4 摘要 摘要 随着生活水平的提高, 口腔健康教育宣传的开展, 越来越多的人意识到 洁牙的重要性 目前, 临床上大多数医院采用超声波洁牙, 但它工作时所发 出刺耳的声音, 会给患者造成一种心理上的恐惧感 激光清洗技术作为一项新兴清洗技术, 已成为传统清洗方法的补充和延伸, 因其固有的优点, 而在工农业生产 科研 医疗 国防建设等方面得到广泛的应用, 激光洁牙设备将成为现代牙科必不可少的工具 与超声波洁牙等技术相比, 激光洁牙属于非机械接触, 不存在超声波洁牙所具有的振动噪声, 不易损伤牙齿表面釉质, 不损伤周边组织 按现有的牙科医疗器械, 患者是无法了解自己口腔内牙齿的受损程度, 是否接受治疗则完全交由医疗人员决定, 而且现有牙科医疗器械还无法为医疗纠纷提供原始医疗资料 ; 随着各大医疗机构完成对药品及费用结算的数字化管理, 实现医疗资料的数字化管理已成为发展趋势 基于此, 本课题研究的主要内容如下 : 1 根据激光光声清除的机理, 确定激光洁牙系统的光路布局, 并由此研制出一台激光洁牙用的样机 2 采用成像系统来监测清洗部位, 并记录洁牙过程 防止激光对医疗人 员眼睛造成损害, 便于医疗人员的操作 3 对洁牙有关的资料进行行管理, 用户据此可查询患者的记录, 观看任 一时刻的洁牙影像和图片 本研究工作的主要特色是 : 1 采用可见光的同轴定位指示, 方便了样机的装调及操作, 并介绍了一种简单的同轴指示的调试方法 2 系统利用视频摄像技术, 对洁牙过程进行监测 医疗人员通过观察显示屏, 可方便地执行洁牙操作, 保护医疗人员眼睛免受激光伤害 I

6 ABSTRACT ABSTRACT As life s quality improved, oral health propagandized, the importance of teeth cleaning is attracted more and more people s attention. In clinic, ultrasonic wave is used to clean the tooth, but the noise from the ultrasonic wave would lead a dread to the patients. As a new cleaning technology, Laser cleaning play the role of complementarity and extendability of traditional cleaning. Due to its inherent advantages, Laser has been widely used in industry, agriculture, scientific research, medical treatment, national defence etc. Laser s equipment would become an indispensability in modern dentistry. Compared with ultrasonic wave, Laser has no mechanical contact, has no such noise during cleaning tooth, and does no harm to enamel and tissues around tooth. According to current dentistry equipment, Patient couldn t know exactly on what degree his tooth is damaged, and only depend on dentist to decide whether to have his tooth cleaned. Moreover, the equipment couldn t provide necessary information in case of medical treatment dissension. And as the digital management on medicament and fee came true, the digital medical treatment management would be the development trend. As mentioned above, primary research woks in this thesis are as follow: 1. According to the principle of Laser cleaning, a Laser cleaning tooth sample is developed, and its optic layout is determined. 2. An Imaging system is designed to record the process during the cleaning, which makes dentist easy to observed the cleaned parts, protects his eyes from being hurt by Laser, and helps dentist to carry out operation conveniently. 3. A system is built for managing the data related to Laser cleaning tooth. III
7 激光洁牙图像采集与数据管理系统的研究 So, user can easily find out the record he wants, and view any video or picture in the record if permitted. The follows are the main feature in the thesis: 1. In order to fix machine and carry out operation more easily, a visible light s coaxial orientation for YAG Laser is adopted, and also an simple and convenient method for fixing is offered. 2. Image Acquisition technologies are used in the system to inspect the process of cleaning. As for that, dentist carries out operation conveniently depend on the display, avoids his eyes being hurt by Laser 3. The data management technology is also introduced in the thesis. So it could provide necessary information in case of medical treatment dissension, and the digital medical management is followed to the trend of the digital management development. 12 第一章绪论 第一章绪论 随着生活水平的提高, 口腔健康教育宣传的开展, 越来越多的人意识到 洁牙的重要性 目前, 临床上大多数医院采用超声波洁牙, 其原理 [1] 是由超 声波发生器产生 28~32kHz 超声频率的电脉冲波, 经超声波换能器, 转换为 微幅机械伸缩振动, 激励工作头产生相同频率的超声振动 同时, 喷出的水, 受超声波振动, 水分子破裂, 出现无数气体小空穴, 空穴闭合时产生巨大的 瞬时压力, 迅速击碎牙石 松散牙垢, 达到洁牙的目的 超声波洁牙其特点是操作时间短, 患者创伤小 但由于超声波洁牙属于 接触清洗, 所具有的超声震动会对釉质造成刻痕, 洁治时需要仔细操作, 确 保超声振动振击在牙石和烟斑上, 而不宜直接作用在牙釉质或牙骨质表面, 更不能将工作头停留在一点上震动 ; 在洁牙过程中, 一些敏感患者的牙颈部 薄弱区域如果受到超声波洁牙机工作头的振动刺激, 洁牙后会有酸痛不适感, 一般经 3-5 天后, 才会慢慢的减轻或消失 ; 而且洁牙操作时机器会发出刺耳 的声音, 给患者造成一种心理上的恐惧感 ; 同时, 许多患者在治疗中接触超 声波器械刺激时, 常易引起牙本质过敏症状 [2] 激光清洗技术作为一项新兴清洗技术, 能适应各种表面污物的清洗, 也 不损伤基体, 已成为传统清洗方法的补充和延伸, 因其固有的许多优点, 而 在工农业生产 科研 医疗 国防建设等方面得到广泛的应用, 激光牙科设 备已成为现代牙科必不可少的工具 与超声波洁牙相比, 激光洁牙具有如下 的优点 : ⑴ 无接触 激光洁牙是以光的形式传递能量, 无需机械接触, 这样就可以清洗非常 脆弱的材料的表面 由于不存在超声波洁牙的所具有的高频振动, 消除了洁 牙后的不适感 ⑵ 可以准确定位 1
13 激光洁牙图像采集与数据管理系统的研究 激光束可以准确定位在欲清洗的材料表面上, 其光斑面积可以从零点几毫米到 1 厘米之间进行调整, 因而可以清洗牙齿上不规则的或者比较隐蔽的表面 ⑶ 可以实时反馈 通过 CCD 摄像和探测光实时监测材料表面反射率或者激光引起的表面 声波, 来判断清洗效果 根据清洗的效果, 在清洗过程中, 可以随时关闭激 光电源终止清洗 目前, 国内外有关的激光用于洁牙的文献报道 [3][4][5][6][7], 处于实验研究阶段 本课题是采用已有的激光光声清除机的清除技术, 对激光清除技术进行二次开发, 研制利用激光来清除牙垢的激光洁牙系统 其主要工作原理是 : 采用调 Q 的激光束击穿空气 产生超声冲击波清除物体表面污垢杂物, 即采用光声效应清除污垢 具有非烧灼 非机械式接触清洗的特点 由于采用调 Q 脉冲激光束, 强烈的超声冲击波与物体表层的相互作用是短暂的, 不会破坏基质外表层, 不留任何痕迹 清除污垢过程, 不采用连续光波的直接照射, 而是经过光声能量转换, 由超声冲击波清除污垢, 故对所要清除物体的污垢基本上是不加选择的 考虑到激光能量会伤害人的眼睛, 医疗人员操作时需戴防护镜, 无形中会增加非正常视力 ( 如近视 远视等 ) 医疗人员操作难度 因此, 课题中采用了视频监控技术, 使系统能实时记录 显示洁牙过程, 实现医疗资料的数字化管理 这样医疗人员通过观察系统显示屏方便地执行洁牙操作, 保护了医 疗人员眼睛免受激光伤害, 而且, 还解决了传统激光牙科器械无法为医疗纠 纷提供原始医疗资料的问题 2
14 第二章激光洁牙机理及光学系统 第二章激光洁牙机理及光学系统 脉冲激光洁牙的机理主要是垂直于物体表面的冲量与平行物体表面的机 械作用力共同作用的结果 本课题所研制的激光洁牙机由光学部分 冷却部 分 电源控制部分以及成像部分等组成, 系统设置了 YAG 激光的可见光同 轴指示, 以利于仪器的装调 维修, 并在清洗工作头内对激光束进行聚焦, 以获得清洗所需的能量密度 2.1 激光的特性激光是英文 Light Amplification by Stimulated Emission of Radiation 词头的缩写而成的, 其含义是 受激辐射产生的光放大 激光同电子束和离子束统称为高能束 共同特点是束流携带着很高的能量在空间传输, 通过聚焦, 在焦点附近可得到 10 4 ~10 15 W/cm 2 功率密度的照射, 是强度最高的热源 激光是在时间上高度集中的光束, 同无线电波一样, 都是电磁波 除了具有普通光源的一般性质外, 由于采用了光学谐振腔, 还具有如下特性 : [8][9] ⑴ 高亮度激光器的发光截面和立体分散角都很小, 而输出功率却很大 发光功率仅为 10mw 的 He-Ne 激光, 可比太阳的亮度高几千倍, 所以说激光是能量 高度集中的光束 当物体被照射时, 就是接受能量的辐照, 如果采用透镜将 激光束聚焦到被射物体, 光斑缩小, 将可进一步提高单位面积的辐照功率密度 ⑵ 高方向性激光的高方向性主要是由受激发射机理和光学谐振腔对振荡光束方向的限制作用所决定的 如单模 He-Ne 激光器输出的光束, 其发散角 2θ 2' rad 激光的高方向性使得激光能有效地传递较长距离, 能聚焦到极高的功率密度 3
15 激光洁牙图像采集与数据管理系统的研究 ⑶ 高单色性激光器发出的全部光辐射只集中在很小的频率范围内, 其单色性极高, 几乎完全消除了聚焦透镜的色散效应, 使得光束能精确地聚焦到焦点上, 得到很高的功率密度 ⑷ 高相干性 因相干性好, 在较长时间内有恒定的相位差, 可以形成稳定的干涉条纹 激光清洗就是激光束高亮度 ( 高功率 ) 高方向性特性的一种技术应用 2.2 激光清洗技术原理连续波激光束的清除污垢的机理是主要利用激光的热效应, 使污垢燃烧 汽化达到清除目的, 而用脉冲激光束进行物体表面污垢清除机理与此截然不同 后者调 Q 脉冲激光的除垢机理相当复杂, 它涉及到空气动力学 等离子体 物理热传导 非线性光学 爆炸动力学等学科 要完善其理论模型须考虑包括激光本身特性 介质特性在内的诸多方面因素, 根据初步的理论探讨与实验观察, 认为该激光的除垢过程主要由以下几个方面组成 : ⑴ 高能量密度激光对空气实行光学击穿, 产生等离子体, 进而产生爆轰冲击波 将物体污垢表面位于激光束的新束腰附近 在焦点区域内的空气吸收激光能量, 温度急剧上升, 导致光学击穿, 并伴有火花现象 这里主要有两个过程, 一是能量吸收过程, 二是电离过程 该区域内原有存在的少量自由电 子与高能光子进行能量交换, 获取能量后的电子进一步碰撞原子或分子, 实 现对原子 ( 或分子 ) 的电离 如此下去, 在激光脉冲作用时间内, 电子数量按 指数增长, 形成所谓 雪崩电离, 从而形成高温高密度的等离子体 等离 子体迅速膨胀, 激光功率密度大于 10 8 W/cm 2 并以 10 4 m/s 的速度向外扩张, 我们称之形成了爆轰超声冲击波 如图 2-1 所示, 当功率密度为 w/cm 2, 波长 1.06μm 激光束作用在铝板锈垢时, 延迟时间为 30ns 时, 冲击波速度约为 9km/s, 约为声速的 27.8 倍, 即达到超声速度 ; 当延迟时间为 200μs 时, 冲击波速度为普通声速速度 4
16 第二章激光洁牙机理及光学系统 ⑵ 爆轰超声冲击波一方面作用于物体污垢上, 传递给污垢约为 10-5 N S 以上的冲量 另一方面, 在逆激光入射方向具有对激光能量强烈的吸收, 使等离子体区继续膨胀, 在冲击波前形成高压高密度区, 同时对激光能量起了离子屏蔽作用, 使后续激光能量不能达到物体表面 图 2-1 等离子体冲击波 V 与延迟时间 t 的关系曲线图此外, 冲击波的等离子体区内存在一种稀疏波, 它与冲击波前形成气压差 爆轰超声冲击波传播速率是各向异性 在脉冲激光作用时间内, 主要是激光束入射的轴向扩展最快, 尤其是逆激光束方向 ; 在脉冲激光消逝之后, 主要是侧向运动, 即沿着物体表面的污垢传播, 形成剪切力, 这是一种机械作用力 图 2-2 脉冲激光除垢过程 由此可认为, 激光除垢机理主要是由垂直于物体表面的冲量与平行物体 表面的机械作用力共同作用的结果, 综上所述我们初步认为激光除垢机理可 5
