二 尖 瓣 脫 垂 與 自 律 神 經 疾 病 二 尖 ( 僧 帽 ) 瓣 脫 垂 台 中 榮 民 總 醫 院 心 血 管 中 心 何 鴻 鋆 醫 師 解 剖 位 置 僧 帽 瓣 ( 又 稱 二 尖 瓣 ) 位 於 心 臟 左 心 房 及 左 心 室 間, 心 舒 張 期 時, 血 液 由 左 心 房 流 向 左 心 室, 心 收 縮 期 時, 僧 帽 瓣 則 關 閉 僧 帽 瓣 脫 垂 僧 帽 瓣 瓣 膜 或 相 關 器 官 中 組 織 發 生 病 變, 致 使 瓣 膜 鬆 大, 鍵 索 變 長 等 變 化 原 因 : 多 為 原 發 的, 即 先 天 造 成 由 於 上 述 之 病 理 變 化, 致 使 僧 帽 瓣 在 心 收 縮 期 時, 突 向 左 心 房 側 正 常 僧 帽 瓣 ( 圖 1) 僧 帽 瓣 脫 垂 ( 圖 2) 1
病 理 解 剖 僧 帽 瓣 黏 液 瘤 性 (myxomatous) 變 性 造 成 過 多 黏 多 醣 囤 積, 進 而 使 瓣 膜 與 肌 腱 索 拉 長. 在 左 心 室 正 常 大 小 時 即 可 能 發 生 可 能 在 健 康 女 性 脫 水 時 產 生, 水 分 補 足 後 消 失 在 懷 孕 或 體 重 增 加 後 可 能 消 失. 可 能 與 fibrillin 基 因 異 常 有 關. 引 發 症 狀 相 關 異 常 : 對 腎 上 腺 刺 激 敏 感 度 增 加 增 加 catecholamines 異 常 乙 型 接 受 器 增 加 atrial natriuretic factor, renin-aldosterone 調 節 異 常 減 少 血 管 內 容 量 鎂 離 子 缺 乏 發 生 率 目 前 僧 帽 瓣 脫 垂 已 是 最 常 見 的 瓣 膜 性 心 臟 病 發 生 率 約 為 2-5% 男 女 之 比 為 1:2~3 尤 好 發 於 身 材 瘦 長 或 胸 廓 畸 型 者 開 始 症 狀 年 齡 : 10-16 歲. 很 少 在 青 春 期 以 前 發 生 症 狀 大 部 份 是 没 有 症 狀 的, 通 常 是 在 醫 師 為 病 患 聽 診 時 意 外 發 現 而 診 斷 少 部 份 有 症 狀, 臨 床 表 現 則 相 當 多 樣, 包 括 了 呼 吸 不 順 暢 非 典 型 心 絞 痛 心 悸 不 整 脈 緊 張 焦 慮 疲 倦 自 律 神 經 失 調 昏 厥 等 少 數 病 患 併 發 嚴 重 閉 鎖 不 全 及 心 臟 功 能 變 化 時, 也 會 產 生 心 臟 衰 竭 的 相 關 症 狀 十 分 罕 見 的 病 患, 會 以 猝 死 或 暫 時 性 腦 部 缺 氧 來 表 現 身 體 檢 查 發 現 瘦 細 體 型 骨 骼 異 常 ( 如 漏 斗 胸 背 直 脊 柱 後 側 凸 ) 休 息 時 心 緩 脈 與 姿 勢 性 低 血 壓 心 臟 聽 診 收 縮 期 clicks 心 尖 聽 到 l mid-to-late systolic murmur. Click 與 心 雜 音 會 隨 姿 勢 改 變. 僧 帽 瓣 閉 鎖 不 全 自 律 神 經 失 調 神 經 內 分 泌 功 能 異 常 Ehlers-Danlos 症 候 群 表 現 ( 如 : 關 節 活 動 異 常, 異 常 abnormal striae, 淤 青, 皮 膚 炎 展 能 力 ) 骨 發 生 不 全 Osteogenesis imperfecta 徵 象 ( 如 : 藍 色 鞏 膜 ) Marfan 症 候 群 徵 象 : 脊 柱 側 凸 背 直 漏 斗 胸 蜘 蛛 狀 指, 手 張 開 比 身 高 高 Stickler 症 候 群 徵 象 : ( 如 : 駝 背 脊 柱 側 凸 mandibular 發 育 不 全 視 網 膜 剝 離 ) 死 亡 率 罹 病 率 一 般 而 言 是 良 性 疾 病 主 要 獨 發 性 僧 帽 瓣 閉 鎖 不 全 原 因 (MR) 90% chordae tendineae 斷 裂 原 因 40% 年 輕 型 中 風 10-15% 心 內 膜 炎 (endocarditis) 有 結 構 異 常 患 者 ( 如 僧 帽 瓣 增 厚 變 形 過 長 ) 較 易 有 併 發 症 ( 如 僧 帽 瓣 閉 鎖 不 全 惡 化 心 內 膜 炎 猝 死 ). 合 併 有 心 雜 音 一 般 死 亡 率 增 加 15-20%. 檢 查 診 斷 方 法 除 了 聽 診 之 外, 心 臟 超 音 波 為 診 斷 僧 帽 瓣 脫 垂 症 候 群 最 有 力 及 準 確 的 工 具 2
處 理 治 療 原 則 為 針 對 症 狀 處 理, 無 法 根 治 無 症 狀 者, 僧 帽 瓣 脫 垂 症 候 群 患 者 應 避 免 熬 夜 吸 菸 喝 酒 及 咖 啡 等 較 刺 激 性 的 飲 食 一 般 日 常 生 活 及 運 動 没 有 特 別 限 制 對 於 心 悸 胸 痛 不 適 焦 慮 交 感 神 經 系 統 亢 進 或 疲 倦 感 較 明 顯 的 患 者, 使 用 乙 型 交 感 神 經 阻 斷 劑 及 鎮 定 劑 的 治 療 效 果 不 錯 少 數 臨 床 上 懷 疑 有 心 律 不 整 的 患 者 可 以 接 受 24 小 時 心 電 圖 檢 查, 如 有 適 應 症, 之 後 亦 可 安 排 心 臟 電 氣 生 理 檢 查 如 有 心 房 纖 維 顫 動 暫 時 性 腦 部 缺 氧 或 中 風 時, 可 以 使 用 阿 斯 匹 靈 或 抗 凝 血 劑 來 預 防 栓 塞 症 的 再 次 發 生 當 極 少 數 病 患 會 產 生 重 度 僧 帽 瓣 閉 鎖 不 全, 心 內 膜 炎, 複 雜 性 心 室 心 律 不 整 及 暫 時 性 腦 缺 血, 則 應 由 心 臟 專 科 醫 師 做 特 別 處 理 危 險 分 級 臨 床 表 徵 處 置 原 則 低 度 危 險 : 無 收 縮 期 心 雜 音 心 臟 超 音 波 無 僧 帽 瓣 逆 流 女 性 <45 歲 對 病 人 說 明 病 情, 無 需 使 用 預 防 性 抗 生 素 心 超 音 波 每 五 年 追 蹤 一 次 中 度 危 險 : 有 收 縮 期 心 雜 音 心 臟 超 音 波 有 輕 度 僧 帽 瓣 逆 流 需 使 用 預 防 性 抗 生 素 心 臟 超 音 波 每 二 至 三 年 追 蹤 一 次 高 度 危 險 : 心 臟 超 音 波 有 中 重 度 僧 帽 瓣 逆 流 需 使 用 預 防 性 抗 生 素 每 年 追 蹤 一 次 心 臟 超 音 波 如 有 心 衰 竭 症 狀, 考 慮 手 術 治 療 自 律 神 經 系 統 Autonomic Nervous System 自 律 神 經 疾 病 自 律 神 經 系 統 (ANS) 以 協 調 方 式 控 制 體 內 環 境 利 用 反 射 機 制 (reflex action),ans 控 制 心 跳 血 壓 消 化 呼 吸 血 中 酸 鹼 平 衡 等 等 身 體 功 能 In the ANS there are 2 nerves between the central nervous system (CNS) and the organ. The nerve cell bodies for the second nerve are organized into ganglia: CNS -> Preganglionic nerve -> Ganglion -> Postganglionic nerve -> Organ At each junction neurotransmitters are released and carry the signal to the next nerve or organ. 3
The ANS has 2 Divisions, Sympathetic and Parasympathetic, Which Differ in Anatomy and Function The sympathetic nerves come from the thoracic and lumbar regions of the spinal cord. The preganglionic nerves are short and synapse in paired ganglia adjacent to the spinal cord The adrenal medulla, technically an endocrine gland, is functionally a part of the sympathetic nervous system Adrenal medulla is like a modified autonomic ganglion + postsynaptic nerve (see diagram below) Parasympathetic nerves come from the cranial and sacral regions of the CNS. They have long preganglionic nerves which synapse at ganglia near or on the organ innervated The 2 branches use the same transmitter, acetylcholine (ACh), in the ganglia. At the organ quite different transmitters are used: Parasympathetic = ACh Sympathetic = norepinephrine (NE). Note: norepinephrine is the same thing as noradrenaline. Note: there are a few sympathetic nerves where the transmitter at the organ is ACh Sympathetic fibers releasing ACh: sweat glands, piloerector muscles, some blood vessels Comparison of the 2 systems The Sympathetic is the "Fight or Flight" Branch of the ANS Emergency situations, where the body needs a sudden burst of energy, are handled by the sympathetic system The sympathetic system increases cardiac output and pulmonary ventilation, routes blood to the muscles, raises blood glucose and slows down digestion, kidney filtration and other functions not needed during emergencies Whole sympathetic system tends to "go off" together In a controlled environment the sympathetic system is not required for life, but it is essential for any stressful situation The Parasympathetic is the "Rest and Digest" Branch of the ANS The parasympathetic system promotes normal maintenance of the body- acquiring building blocks and energy from food and getting rid of the wastes It promotes secretions and mobility of different parts of the digestive tract. Also involved in urination, defecation. Does not "go off" together; activities initiated when appropriate The vagus nerve (cranial number 10) is the chief parasympathetic nerve Other cranial parasympathetic nerves are: III (oculomotor), VII (facial) and IX (glossopharyngeal) The Hypothalamus Has Central Control of the ANS The hypothalamus is involved in the coordination of ANS responses, One section of the hypothalamus seems to control many of the "fight or flight" responses; another section favors "rest and digest" activities 4
The Adrenal Medulla is an Extension of the Sympathetic Nervous System The adrenal medulla behaves like a combined autonomic ganglion and postsynaptic sympathetic nerve (see diagram above) Releases both norepinephrine and epinephrine in emergency situations Releases a mixture of epinephrine (E = 80%) and norepinephrine (NE = 20%) Epinephrine = adrenaline This action is under control of the hypothalamus Comparison of the Actions if the Sympathetic & Parasympathetic Systems Usually (but not always) both sympathetic and parasympathetic nerves go to an organ and have opposite effects You can predict about 90% of the sympathetic and parasympathetic responses using the 2 phrases: "Fight or Flight" and "Rest and Digest". Gastroparesis Gastroparesis -- causes Slow stomach emptying from stomach nerve damage a disorder in which the stomach takes too long to empty its contents. most often a complication of type 1 diabetes. At least 20% of people with type 1 diabetes develop gastroparesis. It also occurs in people with type 2 diabetes, although less often. Gastroparesis happens when nerves to the stomach are damaged or stop working. If the vagus nerve is damaged, the muscles of the stomach and intestines do not work normally, and the movement of food is slowed or stopped.
