患者對配戴 provisional denture 的外觀滿意最終治療計畫 Maxilla 22 23 25 ext Cement retained Implants support FPD Implants: 11 13 14 17 21 23 24 26 FPD: 13X1121X23 14X16 24XX 27 Mandible 34 35 crown Onlay bone graft for 45 46 area Cement retained Implant support FPD Implant:36 37 45 46 FPD:3637 4546 手術模版評估及製作過程利用 ortho resin 翻製上顎假牙作為 Nobel guide 拍攝 CT 用的 stent, 如此 stent 便為假牙外型, 利用 GP 作 8 個定位點, 讓患者裝戴拍攝 CBCT, 另外對 stent 單獨拍攝 CT, 再用 Nobel guide 設計軟體組合患者上顎骨與 stent 利用 tempron 複製出原本假牙外形, 製作臨床上接近固定假牙前牙外形的工具, 讓患者配戴用以模擬患者前牙因為 fix 沒有 flange 時的 lip support

利用 esthetic try in 工具, 評估沒有 flange 的 lip support 利用 Nobel guide 軟體設計植牙位置讓患者戴 stent 拍攝 CBCT, 以及讓 stent 單獨拍攝 CBCT 將 double scan 的資料在電腦上進行組合, 並用 Nobel guide 軟體評估設計植牙位置

手術模版與螺絲固定臨時植體贋復物製作確認設計無誤後製作出 Nobel guide 後, 將 Nobel guide 固定在利用 stent mounting 的咬合器上, 利用 heavy body 製作 bite, 用以定位 Nobel guide, 臨床操作時先利用這個 bite 將 Nobel guide 固定在上顎, 再開始打固定 Nobel guide 的 anchor pin 將套件固定在 Nobel guide 上, 倒出模型, 得到的模型內有 implant replica 為設計的植體位置,

再將模型利用 bite mounting 在咬合器上利用 Nobel guide 倒出的模型選擇臨時假牙的 abutment 並將 esthetic try in 的工具修改成 screw retained temporary prosthesis, 在口內 pick up 植體臨時假牙 abutment 下顎植牙及補骨手術

取右下 Ramus 的 bone block 以 screw 和 mini implants 固定在 45 46 缺牙區, 等待 healing 半年 3 周後 mini-implant 裝上 metal housing, 假牙 re-line 後佩戴 function 利用 Nobel guide 進行上顎植牙手術利用 bite 固定 Nobel guide 後鑽入 3 支 anchor pin

依序使用套筒及鑽針擴大植入位置並鎖上植體對即拔即植的 23 implant 在取下 Nobel guide 後進行 GBR 術後放射線檢查

螺絲固定臨時植體贋復物裝戴 2 周後給予 screw reatined temporary FPD, 由 11, 13, 14,16,24, 27 support,23 暫時 submerged, 接上臨時假牙 abutment 的植牙, 再將先做好的假牙在口內 pick up 放射線檢查

A:Nobel guide 的製作設計流程, 利用 acrylic resin 複製出理想的假牙外型作為 stent, 不必加入像是硫酸鋇的 radiopaque 的材料, 而是利用不同高度的 6~8 個 1.5X1 mm 的 GP 做為 double scan 影像組合的定位點, 患者配戴 stent 拍攝 CBCT, 假牙外型的 stent 亦單獨拍攝的 CBCT, 利用 GP 定位將兩者在電腦軟體內組合, 設計時可以看出假牙外型和相對應的植牙條件 Q: 這個 case 使用了 Nobel guide, 術前應可理想設計出植牙的位置, 但是仍有部分植體的位置看來是種在接近牙齒 embrasure 的位置, 設計時能否避免? A: 使用 Nobel guide 可以在設計時清楚看到將來假牙位置與相對應骨頭條件可斟酌設置植體位置, 不過這個患者有些缺牙區缺牙較久, 特別是右上缺牙區骨頭條件較差且患者不同意在 GA 下進行較大的 bone graft 將 ridge 重建至較理想的寬度, 於是在部分植牙位置斟酌骨頭條件而沒有辦法將植體設計在最理想的位置, 也必需將兩相鄰植體間隔至少 3mm 的水平距離而作調整

Q: 此 case 用了 Nobel guide, 假牙也預先做好,2 周後給予 early loading, 有考慮術中同時給予 immediate loading 嗎? A: 手術當天因操作經驗然不足, 種植顆數較多, 雖用 Nobel guide 減少許多操作時間, 惟植牙後 23 即拔即植區域要立即 GBR, 因時間的關係沒有給予 immediate loading, 而在拆線後組織愈合良好後給予 early loading Q: 在 crew retained umplant supported temporary FPD 因為牙齒長度較長而加上粉紅色的牙肉色 acrylic resin, 同時部分植體位置斟酌骨頭條件而不能放在最理想的地方, 有考慮在這個 case 選擇 hybrid denture 的 design 嗎? A: 這個 case 右上缺牙較久,vertical space 較大, 假牙做起來比較長而在 papilla 加入牙肉色 acrylic resin 讓牙齒比例看起來比較好看, 惟在其他缺牙區 vertical space 約 12mm, 若選擇 hybrid denture 的 design 空間可能不足 Q: 使用 mini implant 固定 bone block 且給予 loading 可能造成 bone graft 的 mobility 影響 bone 的愈合與生長, 建議 bone block 區域應不給予 compression, 且某些廠牌的 mini implant 有表面處理具可骨整合, 在取出的時候可能斷掉
