肠 系 膜 云 雾 征 的 影 像 学 意 义 林 凌 华
Misty Mesentery (MM ) 的 定 义 由 Mindelzun 于 1996 年 提 出, 是 指 肠 系 膜 脂 肪 被 炎 症 细 胞 液 体 ( 如 水 肿 淋 巴 液 或 血 液 等 ) 和 肿 瘤 浸 润 或 肠 系 膜 纤 维 化 等 导 致 肠 系 膜 密 度 增 高, 形 成 不 规 则 的 条 片 状 高 密 度 影, 呈 云 雾 状, 也 有 人 称 之 为 缆 绳 状 脂 肪 (Fat Stranding), 或 肠 系 膜 脂 肪 缆 绳 征, 与 各 种 急 腹 症 中 肠 系 膜 血 管 瘀 血 扩 张 形 成 的 肠 系 膜 血 管 缆 绳 征 有 所 不 同
按 分 布 分 型 弥 漫 性 分 布 : 多 见, 指 肠 系 膜 密 度 弥 漫 性 增 高, 其 内 血 管 边 界 不 清, 同 时 伴 有 腹 水 和 其 他 部 位 水 肿, 多 见 于 全 身 性 疾 病 引 起 者 节 段 性 分 布 : 围 绕 肠 系 膜 血 管 及 其 分 支 走 行 分 布 的 脂 肪 密 度 增 高, 可 见 于 外 伤 局 灶 性 炎 症 良 恶 性 肿 瘤 压 迫 浸 润 等
按 病 因 分 型 (1) 全 身 性 疾 病 : 如 血 清 蛋 白 减 少 症 肝 硬 化 肾 病 心 衰 等 当 炎 性 细 胞 和 液 体 浸 入 肠 系 膜 时, 其 CT 值 明 显 增 高 全 身 性 疾 病 导 致 者 常 伴 皮 下 组 织 水 肿 和 腹 水, 相 关 文 献 报 道 肝 硬 化 引 起 者 常 见 Chopra 等 认 为 肝 硬 化 患 者 机 体 水 钠 潴 留 呈 高 水 容 量 状 态 是 肠 系 膜 水 肿 的 主 要 原 因
(2) 血 管 病 变 : 门 静 脉 栓 塞 SMA SMV 栓 塞 及 脉 管 炎 布 加 综 合 征 下 腔 静 脉 阻 塞 绞 窄 性 肠 梗 阻 等 均 可 引 起 MM CT 上 肠 系 膜 密 度 增 高, 与 其 内 血 管 边 界 显 示 不 清 当 CT 增 强 扫 描 发 现 肠 系 膜 血 管 密 度 减 低 充 盈 不 佳 或 血 管 腔 变 细 时, 可 做 出 肠 系 膜 血 管 血 栓 或 栓 塞 的 诊 断
(3) 淋 巴 性 肠 系 膜 水 肿 引 起 的 MM : 由 于 淋 巴 系 统 阻 塞 和 其 他 原 因 如 淋 巴 系 统 发 育 异 常 炎 症 肿 瘤 外 伤 和 放 射 治 疗 后 引 起 淋 巴 液 通 过 淋 巴 管 道 渗 入 到 肠 系 膜 中, 造 成 肠 系 膜 水 肿 先 天 性 解 剖 异 常 导 致 淋 巴 液 间 接 流 到 肠 系 膜 内 引 起 肠 系 膜 淋 巴 性 水 肿 ; 急 性 肠 系 膜 淋 巴 结 炎 导 致 的 肠 系 膜 水 肿, 好 发 于 回 肠 远 端 的 肠 系 膜
(4) 炎 症 引 起 的 MM : 各 种 腹 腔 炎 症 均 可 导 致 MM, 可 呈 节 段 性 改 变, 也 可 呈 弥 漫 性 改 变 肠 系 膜 的 炎 症 常 继 发 于 腹 腔 脏 器 的 感 染, 如 急 性 胰 腺 炎 坏 疽 性 胆 囊 炎 阑 尾 炎 肠 炎 憩 室 炎 及 胃 十 二 指 肠 穿 孔 等
(5) 出 血 外 伤 引 起 的 MM : 外 伤 导 致 供 应 肠 系 膜 肠 腹 膜 后 或 盆 腔 的 血 管 破 裂 出 血, 出 血 渗 入 到 肠 系 膜 内 导 致 MM, 也 可 引 起 肠 系 膜 撕 裂 或 血 肿 形 成 Dowe 等 口 认 为 当 腹 外 伤 患 者 出 现 肠 壁 增 厚 邻 近 肠 系 膜 云 雾 征 或 有 出 血 时 提 示 肠 系 膜 损 伤, 是 急 诊 手 术 治 疗 指 征 ; 但 单 独 肠 系 膜 云 雾 征 不 能 作 为 剖 腹 探 查 的 指 征
(6) 肿 瘤 引 起 的 MM 肠 系 膜 原 发 肿 瘤 腹 腔 脏 器 肿 瘤 侵 犯 肠 系 膜 或 转 移 性 肿 瘤 均 可 引 起 MM 肿 瘤 引 起 的 MM 并 不 都 是 肿 瘤 侵 犯 浸 润 肠 系 膜, 也 可 为 肿 瘤 侵 犯 压 迫 肠 系 膜 血 管 或 淋 巴 管 导 致 肠 系 膜 水 肿 或 引 起 炎 症 所 致 腹 腔 外 肿 瘤 伴 发 者 并 无 明 确 的 肿 瘤 转 移 至 肠 系 膜 的 征 象 因 此, 恶 性 肿 瘤 合 并 MM, 也 并 不 完 全 都 预 示 肿 瘤 浸 润 也 有 作 者 认 为,MM 是 腹 膜 对 腹 腔 恶 性 肿 瘤 的 非 特 异 性 反 应
(7) 先 天 性 原 因 引 起 的 MM: 肠 系 膜 脂 膜 炎 mesenteric panniculitis) 是 罕 见 的 非 特 异 性 炎 症, 在 病 理 组 织 学 上 主 要 表 现 为 肠 系 膜 脂 肪 组 织 变 性 坏 死 炎 症 及 纤 维 化, 大 体 表 现 为 肠 系 膜 内 弥 漫 性 或 局 限 性 的 单 发 或 多 结 节 性 肥 厚 硬 化 及 挛 缩, 并 在 该 疾 病 自 然 病 程 的 某 一 阶 段 分 别 称 为 肠 系 膜 脂 膜 炎 肠 系 膜 脂 肪 营 养 不 良 硬 化 性 肠 系 膜 炎 退 缩 性 肠 系 膜 炎 及 肠 系 膜 Weber- Christian 病, 易 累 及 肠 系 膜 根 部, 也 可 多 部 位 发 病, 常 伴 发 于 其 他 先 天 性 炎 性 病 变 (8) 特 发 性 肠 系 膜 炎 : 原 因 不 明, 可 自 限 或 加 重
