层, 成年后较少 成骨细胞是具有细小突起的细胞, 胞体呈矮柱状或椭圆形, 其突起常伸入骨质表层的骨小管内, 与表层骨细胞的突起形成连接 核圆形, 多位于细胞的游离端 ( 图 1.1) 胞质嗜碱性, 电镜下可见大量粗面内质网和发达的高尔基复合体 成骨时, 成骨细胞分泌骨基质的有机成分, 称为类骨质 (osteoid), 同 时以类似顶浆分泌的方式向类骨质中释放一些小泡, 称基质小泡 (matrix vesicle) 基质小泡直径约 0.1 μm, 有膜包被, 膜上有碱性磷酸酶 焦磷酸酶和 ATP 酶, 泡内含钙和小的羟磷灰石结晶 一般认为, 基质小泡是使类骨质钙化的重要结构 近年发现, 骨基质中的钙结合蛋白均由成骨细胞分泌产生 当成骨细胞被类骨质包埋后, 便成为骨细胞 (4) 破骨细胞 (osteoclast): 主要在骨组织表面, 数目较少 破骨细胞是一种多核的大细胞, 直径约 100 μm, 含有 2-50 个核 目前认为它由多个单核细胞融合而成, 无分裂能力 光镜下, 破骨细胞贴近骨基质的一侧有纹状缘 ( 图 4
13 第一章文献综述 1.1), 胞质呈泡沫状, 在 HE 染色的切片中胞质易为伊红染色 电镜下可其贴近骨基质一侧有许多不规则的微绒毛, 称为皱褶缘 (ruffled border)( 图 1.4), 即光镜下的纹状缘 在皱褶缘的周缘有一环形胞质区, 内有多量微丝, 而无其它细胞器, 称为亮区 (clear zone) 亮区的细胞膜平整并紧贴于骨基质表面, 形成一道 环形胞质围墙, 使所包围的区域成为封闭的微环境区 破骨细胞功能活跃时, 向 此区释放多种蛋白酶 碳酸酐酶 乳酸及柠檬酸等, 在酶及酸的作用下使骨基质 溶解 皱褶缘可增大吸收面积, 电镜下可见皱褶缘基部有吞饮泡和吞噬泡, 泡内 含小骨盐晶体及解体的有机成分, 表明破骨细胞有溶解和吸收骨基质的作用 2. 骨缺损与修复 图 1.4 破骨细胞超微结构模式图 骨不仅坚硬且具一定弹性 这些物理特性是由它的化学成分所决定的 骨是由无机材料和有机材料巧妙结合在一起形成的复合体, 有机质由骨细胞分泌产生, 约占骨重的 1/3, 其中绝大部分 (95%) 是胶原纤维, 其余基质, 即中性或弱酸性的糖胺多糖组成的凝胶 无机质主要是钙盐, 约占骨重的 2/3, 主要成分为 5
14 第一章文献综述 羟基磷灰石结晶 [HA, Ca 10 (PO 4 ) 6 OH 2 ] 有机质与无机质的比例随年龄增长而逐渐变化, 幼儿骨的有机质较多, 柔韧性和弹性大, 易变形, 遇暴力打击时不易完全折断, 常发生柳枝样骨折 老年人有机质渐减, 胶原纤维老化, 无机盐增多, 因而骨质变脆, 稍受暴力则易发生骨 折 骨具有再生和自修复能力, 但如果对于由肿瘤 外伤 骨疾及骨异常生长所 造成的骨缺损, 在单纯依靠骨的自修复无法愈合的情况下, 则需要采用外科手术 治疗, 骨缺损的质量是长期困扰外科医生的一个棘手难题 骨缺损的修复实际上是一个骨再生的过程, 在理想情况下, 当骨修复完成后, 常无瘢痕残留 目前多采用自体骨嫁接或异体骨移植, 或者广泛采用金属 陶瓷 高分子材料以及复合材料等各种人造骨替代材料的方法进行修复 临床已经证明, 自体骨移植是治疗骨缺损的最佳方法, 但其来源极其有限, 而且取骨区有一定的并发症 异体骨移植有着优越的组织学特点, 但存在免疫排斥反应, 容易感染病毒, 而且制样 处理和存储的成本相对较高 而各种人工骨替代材料主要品种有不锈钢 钛合金 氧化铝陶瓷 生物活性玻璃 羟基磷灰石陶瓷 聚甲基丙烯酸甲酯骨水泥 聚 α- 羟基酸 生物复合材料等 虽然每一种材料及方法各有其特点, 但在生物方面和力学功能上总是不尽人意 为了客服这些局限, 人们开始运用组织工程的方法, 研究可用于骨缺损修复的人工材料 3. 骨组织工程学简介 组织工程是近年来随着现代生物材料技术和细胞生物技术等多种新兴技术的发展而出现的多学科交叉的一门边缘学科, 形成于 20 世纪 80 年代, 其基本含义是运用生命科学和工程学的方法和技术, 在认识正常和病理两种状态的组织结构和功能关系的基础上, 研制用于修复维持或促进组织器官损伤后的功能和形态恢复的生物替代物的一门新学科 其优点在于可形成具有生命力的活体组织, 对病损进行形态 结构和功能的重建并达到永久性替代 ; 可以最少量的组织细胞修复大块的组织缺损, 实现以微创修复创伤和功能重建 ; 并可按组织缺损情况塑形, 6
15 第一章文献综述 达到形态完美修复 图 1.5 组织工程的基本原理骨组织工程学就是利用组织工程学进行骨组织缺损的修复或重建, 是继软骨组织之后研究得较早 较多的对象 1993 年,Kizuhito 等将软骨细胞种植于羟基磷灰石人工骨上培养, 然后植入体内桥接骨缺损获得成功 同年,Vacanti 等将小牛骨膜细胞种植于聚羟基乙酸支架中, 然后移植于裸鼠体内, 结果证实骨细胞可以增殖成为骨骼, 形成了新生骨 [1] 1995 年,Crane 等全面提出了骨组织工程研究的概念 方法 现状和前景, 引起了广大学者的关注 [2] 骨组织工程的概念也因此得到了建立和发展, 此后十几年内, 骨组织工程在基础研究和临床应用方面都取得了很大的进步 采用骨组织工程技术修复骨缺损与其他骨移植 ( 自体骨 异种骨 异体骨 ) 相比有以下优点 : 需要供体组织少 ( 细胞可在体外培养 增殖 ), 供给来源丰富, 供体损伤小 ; 无抗原性或者抗原性甚微 ; 可根据修复缺损的需要将植入物制成精 确的三维形状 ; 可利用仿生涉及技术设计出与天然骨相似的组织工程人化骨, 为 大范围骨缺损的修复提供新的途径 ; 组织工程人化骨具有生命力, 可建立活骨移植, 缩短骨缺损的修复时间提高骨缺损的修复质量 和其它组织的组织工程研究原理和方法一样, 骨组织工程的研究主要也是集中在可用于骨组织修复的种子细胞 支架材料 ( 细胞外基质 ) 和骨的构建三个方面 骨的组织工程学要求被种植的种子细胞必须是有成骨潜能的细胞 目前已分别有研究利用骨外膜 松质骨 骨髓 骨和骨外组织来源的成骨细胞及软骨细胞作为骨组织工程学的种子细胞 骨髓的成骨能力来源于骨髓基质系统的基质干细 7
16 第一章文献综述 胞, 它能自动分化为成骨细胞, 即使在体外脱离诱导物的培养条件下也能成骨, 在有诱导物如骨形态发生蛋白 (BMP) 存在时可促进其向骨形成方向分化 骨髓来源的成骨细胞一般是采用骨髓穿刺 抽吸, 得到骨髓基质细胞, 经体外培养, 诱导 扩增, 可获得大量成骨细胞 国内外一些学者的研究表明, 二甲胺四环素 锌离子 转化生长因子 β(tgf β) 胰岛素样生长因子 Ⅱ(IGF Ⅱ) 碱性成纤 维细胞生长因子 (bfgf) 血小板衍化生长因子 (PDGF) 基因重组人骨形成蛋 白 2(rhBMP 2 ) 及表皮生长因子 (EGF) 等均可促进骨髓基质细胞向成骨细胞 分化和生长 骨组织工程研究重点是寻求能够作为细胞移植与引导新骨生长的人工合成与天然的支架结构材料, 作为种子细胞外基质的替代物 支架材料不仅影响种子细胞的生物学特性和培养效率, 控制组织或者器官的形状, 而且还决定移植后能否与受体很好地适应并结合在一起, 从而发挥其修复骨缺损的作用 因此, 在组织工程中支架材料的选择十分重要, 在目前的骨组织工程研究中, 寻找理想的支架材料也是一大热点 4. 骨组织工程支架材料理想的组织工程支架材料应具备以下几个条件 :(1) 具有良好的生物相容性和细胞相容性, 在体外培养时无细胞毒性, 植入体内时不会引发机体炎症和排斥反应 ;(2) 具有可降解性和适宜的降解速度, 并不影响新生组织的结构和功能 ; (3) 呈良好的三维结构和孔隙率, 有利于细胞的植入 粘附, 又有利于细胞营 养成份的渗入和代谢产物的排出 ;(4) 具有可塑性, 可被加工成需要的形状并且具有一定的机械强度, 符合细胞 组织 器官生物力学要求的强度 4.1 支架材料的分类 目前, 骨组织工程支架材料大体分为两类 : 一类是天然生物材料, 由天然生物组织经一系列理化方法处理而得 有胶原 明胶 珊瑚 藻酸盐 几丁质 氨基葡聚糖, 以及脱钙骨基质和经物理 化学及高温处理的动物骨等 ; 另一类是目前大多使用的人工合成材料, 采用人工技术加工合成 如 : 钙磷陶瓷 生物活性 8
