47 1/F and intended to transport bamboo poles from ground to the 1/F. While he was transporting the bamboo poles, he fell from the scaffold. He was rushed to hospital for treatment but he was certified dead on the same day. Circumstances The accident happened at a threestorey high clubhouse where external wall painting was to be carried out. To facilitate the painting work, double row bamboo scaffolds were to be erected. On the day of accident, a single row bamboo scaffold had been erected up to the 3/F of the building. The deceased was assigned to assist the erection of the outer row of the scaffold. Before the accident occurred, he was seen climbing up the single row bamboo scaffold to the 1/F. He stayed at the 7th or the 8th ledger of the scaffold to transport bamboo poles from the ground level to the planting area on the 1/F of the building s external wall. The bamboo poles were placed vertically on the floor and leaned against the scaffold. At time of the accident, the deceased was working alone. He fell a distance of 4.5m down to the ground while he was working, sustained serious head injuries and was certified dead at a hospital that same day. Findings and Observation 1. At where the accident happened, approximately 20 bamboo poles were placed vertically at ground level and leaned against the bamboo scaffold. Some bamboo poles were placed vertically on planting areas on the 1/F and 2/F of the building s external wall. Each bamboo pole was 6.7m long and 45mm in diameter. 3/F with knots. Each independent lifeline was approximately 3m apart. The nearest independent lifeline from where the deceased worked was 3.2m away. 3. The deceased had worn a safety harness that was equipped with a fall arrestor. However, he did not attach his harness to any of the 4 independent lifelines as his lanyard was only 2m long and the nearest independent lifeline was 3.2m away from him. 4. No metal scaffold or working platform was provided for works at the height of the accident scene. No safety net was erected underneath where the deceased person was working to catch him in case of fall. Prevention recommended 1. Adequate steps should be taken to prevent any person on the site from falling from a height of 2 metres or more. Steps should include, but not be limited to, the following: (a) A risk assessment should be conducted prior to the commencement of the erection of bamboo scaffolds. Safe working procedures should be developed and implemented. (b) A sufficient number of independent lifelines fixed to suitable anchors should be provided and maintained close to the area where works at height are carried out. working at height have worn suitable fall arresting equipment which are attached to an independent lifeline throughout the duration of the work. Legislations applicable 1. Construction Sites (Safety) Regulations, Regulation 38B(1) The contractor responsible for any construction site shall take adequate steps to prevent any person on the site from falling from a height of 2 metres or more. 2. Construction Sites (Safety) Regulations, Regulation 38B(1A) Any contractor who has direct control over any construction work shall take adequate steps to prevent any person on any place where the construction work is being carried out from falling from a height of 2 metres or more. 2.
