2 声明 Statement 1. 本公司依据国际海运危险货物规则 (37-14 版 ) 以及委托人 ( 托运人或其他代理人 ) 提供的物品及其运输信息, 确定货物的航空运输条件并出具此鉴定书 The certification is issued by SEM.Test according to International Maritime Dangerous Goods Code (Amdt.37-14) and the information of the goods and the information of its shipping provided by the consignor (shipper of his agent). 2. 委托单位必须如实提供样品及资料, 并保证申报品名和样品与运输货物相同 The consignor should provide sample and relevant data, at the same time, they should guarantee the consistence of the product's name they declared, the samples they provided and the goods to be transported. 3. 本鉴定书的鉴定结论仅对委托单位提供的样品负责 The conclusion of this certification is responsible for the sample provided by the consignor. 4. 本鉴定书经主检员审核人和批准人签字并加盖本公司印章后生效 This certification will be effective only after it is signed by the Appraiser, checker and approver, and stamped by SEM.Test. 5. 未经本公司许可, 不得部分复制摘用本鉴定书内容 This certification shall not be reproduced except in full, or extracted, without the written approval of SEM.Test. 6. 鉴定书涂改无效 The certification is invalid if it is blotted out. 7. 本鉴定书仅在本年度内有效 This certification is only valid within the year.
