Objectives Definition and Operationalization Age-related physiological decline and secondary causes frailty Evaluation and management Frailty in Long-term care patients.
Definition and Operationalization
Definition and Operationalization Frailty is a dynamic state affecting an individual who experiences losses in one or more domains of human functioning (physical, psychological, and social), which is caused by the influence of a range of variables and which increases the risk of adverse outcomes. Gobbens, et al 2010 Nurs. Outlook 58, 76 86. Count deficits in health (which can be symptoms, signs, diseases, disabilities or laboratory, radiographic or electrocardiographic abnormalities) Searle SD et al BMC geriatrics. 2008;8 24.

Sarcopenia Decline in muscle mass, starting at 35 y/o (selective atrophy of type II muscle fiber): decline 15% from 30-70 yr Muscle strength decline: 15%/decade in 60-70 yr, 30%/decade after 70yr Lean body mass replaced by fibrotic and/or adipose tissue Impact on physical function: decreased strength, weakness, fatigue, exercise tolerance, poor balance, slowed gait, and falling

Immune Dysfunction elevated catabolic cytokines generalized inflammation Increased CRP Increased level of auto-ab not related to identifiable autoimmune dz unknown significance Increased levels of catabolic cytokines: IL-6, IL- 1B, and TNF-a catabolic influence on muscle (sarcopenia) stimulation of adrenocortical axis: neuroendocrine dysregulation development of disability and mortality
Neuroendocrine Dysregulation Stress Response Hormones Growth Hormone Estrogen and Testosterone DHEA
Neuroendocrine Dysregulation stress responses hormones Higher baseline sympathetic nervous system activity seen with age Release of epinephrine, NE, and cortisol Increased activity of the locus ceruleus (brain center for sympathetic system) elevate CRF increase cortisol
Neuroendocrine Dysregulation growth hormone GH/IGF-1: growth of immature organism, maintenance of lean body mass Aging loss of pulsatile GH secretion Impaired GH secretion decline body mass and bone mass
Neuroendocrine Dysregulation estrogen and testosterone Female: menopause, decline in BMD, loss of lean body mass, increased fat mass, increased CV risk Male: decline in testosterone, increasing sensitivity of negative feedback effects of testosterone on the hypothalamus Both: decline in lean tissue/sarcopenia
Neuroendocrine Dysregulation DHEA (dihydroxyepiandrosterone) DHEA: secreted by adrenal cortex (starting 5 yrs), peaked at age 20 Maintain lean body mass and immune function, suppress IL-6/IL-1B, induce secretion of IL-2
Potential Molecular Mechanisms Predisposing to Frailty Cumulative oxidative damage to DNA Mitochondrial DNA deletions/mutations Replicative senescence -- telomere shortening Protein alterations -- glycation -- oxidation

Evaluation and Management
Comprehensive Geriatric Assessment Multidisciplinary diagnostic process to determine a frail elderly person s medical, psychosocial, and functional capabilities and limitations Develop an overall plan for treatment and long-term follow-up
Principles of Symptom Assessment and Management in Frail Older Adults Perform regular, routine, and comprehensive symptom assessment Screen all frail older adults for pain, weight loss, weakness, fatigue, dyspnea, nausea, constipation, insomnia, depression, anxiety Assess interference of symptoms with activities of daily living
Principles of Symptom Assessment and Management in Frail Older Adults Identify underlying conditions that may be contributing to symptoms Increase clinician accessibility with more frequent or longer physician contacts Nurse contacts Home visits Telephone contacts and telecare
Management Treat underline medical diseases and/or neuropsychological problems Social support Nutritional supplement Rehabilitation and exercise Hormonal therapy

Take Home Messages Many definitions and operationalizations were available. Core concept accumulations of deficiencies and lost of reserves with increased vulnerability Frailty is not equal to disability! Frailty and sarcopenia shared many similar attributes including Low gait speed, decreased mobility Poor muscle strength
Take Home Message Evaluation: CGA Management (Interdisciplinary approach) : Underline dz/ nutrition/ rehab/exercise Frailty assessments can also be applied to long-term care settings. Prevalence higher than commnity Still predict poor outcomes
