1 本局檔號 :DSE/CR 3/ 年香港中學文憑考試 2016/ 2017 年度考試通告第 2 號甲類 乙類及丙類 ( 其他語言 年 6 月 ) 科目考試學校考生報名事宜 一. 學校考生報名 (a) 2017 年香港中學文憑甲類 乙類及丙類 ( 其他語言 年 6 月 ) 科目考試將於 2016 年 9 月 13 日至 10 月 7 日接受報名 學校擬保送學生參加本考試, 必須透過香港中學文憑考試網上服務 ( 之報名系統提交報名申請 考評局不接受任何表格形式申請 (b) 有關選擇報考科目的詳細資料, 請參閱附件一 (c) 學校在提交考生報名資料予考評局前, 必須確保所有報考 2017 年香港中學文憑考試的學生為學校 2016/2017 學年的中六正讀生 有關報考乙類科目的詳細資料, 請參閱附件一第二段 (d) 學校須提醒其學生於應考時必須遵守香港中學文憑考試考試規則及考生手冊內所列的考試規則 考生違反任何考試規則或要求會受到處分 二 年香港中學文憑考試行政安排的重要事項學校請留意以下有關 2017 年香港中學文憑考試 ( 文憑試 ) 行政安排的重要事項 : 範圍報考乙類科目 ( 應用學習科目 ) 丙類科目 ( 其他語言 年 6 月 ) 考試之考試日期 修訂學校必須在報名期間於報名系統為有關考生 [ 包括附件一第二 (c) 段所述的中四或中五重讀生 ( 如有 )] 選報乙類科目, 否則, 該考生之乙類科目成績將不會列於 2017 年香港中學文憑考試成績通知書及證書 ( 如適用 ) 上, 而其成績亦不能計算至 2018 年及往後的香港中學文憑考試 此外, 由於部分乙類科目的名稱非常近似, 考試事務主任在選擇科目時, 必須小心注意科目名稱 英國劍橋國際考試委員會將於 2016 年 11 月初公布丙類科目 ( 其他語言 年 6 月 ) 考試 ( 即法語及西班牙語 ) 的考試日期 若考試日期與甲類科目的考試日期有衝突, 相關考生會被安排在同日下午應考丙類科目的考試

2 丙類科目 ( 其他語言 年 6 月 ) 考試之發放成績安排 嚴重逾期報名 / 更改報考科目資料申請 丙類科目 ( 其他語言 年 6 月 ) 考試的成績將於 2017 年 8 月下旬發放, 因此未能配合大學聯合招生處院校遴選之時間表 本局待英國劍橋國際考試委員會就成績發放日期作最後落實後再通知學校 考評局通常不接受 2016 年 12 月 14 日以後提出的申請 於此限期後提出的申請均會由秘書長按個別情況作特別考慮 再者, 於考評局發出准考證時指定的日期後提出的任何嚴重逾期更改申請將不獲接納 三. 報考程序及為學校提供的支援 (a) (b) 報考指引供學校使用為方便學校替學生準備 2017 年香港中學文憑考試的報考申請, 本局編制了一份 學校報考指引 ( 九月份報名 ) ( 附件二 ), 內容包括 : 報考流程的細節 重要事項及學校 / 考生需下載的有關文件 校長及考試事務主任可參照以下第四段所列的重要日期及細閱上述指引在適當時段進行各項行政工作 學校簡介會為使校務人員能夠更瞭解報考 2017 年文憑試的安排, 本局將分別於 2016 年 9 月 12 日及 13 日下午舉辦兩場內容相同的學校簡介會 簡介會分為兩節 : 第一節 : 九月份報名的程序第二節 : 特別考試安排的申請程序 有關簡介會的程序表列於附件四 學校代表可選擇出席其中一場簡介會 ( 第一節或 / 及第二節 ) 為方便安排, 校長 / 考試事務主任須於 2016 年 9 月 9 日 ( 星期五 ) 或之前登入 學校資訊管理網上服務 (SIM) 報名 ( 登入香港中學文憑考試網上服務 學校資訊管理 編修 活動報名 ) 由於座位有限, 每間學校可各委派最多兩名代表出席每節的簡介會 本局會於 2016 年 9 月 13 日上載簡介會的簡報至香港中學文憑考試網上服務之相關系統主頁及考評局網頁之 學校及教師 專頁 四. 考試時間表及重要日期一覽表 2017 年香港中學文憑考試的考試時間表列於附件三 以下為報考 2017 年香港中學文憑考試的重要日期一覽表, 以供參照 : 事項日期參考文件 2016 年 9 月至 10 月 1. 經 學校資訊管理網上服務 (SIM) 報名出席簡介會 2. 為特殊需要考生申請特別考試安排 2016 年 9 月 9 日或之前附件四程序表 2016 年 9 月 6 日至 27 日附件二第四部分 : 申請特別考試安排 - 2 -

5 附件一 2017 年香港中學文憑考試 甲類 乙類及丙類 ( 其他語言 年 6 月 ) 科目考試 選擇報考科目 2017 年香港中學文憑考試九月份報名涵蓋甲類 乙類及丙類 ( 其他語言 年 6 月 ) 科目 考生於 2016 年 6 月及 9 月所報考 2017 年香港中學文憑考試的科目數目將綜合計算, 總數不可超過八科之上限 若考生選擇報考八科, 則至少其中一科必須屬下列範疇的科目 : (a) 甲類科目中的倫理與宗教 / 音樂 / 體育 / 視覺藝術 (b) 乙類科目中的應用學習科目 (c) 丙類科目中的其他語言科目 1. 報考甲類料目考試 (a) 甲類科目設 24 科高中科目 在同一次考試, 考生只可選擇以一種語文報考可用中文或英文應考之甲類科目 (b) 學校擬保送學生參加設有校本評核部分的科目, 學校必須符合為其學生報考的科目之校本評核要求 若學校沒有參與校本評核計劃, 則考評局不會接受其學生參加有關科目之考試 (c) 重讀中六的學校考生必須在中六重新接受評核, 並完成所報考科目相關的校本評核要求 若其學校沒有開設有關的選修科目, 而重讀生想透過該校報考, 可向考評局申請豁免參與該科目之校本評核部分 有關考生整科成績將以公開考試分數計算 ( 見考試規則第 7.9 項 ) 這項豁免並不適用於中國語文 英國語文及通識教育等核心科目 有關豁免之申請須於 2016 年 9 月 13 日或之前向考評局提出 申請表格可於 2016 年 9 月 6 日起於報名系統下載, 學校須填妥申請表格及提交有關證明文件以供考評局考慮 (d) 學校擬保送學生參加需有實驗室或工場設備的實習科目 ( 即生物 / 化學 / 物理 / 組合科學 / 綜合科學 / 設計與應用科技 / 科技與生活 ), 必須已申報能提供有關的實驗室或工場設備作實習用途 (e) 考生擬報考企業 會計及財務概論 / 設計與應用科技 / 倫理與宗教 / 資訊及通訊科技 / 音樂 / 科技與生活 / 視覺藝術科目之考試, 須選擇卷別 / 單元 考生擬報考音樂科 / 體育科之考試須選擇樂器 / 體育科實習試活動項目 報考音樂科卷二及 / 或卷四乙而未獲豁免考試之考生必須根據報名時所選的樂器及 / 或聲樂應考 其他未有在報名時所選的樂器或聲樂的演出將不獲主考員評核 另外, 若考生未能演 1

6 出報名時所選的全部樂器及 / 或聲樂, 則會被扣分 有關要求的詳情請參閱 2017 年香港中學文憑考試規則及評核大綱 或考評局網頁 香港中學文憑 評核資訊 科目資訊 學校亦應參閱附件二 學校報考指引 ( 九月份報名 ) 第二部分有關在報名系統上輸入有關科目選項須知 (f) 考生擬報考數學科考試, 可選擇報考必修部分或必修部分加延伸部分任何一個 單元 如考生選擇必修部分加延伸部分任何一個單元, 亦只會計算為報考一科 必修部分及延伸部分必須以相同的語文應考 (g) 考生擬報考生物 / 化學 / 物理 / 組合科學 / 綜合科學, 只可在同一次考試中報考下列各組合中的其中一個科目 : 組別一 : 生物 綜合科學 組合科學 ( 生物及化學 ) 組合科學 ( 生物及物理 ) 組別二 : 化學 綜合科學 組合科學 ( 生物及化學 ) 組合科學 ( 化學及物理 ) 組別三 : 物理 綜合科學 組合科學 ( 生物及物理 ) 組合科學 ( 化學及物理 ) 2. 報考乙類科目 ( 應用學習科目 ) (a) 乙類科目設 40 科應用學習科目, 由課程提供機構發展, 並經教育局批核 (b) 乙類科目的評估工作由有關的課程提供機構執行, 考評局則負責監察和調整最終成績 (c) 於 2016/2017 學年重讀中四或中五的學生如現正修讀應用學習科目第二年 (Y2) 的課程, 則可以學校考生身分報考 2017 年香港中學文憑考試 只限應用學習科目, 但須得到學校校長及課程提供機構的支持 學校應於報名期間透過報名系統為有關考生提交報考 只限應用學習科目 的申請, 考生須按指定的付款期限繳付考試費 中四或中五重讀生以學校考生身分報考應用學習科目, 毋須透過學校向考評局提交有關的證明文件以作申請特別批核 ; 校長只需在取得課程提供機構的確定後透過報名系統在提交報考申請前揀選有關聲明以作確認 該考生之乙類科目成績將會列於 2017 年香港中學文憑考試成績通知書及證書 ( 如適用 ) 上 學校須提醒有關的中四或中五重讀生 ( 如有 ), 如他 / 她沒有於 2017 年提交報考 只限應用學習科目 的申請, 該學生之應用學習科目成績將不會列於 2017 年香港中學文憑考試成績通知書及證書 ( 如適用 ) 上, 而其成績亦不可計算至 2018 年及往後的香港中學文憑考試 假若申請不獲接納, 考評局將會向有關考生退回已繳付的考試費 (d) 以學校考生身分報考 2017 年考試的中六重讀學生如獲課程提供機構的支持, 可重讀同一應用學習科目, 有關課程提供機構只需向考評局提交第二年 (Y2) 課程的評核成績 ( 應佔整個科目總分最少百分之五十 ) 以供調整 學校必須於報名期間透過報名系統一併提交有關考生的應用學習科目及其他科目的報考申請 2

9 暫准參加文憑試的學校注意事項 : 如學校仍屬暫准參加文憑試, 將不會獲發校長 ( 使用者編號尾 4 字為 P001 或 P002) 及學校系統管理員 ( 使用者編號尾 4 字為 M001 或 M002) 的使用者編號及預設登入密碼, 直至學校取得與考批核 有關學校須於指定限期前, 提交完整的文件予考評局處理與考文憑試之申請事宜, 並於取得與考批核後, 於報名截止日期 2016 年 10 月 7 日或之前, 為學生報考 2017 年香港中學文憑考試 否則該學校的學生只可以自修生身分報考, 並要符合自修生參加考試之規則 ( 有關自修生報考資格之詳情, 請參閲 2017 年香港中學文憑考試規則 ) 2. 報名程序 輸入報名資料 列印 考生報名資料核對表 由校長確認並提交報名資料予考評局 列印繳費單 2.1 輸入報名資料考試事務主任為學生準備報名資料時, 可透過下列其中一個模式進行 : 方法一經 網上校管系統 ( WebSAMS) 的聯遞系統 (CDS) 傳送每班考生的報考資料檔案至報名系統 請參閱附錄二 (a) 學校使用網上學校行政及管理系統 ( WebSAMS) 的補充資料 ( 只提供英文版 ), 瞭解詳細步驟 方法二由學校的電腦系統產生考生報考資料檔案, 然後上載至報名系統 方法三在報名系統上逐一輸入每名考生的報名資料 建議學校 : 在上載檔案前檢視報名系統內所預設的學校科目代碼 在上載資料到報名系統後為個別考生選擇有關科目的卷別 / 單元 [ 參考以下瞭解更多資訊 ] 2

10 注意事項 : 就音樂科及體育科, 無論學校是否使用 網上校管系統 ( WebSAMS), 考試事務主任必須登入報名系統為有關考生逐一輸入音樂科 / 體育科的補充資料 ( 音樂科的樂器 / 體育科的實習試活動項目的選擇 音樂科的試卷豁免申請 ) 音樂科 / 體育科的補充資料表格樣本可於 2016 年 9 月 13 日起於報名系統下載 提交報名資料予校長確認前, 請小心核對以下事項 : 考生的英文姓名全寫及中文姓名, 必須與考生的身分證明文件上的姓名相符 小心輸入每名考生所報考的科目及其所選報考的卷別 / 單元 / 樂器 ( 音樂科 )/ 實習試活動項目 ( 體育科 ), 應考語文以及申請減免考試費的資助幅度 為方便考評局以電郵向考生發放有關考試的重要訊息, 或有緊急事故時以短訊聯絡考生, 考生必須向校方提供其電郵地址, 號碼 [ 參考以下瞭解更多資訊 ] 雖然乙類科目沒有設公開考試, 考試事務主任必須在報名期間於報名系統為考生 包括中四或中五重讀生 ( 如有 ) 選報乙類科目, 否則, 該考生之乙類科目成績將不會列於 2017 年香港中學文憑考試成績通知書及證書 ( 如適用 ) 上, 而其成績亦不能計算至 2018 年及往後的香港中學文憑考試 此外, 由於部分乙類科目的名稱非常近似, 考試事務主任在選擇科目時, 有關考生的電郵帳戶的注意事項 : (a) 學校如為其學生提供學校域名的電郵地址作為考生之聯絡資料, 請於報考前向有關電郵供應商申請接受由考評局發出的電郵 ( (b) 未有電郵地址的考生須建立一個新的帳戶, 切勿與其他人士共用同一電郵帳戶 考生必須確保有關電郵帳戶的設定能接受由考評局發出的電郵 ( 否則, 考評局發出的電郵或可能在 雜件箱 或 垃圾郵箱 收取 ) 2.2 列印 考生報名資料核對表 完成報名資料後, 考試事務主任應列印 考生報名資料核對表 予有關考生核對 如考生報告資料有錯誤, 考試事務主任應在報名系統上更正資料 當考生確認核對表上的資料無誤, 應在核對表上簽署 校方應把已簽署的核對表正本存檔, 並提供複印本供學生保存作日後參考用 考生擬於報名截止日期 2016 年 10 月 7 日後增加 / 更改 / 刪除科目, 除繳交所需的考試費 ( 如適用 ) 外, 考生須另繳附加費 3

11 2.3 由校長確認並提交報名資料予考評局考試事務主任完成報名資料後, 須呈交報名申請予校長確認 校長必須於截止日期 2016 年 10 月 7 日或之前確認所有報名申請, 並提交予考評局 2.4 列印繳費單 報名申請獲校長確認後, 考試事務主任應列印繳費單予有關考生 考生須於 2016 年 10 月 17 日下午 11 時 59 分前繳妥考試費 已申請減免 全額 考試費的考生亦會收到繳費單供存檔用, 惟有關考生不需繳交考試費 考試事務主任應指示考生登入考評局的網頁下載 繳付考試費用須知 ( 香港中學文憑 考務安排 考試報名及費用 考試費用 ), 暸解更多繳費方法的資料 3. 繳付費用 3.1 考試費 語言科目 ( 即英國語文科 中國語文科及六種其他語言科目 ) 其他科目 每科 595 元 每科 398 元 3.2 繳費限期及方法 繳費限期 2016 年 10 月 17 日或 11 時 59 分或之前 繳付方法 : 使用網上理財 ; 使用 繳費靈 ; 或 於任何一間香港境內的 7-Eleven 或 OK 便利店以現金付款 考評局不會接受考生 / 家長的個人支票, 或校方以一張支票支付所有報考考生的考試費 3.3 逾期繳費 (1) 考生如於 2016 年 10 月 18 日至 10 月 22 日中午 12 時正前繳交考試費, 即作逾期繳付考試費論 考生除繳交所需的考試費外, 須另繳交附加費 238 元, 否則其報名申請不會獲接納 考生須親身或委託代表於 2016 年 10 月 22 日中午 12 時正前到考評局修頓中心 12 樓辦事處櫃枱繳妥有關費用 (2) 根據 2017 年香港中學文憑考試規則第 5.9 項, 考生繳付全部考試費後, 其報考申請方屬有效, 故此考生如未能於 2016 年 10 月 22 日中午 12 時正前繳妥考試費, 則其報考申請會被取消 4

16 附錄 2(a) Attachment 2(a) Hong Kong Diploma of Secondary Education Examination (HKDSE) 2017 Registration of School Candidates for Category A, Category B and Category C (Other Languages June 2017 Series) subjects Supplementary Information for Schools Using WebSAMS Schools should refer to the Examination Circular No. (2) 2016/2017 when reading this document. School Choosing to Use WebSAMS for Registration of Candidates 1. Schools may choose to use the HKEAA module within the Web-based School Administration and Management System (WebSAMS) to process examination entries for the coming HKDSE. Schools may join the WebSAMS on a voluntary basis for the purpose of examination processing in In case the following circumstances are applicable, principals of schools are requested to complete the attached reply slip and return it to the Authority by fax (Fax No.: ) on or before Wednesday, 7 September (1) For schools that have used WebSAMS for the September Registration of the 2016 HKDSE as listed at Attachment 2(b), it is assumed that they will continue to use the HKEAA module of the WebSAMS to transmit the examination entries to the Registration System. As such, principals of these schools need not complete the reply slip. Should any school principals decide to the contrary, they are requested to inform the Authority using the reply slip provided. (2) For schools intending to use the HKEAA module of the WebSAMS to transmit the examination entries to the Registration System but are not on the school list at Attachment 2(b), please inform the Authority by completing the reply slip and return it to the Authority by the above deadline. Points to Note for Using WebSAMS 2. WebSAMS System Upgrade Schools using WebSAMS should upgrade the WebSAMS BUILD version or above which is now available for download from the WebSAMS download site. 3. Parameter File The Authority will transmit the Parameter File including the subject and component table for the 2017 HKDSE to each school on Thursday, 8 September Schools should decrypt the CDS message at CDS module (CDS > Incoming Msg) and import the Parameter File at the HKEAA module (HKEAA > HKDSE > Data Communication >Process Incoming Data). If the Parameter File was not received by 4:00 pm, 8 September 2016, schools concerned should contact the Public Examinations Information Centre (PEIC) of the HKEAA at as soon as possible. 1 of 3

Steps to be Processed in WebSAMS WebSAMS schools should follow the steps below to prepare their candidates entry data for the September registration by transmitting the data file(s) via CDS: Step 1: Start Inter-Year Processing (IYP) or Migrate to New School Year According to Regulation 3.2 of the 2017 HKDSE Examination Regulations, only bona fide S6 students in the school year 2016/2017 may enter for the 2017 HKDSE. In this connection, schools are required to start the IYP to promote their students to Secondary 6 and attach subjects to the students for the school year 2016/2017 using the Plan New School Year and Student functions on WebSAMS if they have not done so. For details, please refer to the WebSAMS User Manual on Inter-Year Processing. Step 2: Maintain Subject Mapping and Examination Language It is most important for schools to maintain the mapping of the subjects to be entered by their students in WebSAMS to the Category A, Category B and Category C (Other Languages June 2017 Series) subjects of the HKDSE as well as mapping the language version and paper (if required) at the HKEAA module (HKEAA > HKDSE > Subj Mapping & Exam Lang > School Subject) and HKEAA module (HKEAA > HKDSE > Subj Mapping & Exam Lang > APL Subject). Schools can click the hyperlink Outstanding Subject Mapping and Exam Lang Status List (R-HKE049) to check whether there is any missing subject mapping. Step 3: Maintain Registration Data (1) As a preliminary check on student particulars data prior to the extraction of registration data, schools may make use of the Report Student with Missing/Invalid Particulars (HKDSE Registration) List (R-HKE050) at the HKEAA Module (HKEAA > HKDSE > Report) to detect any relevant data errors and then take follow up action accordingly. (2) Schools should generate the candidates entries through the Maintain Registration Data function at the HKEAA module (HKEAA > HKDSE > Maint HKDSE Reg Data). Please note that only the selected candidate records will be generated and transmitted through the WebSAMS CDS module to the HKEAA Registration System. Schools should carefully check the subjects* (including ApL subjects, if applicable) and the selection of paper(s) for each student in this step. (3) Schools may use the Batch Amendment function to modify the language version and paper(s) for their students by batch. (4) To facilitate the HKEAA in contacting candidates or disseminating important examination-related information to them via or SMS in case of emergency, the candidates should be asked to provide their addresses (mandatory) and SMS phone numbers (optional). Schools may update the above information in the corresponding student s profile in the Student module (Student > Profile > Address). * Remark: For the subjects of Music and Physical Education, schools must enter the supplementary information (e.g. selection of musical instruments / physical activities, application for paper exemption for the subject of Music) of individual candidates into the HKEAA Registration System one by one. 2 of 3

18 Step 4: Confirm Fee Remission Student Finance Office (SFO) will transmit the result data file for student financial assistance by batches to schools via CDS. Schools should import the result data file at the WFSFAA(SFO) module and confirm the level of assistance for examination fee remission of those eligible candidates in the HKEAA module (HKEAA > HKDSE > Confirm Fee Remission) before transmitting the data file via CDS. Please note that the first two batches of result data files have already been delivered via CDS to schools on 18 August and 29 August 2016 respectively. The next batch of result data file before the start of September Registration will be transmitted on 15 September For more details, please refer to letter from SFO vide ref. (45) in WFSFAA/SFO/TT/523/01 Pt. 1 dated 15 August 2016 and Examination Circular No. (3) 2016/2017. Step 5: Transmission of Data Files from WebSAMS to the HKEAA Registration System Schools should confirm the generation of data file by class in the HKEAA module (HKEAA > HKDSE > HKDSE > Data Communication > Prepare Outgoing Data). Once the data file(s) of specific class(es) is/are confirmed, CDS message(s) will be created in the CDS module. Schools should preview and encrypt the specific message(s) at CDS module (CDS > Outgoing Msg) before the start of transmission. In case of any transmission failure, schools should resend the data file again as soon as possible. Re-transmission after the closing date of registration is not allowed and will be regarded as late entries. After the file(s) has/have been successfully transmitted to the HKEAA Registration System, SEO should proceed with the follow-up steps in the Registration System, including modifying individual candidates entries (if necessary), inputting supplementary information for the subjects of Music / Physical Education (if any), printing Candidate Entry Information Checklists (CEIC) for distribution to candidates for checking and confirmation of their entry data and submission to Principal for endorsement. Principals should endorse all candidates entries before 7 October Any late submission after 7 October 2016 will incur a supplementary fee in addition to the normal examination fee. 5.
