Passport & Visa Services China Visa Application Cheat Sheet The following is a guide to completing the China visa application. The application is a fillable PDF. Please note after clicking the first field on the application, it may take a moment for the form to respond. All questions which are left blank MUST be completed with the answer "N/A". Incomplete or inaccurate answers will result in rejection of application and delayed processing. # Question Answer 1.1 Surname; Given name Last name; First name 1.3 Name in Chinese characters Must be completed by anyone of Chinese, Taiwanese, Hong Kong or Macau decent. May not be completed by hand - must be digitally input. 1.5 Name in ethnic script Only applicable to those of non-english/non-chinese based alphabetic languages. China Visa Application (Cheat Sheet) 1.7 Former nationality For any applicant of Chinese, Taiwanese, Hong Kong or Macau decent, applicant MUST enter "PR China". 1.10 Place of birth For any applicant of Chinese, Taiwanese, Hong Kong or Macau decent, applicant MUST enter "City, Country, PR China". 1.11 Local ID card number Only applicable to former Chinese nationals 1.14 Passport type Generally 'Ordinary' 1.17 Place of Issue (passport) Refer to the information page of the passport "Place of Issue" or "Issuing Authority". Some US Passports will list a specific location, for most it will list Issuing Authority: US Dept Of State. 2.5 Residence...in China For tourist list hotel; For business list office address of inviting party 2.6 Who will pay... For tourist, 'Yourself' For business, mark "Other" and insert company name 2.7 Medical insurance May be "N/A", but should be completed if applicant has health insurance. 2.8 Name, address...inviter For tourist, "N/A" For business, list office address of inviting party (same as 2.5) 2.9 Name, address...friend Generally "N/A" unless staying at family/friends house. 3.6 Major family members Either co-habitants (husband, wife, children) or parents, other family members.
