2 What is Sun MaxiMed-i? It is an individual top up hospitalisation and surgical takaful plan with surplus sharing to provide for expenses on hospitalisation, surgical, non-surgical and other related cost incurred due to injuries or illnesses. Why Sun MaxiMed-i? Escalating medical costs are a growing burden for families across all age groups where other important priorities such as retirement or children s education might have to be put aside when one faces a considerable medical problem. This is where Sun MaxiMed-i will come in handy as it serves as an additional layer of coverage on top of your existing medical plan to maximise your level of protection. How Sun MaxiMed-i works? Best Doctors Services A platform that connects you with world leading medical experts, allowing you to learn more about your health condition and the treatment that suits you best. Family Discount 10% family discount when your family members sign up together with you with an individual contract to be issued for each person covered. Retirement Option* An option to reduce your deductible amount to zero and turn Sun MaxiMed-i into a full plan with no further underwriting required when you are between the age of 55 to 59 so that you can continue to enjoy adequate protection when you need it most. Portable Protection continues even after you move on to a new job or after you have retired. Deductible and Base Plan Period Alignment There are 6 different levels of deductible to choose from, depending on your needs. Deductible means the amount you have to pay before Sun MaxiMed-i come in to cover for the remaining medical expenses. If your existing medical coverage is high, opt for a higher deductible at a lower contribution. On the other hand, if you think your current coverage might not be able to sustain a chronic health problem, then the lower deductible plans may be the right fit. Sun MaxiMed-i also comes with the feature to align the deductible period with your existing medical protection plan (base plan) by adjusting the deductible anniversary. It is recommended to align the period so that it can truly complement your medical needs. Tax Relief Tax relief for contribution paid on medical takaful benefit in addition to the usual tax relief for family takaful and Employees Provident Fund (EPF). * The contract must have been in force for at least 5 years.

Surplus sharing Tabarru' (donation) collected will be allocated into the participants tabarru fund (PTF). It will be used to help all participants who are in financial hardship and to pay the takaful benefit. At the end of each financial year, any surplus declared from the PTF after repayment of Qard (if any interest-free loan we provide in the event of a deficit in the PTF) will be shared between eligible participants and Sun Life Malaysia at the ratio of 50:50. Investment profit The balance of contribution after deduction of wakalah fee shall be retained in the participant s account. The contract holder and Sun Life Malaysia will share the investment profit (if any) arising from the participant s account in the proportion of 90% and 10% respectively at the end of each financial year. In the event of investment loss arising from the participant s account, there will be no investment profit sharing from the participant s account for that financial year. Commissions The commission payable is only 15% of every contribution paid and will be borne by the contract holder, which forms part of the wakalah fee. Example: The annual contributions quoted here are for males aged 35 with occupational class 1 and standard risk. Exclusions (Note: This list is non-exhaustive. Please refer to the contract documents for further information on exclusions) No benefit will be payable for any medical treatment received by the person covered in connection with illness defined under specified illnesses and/or any medical or physical conditions arising within the waiting period, except for accidental injuries. This plan does not cover any hospitalisation, surgery or charges resulted directly or indirectly, wholly or partly from, or as a result of the following: i) Pre-existing condition; ii) Specified Illnesses occurring during the first 120 days of continuous cover; iii) Any medical or physical conditions arising within the first 30 days of the person covered s cover from the contract issue date or date of last reinstatement, except for accidental injuries; iv) Plastic/Cosmetic surgery, circumcision, eye examination, glasses and refraction or surgical correction of near-sightedness (Radial Keratotomy or Lasik) and the use or acquisition of external prosthetic appliances or devices such as artificial limbs, hearing aids, implanted pacemakers and prescriptions thereof; v) Dental conditions including dental treatment or oral surgery except as necessitated by accidental Injuries to sound natural teeth occurring wholly during the contract year; vi) Private nursing, rest cures or sanitaria care, illegal drugs, intoxication, sterilisation, venereal disease and its sequelae, AIDS (Acquired Immune Deficiency Syndrome) or ARC (AIDS Related Complex) and HIV related diseases, and any communicable diseases required quarantine by law;

7 vii) Any treatment or surgical operation for congenital abnormalities or deformities including hereditary conditions; viii) Pregnancy, child birth (including surgical delivery), miscarriage, abortion and prenatal or postnatal care and surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining to infertility. Erectile dysfunction and tests or treatment related to impotence or sterilisation; ix) Hospitalisation primarily for investigatory purposes, diagnosis, X-ray examination, general physical or medical examinations, not incidental to treatment or diagnosis of a covered disability or any treatment which is not medically necessary and any preventive treatments, preventive medicines or examinations carried out by a Physician, and treatments specifically for weight reduction or gain; x) Any out-patient treatment unless specifically provided under this xi) contract; Suicide, attempted suicide or intentionally self-inflicted Injury while sane or insane. xii) War or any act of war, declared or undeclared, criminal or terrorist activities, active duty in any armed forces, direct participation in strikes, riots and civil commotion or insurrection; xiii) Ionising radiation or contamination by radioactivity from any nuclear fuel or nuclear waste from process of nuclear fission or from any nuclear weapons material; xiv) Expenses incurred for donation of any body organ by a person covered and costs of acquisition of the organ including all costs incurred by the donor during organ transplant and its complications; xv) Investigation and treatment of sleep and snoring disorders, hormone replacement therapy and alternative therapy such as treatment, medical service or supplies, including but not limited to chiropractic services, acupuncture, acupressure, reflexology, bone setting, herbalist treatment, massage or aroma therapy or other alternative treatment; xvi) Care or treatment for which payment is not required or to the extent which is payable by any other takaful/insurance or indemnity covering the person covered and disabilities arising out of duties of employment or profession that is covered under a workman s compensation takaful contract/insurance policy; xvii) Psychotic, mental or nervous disorders (including any neuroses and their physiological or psychosomatic manifestations); xviii) Costs/expenses of services of a non-medical nature, such as television, telephones, telex services, radios or similar facilities, admission kit/pack, and other ineligible non-medical items; xix) Sickness or injury arising from racing of any kind (except foot racing), hazardous sports such as but not limited to skydiving, water skiing, underwater activities requiring breathing apparatus, winter sports, professional sports and illegal activities; xx) Private flying other than as a fare-paying passenger in any commercial scheduled airlines licensed to carry passengers over established routes; or xxi) Expenses incurred for sex change procedures. Specified illnesses means the following disabilities and its related complications, occurring within the first 120 days from the contract issue date. However, if there is break in coverage prior to the expiry date of the said 120 days, a fresh period of the said 120 days shall apply again from the date of reinstatement. i) Hypertension, diabetes mellitus and cardiovascular disease; ii) All tumours, cancers, cysts, nodules, polyps, stones of the urinary system and biliary system; iii) All ear, nose (including sinuses) and throat conditions excluding flu and sore-throat; iv) Hernias, haemorrhoids, fistulae, hydrocele, varicocele; v) Endometriosis including disease of the reproduction system; and vi) Vertebro-spinal disorders (including disc) and knee conditions. Waiting period means the first 30 days between the beginnings of a person covered s disability and the contract issue date of the person covered. This shall not be applicable after the first year of cover. However, if there is a break in the coverage, the waiting period will apply again. Pre-existing condition means any person covered s condition or illness that existed before the contract issue date or date of last reinstatement, which the person covered have reasonable knowledge of. A person covered may be considered to have reasonable knowledge of a preexisting condition where the condition is one for which: i) the person covered had received or is receiving treatment; ii) medical advice, diagnosis, care or treatment has been recommended; iii) iv) clear and distinct symptoms are or were evident; or its existence would have been apparent to a reasonable person in the circumstances. Important notes 1) This is a product brochure and not a contract document. Please refer to the product disclosure sheet for more information on the plan as well as the contract documents for the complete terms and conditions of this plan. 2) You should be satisfied that this plan will best serve your needs and ensure that the contribution payable under this plan is an amount that you can afford. 3) You may cancel your contract within 15 days of its delivery and the amount of refund that you will receive is the contributions less any expenses incurred for any medical underwriting required. 4) If you terminate your contract in the early years, you may get back less than the amount that you have paid. 5) If you do not pay the contribution within 30 days of grace period, there will be a possibility that at any time the value of your participant s account might be insufficient to support the contract and your contract will lapse. 6) The contribution amount paid is allowed as an income tax deduction subject to the Inland Revenue Board. 7) All taxes, including without limitation any goods and services tax, and/or other forms of sales or consumption tax, whether currently in force or implemented after the date of the contract will be charged in accordance with the applicable legislation at the prevailing rate. Where necessary, Sun Life Malaysia will amend the terms of the contract to take into account any such tax. 8) The amount of contributions and/or wakalah fee and/or tabarru is not guaranteed and may be revised by giving you at least 30 days written notice and the revision will be effected on the next contract anniversary after the expiry of such notice period.

8 9) Sun Life Malaysia reserves the right to cancel the product as a whole if it decides to discontinue acceptance of risk for this product by giving notice to the contract holder at least 30 days prior to the contract anniversary. In this situation, the value of participant s account, if any, is payable and no surrender charge is applicable. 10) Best Doctors is not an insurance/takaful plan/product and it does not replace your relationship with your current doctor or medical specialist. Sun Life Malaysia reserves the right to withdraw the service or to change the terms on which the service is provided to customers at any time. Deduktibel bermaksud amaun yang perlu anda bayar sebelum Sun MaxiMed-i mula membayar baki perbelanjaan perubatan. Berikut disenaraikan perkhidmatan komplementari yang ditawarkan oleh Best Doctors: i) Perkhidmatan InterConsultation TM Dengan perkhidmatan ini, kes perubatan anda akan disemak oleh salah seorang daripada pakar utama dunia. Ia menyediakan analisis mendalam mengenai keadaan spesifik anda dan satu laporan bertulis yang lengkap akan disediakan untuk anda gunakan bersama doktor yang merawat anda. Ia memastikan anda mendapat diagnosis yang tepat dan pilihan rawatan yang terbaik. ii) DocOnLine (Doktor Online) Dengan perkhidmatan dalam talian, anda boleh menghantar sebarang pertanyaan perubatan kepada Pengamal Perubatan (GP) untuk mendapatkan jawapan yang jelas dan boleh dipercayai. iii) Perkhidmatan AskTheExpert Dengan perkhidmatan ini, anda akan mendapat bimbingan dari pakar global. Best Doctors boleh mendapatkan jawapan dengan pantas untuk soalan-soalan anda. Yakinlah bahawa anda akan mendapat jawapan yang khusus mengikut keperluan anda dan daripada sumber yang boleh dipercayai. Nikmati perlindungan untuk keluarga terdekat anda dan jimatkan kos dengan Pakej Keluarga (iaitu pasangan dan anak tanggungan). Setiap orang yang dilindungi akan dikeluarkan kontrak individu selain menikmati diskaun 10% daripada sumbangan yang asal. Tambahan lagi, perlindungan juga boleh dilanjutkan kepada ahli keluarga tambahan atau baru selepas kontrak dikeluarkan kerana tambahan tanggungan yang seterusnya adalah dibenarkan. * Tertakluk kepada terma-terma dan syarat-syarat Ketika anda berumur di antara 55 dan 59 tahun (berdasarkan umur pada hari jadi terakhir), anda boleh memilih untuk mengurangkan amaun deduktibel kepada sifar dan menukar Sun MaxiMed-i kepada pelan penuh, tanpa Pilihan Persaraan (samb.) Boleh-alih memerlukan sebarang pengunderaitan, dengan syarat bahawa kontrak telah berkuatkuasa sekurang-kurangnya 5 tahun. Dengan tiada deduktibel untuk dibimbangkan, anda boleh terus menikmati perlindungan walaupun setelah bersara. Selepas melaksanakan pilihan ini, kadar sumbangan, kadar yuran wakalah dan kadar tabarru yang berlainan akan dikenakan. Perkongsian lebihan Tabarru (Derma) yang dikutip akan diperuntukkan ke dalam dana tabarru peserta (PTF). Ia akan digunakan untuk membantu semua orang yang dilindungi yang mengalami kesukaran kewangan dan untuk membayar manfaat takaful. Pada akhir setiap tahun kewangan, sebarang lebihan yang diperolehi dari PTF selepas pembayaran balik Qard (jika terdapat pinjaman tanpa faedah yang kami sediakan sekiranya berlaku defisit dalam PTF) akan dikongsi antara peserta yang layak dan Sun Life Malaysia pada nisbah 50:50. Keuntungan pelaburan Baki sumbangan selepas ditolak yuran wakalah akan dikekalkan dalam akaun peserta. Pemegang kontrak dan Sun Life Malaysia akan berkongsi keuntungan pelaburan (jika ada) daripada akaun peserta, masing-masing pada nisbah 90% dan 10% pada akhir setiap tahun kewangan. Sekiranya berlaku kerugian pelaburan dalam akaun peserta, tiada perkongsian keuntungan pelaburan akan dibuat dari akaun peserta untuk tahun tersebut. Komisen Komisen yang perlu dibayar hanyalah 15% daripada setiap sumbangan dibayar dan akan ditanggung oleh pemegang kontrak, yang membentuk sebahagian daripada yuran wakalah. Contoh: Sumbangan tahunan yang dinyatakan di sini adalah untuk lelaki berumur 35 tahun dengan kelas pekerjaan 1 dan risiko standard. Pengecualian-pengecualian (Nota: Senarai ini adalah tidak menyeluruh. Sila rujuk kepada dokumendokumen kontrak untuk maklumat lanjut mengenai pengecualianpengecualian) Tiada manfaat akan dibayar untuk sebarang rawatan perubatan yang diterima oleh orang yang dilindungi yang berkaitan dengan penyakit yang disenaraikan di bawah penyakit tertentu dan/atau sebarang keadaan perubatan atau fizikal yang timbul semasa tempoh menunggu, kecuali untuk kecederaan akibat kemalangan. Pelan ini tidak melindungi sebarang kemasukan hospital, pembedahan atau caj yang disebabkan secara langsung atau tidak langsung, keseluruhan atau sebahagian, atau akibat mana-mana satu yang berikut: i) Keadaan sedia ada; ii) Penyakit tertentu yang berlaku dalam tempoh 120 hari bagi perlindungan berterusan; iii) Sebarang keadaan perubatan atau fizikal yang wujud dalam tempoh 30 hari pertama dari tarikh kontrak dikeluarkan atau tarikh pengembalian semula yang terakhir orang yang dilindungi, kecuali untuk kecederaan akibat kemalangan; iv) Pembedahan plastik/kosmetik, penyunatan, pemeriksaan mata, cermin mata dan pembiasan atau pembedahan pembetulan rabun dekat (Keratomi Radial atau Lasik) dan penggunaan atau pemerolehan peralatan prostetik luaran atau peralatan seperti tangan dan kaki palsu, alat bantuan pendengaran, alat perentak jantung yang diimplan dan preskripsinya;

14 v) Keadaan pergigian termasuk rawatan pergigian atau pembedahan oral kecuali disebabkan oleh kecederaan akibat kemalangan pada gigi asli yang berlaku sepenuhnya dalam tahun kontrak; vi) Penjagaan persendirian, pemulihan secara rehat atau penjagaan kebersihan, dadah yang tidak dibenarkan, keracunan, pensterilan, penyakit kelamin dan komplikasinya, AIDS (Sindrom Kurang Daya Tahan Penyakit) atau ARC (Kompleks berkaitan dengan AIDS) dan penyakit berkaitan dengan HIV, dan sebarang penyakit berjangkit yang memerlukan kuarantin oleh undang-undang; vii) Sebarang rawatan atau pembedahan bagi keabnormalan/ kecacatan kongenital termasuk penyakit keturunan; viii) Kehamilan, kelahiran (termasuk kelahiran secara pembedahan), keguguran, pengguguran kandungan dan penjagaan sebelum lahir/selepas lahir dan kaedah-kaedah perancangan keluarga secara pembedahan, mekanikal atau kimia atau rawatan berkaitan dengan kemandulan. Mati pucuk dan ujian atau rawatan berhubung dengan impoten atau pensterilan; ix) Kemasukan hospital yang bertujuan terutamanya untuk penyiasatan, diagnosis, pemeriksaan x-ray, pemeriksaan fizikal atau perubatan am, yang tidak berkaitan dengan rawatan atau diagnosis hilang upaya yang dilindungi atau sebarang rawatan yang tidak diperlukan dari segi perubatan dan sebarang rawatan pencegahan, ubat-ubatan atau pemeriksaan pencegahan yang dilakukan oleh pakar perubatan, dan rawatan yang khususnya untuk pengurangan atau penambahan berat badan; x) Sebarang rawatan pesakit luar kecuali yang diperuntukan secara khusus di bawah kontrak ini; xi) Bunuh diri, percubaan membunuh diri atau mencederakan diri sendiri dengan sengaja semasa waras atau tidak waras; xii) Peperangan atau tindakan peperangan, yang diisytiharkan atau tidak, aktiviti jenayah atau keganasan, berkhidmat secara aktif dalam mana-mana angkatan bersenjata, penyertaan secara langsung dalam tindakan, rusuhan dan kekecohan awam atau pemberontakan; xiii) Radiasi pengionan atau pencemaran radioaktif dari sebarang bahan api nuklear atau sisa nuklear dari proses pembelahan nuklear atau daripada sebarang senjata nuklear; xiv) Perbelanjaan yang ditanggung untuk pendermaan sebarang organ oleh orang yang dilindungi dan kos untuk mendapatkan organ termasuk semua kos yang ditanggung oleh penderma semasa pemindahan organ atau komplikasinya; xv) Penyiasatan dan rawatan bagi masalah tidur dan berdengkur, terapi penggantian hormon dan terapi alternatif seperti rawatan, perkhidmatan atau bekalan perubatan, termasuk tetapi tidak terhad kepada perkhidmatan kiropraktik, akupunktur, akupresur, refleksologi, pembetulan tulang, rawatan herba, terapi urutan atau aroma atau rawatan alternatif yang lain; xvi) Penjagaan atau rawatan yang mana bayaran adalah tidak diperlukan atau sehingga yang akan dibayar oleh perlindungan takaful/insurans yang lain atau indemniti yang melindungi orang yang dilindungi dan hilang upaya yang wujud akibat tugas pekerjaan atau profesion yang dilindungi di bawah kontrak takaful/ polisi insurans pampasan pekerja; xvii) Psikotik, gangguan mental atau saraf (termasuk sebarang neurosis dan manifestasi psikologikal atau psikomatik); xviii) Kos/Perbelanjaan bagi perkhidmatan bukan perubatan, seperti televisyen, telefon, perkhidmatan teleks, radio atau kemudahan seumpamanya, kit/pek kemasukan hospital dan sebarang barangbarang bukan perubatan yang tidak layak; xix) Penyakit atau kecederaan yang wujud akibat sebarang jenis perlumbaan (kecuali perlumbaan kaki), sukan berbahaya seperti dan tidak terhad kepada terjunan udara, ski air, aktiviti dalam air yang memerlukan peralatan pernafasan, sukan musim sejuk, sukan professional dan aktiviti-aktiviti haram; xx) Penerbangan persendirian selain sebagai penumpang yang membayar tambang bagi sebarang penerbangan komersial berjadual yang dilesenkan untuk membawa penumpang mengikut laluan yang ditetapkan; atau xxi) Perbelanjaan yang ditanggung untuk prosedur pertukaran jantina. Penyakit tertentu bermaksud hilang upaya yang seperti berikut dan komplikasi berkaitan dengannya, berlaku dalam tempoh 120 hari pertama dari tarikh kontrak dikeluarkan. Walau bagaimanapun, jika berlaku pemberhentian perlindungan dalam tempoh perlindungan sebelum tarikh tamat tempoh 120 hari tersebut, tempoh yang baru selama 120 hari akan dikenakan semula dari tarikh pengembalian semula. i) Tekanan darah tinggi, kencing manis dan penyakit kardiovaskular; ii) Kesemua ketumbuhan, kanser, sista, nodul, polip, batu dalam sistem kencing dan sistem biliari; iii) Kesemua penyakit telinga, hidung (termasuk sinus) dan tekak kecuali selsema dan sakit-tekak; iv) Hernia, hemoroid, fistula, hidrosele, varikosele; v) Endometriosis termasuk penyakit sistem pembiakan; dan vi) Gangguan spina vertebro (termasuk diska) dan keadaan lutut. Tempoh menunggu bermaksud 30 hari pertama di antara permulaan hilang upaya bagi orang yang dilindungi dan tarikh kontrak dikeluarkan untuk orang yang dilindungi. Ini tidak akan dikenakan selepas perlindungan tahun pertama. Walau bagaimanapun, jika pemberhentian perlindungan berlaku dalam tempoh perlindungan, tempoh menunggu akan dikenakan semula. Keadaan sedia ada bermaksud sebarang keadaan atau penyakit atas orang yang dilindungi yang wujud sebelum tarikh kontrak dikeluarkan atau tarikh pengembalian semula terakhir, yang diketahui sewajarnya oleh orang yang dilindungi. Orang yang dilindungi dianggap mengetahui sewajarnya keadaan sedia ada tersebut yang mana: i) orang yang dilindungi telah menerima atau sedang menerima rawatan; ii) nasihat perubatan, diagnosis, jagaan atau rawatan telah dicadangkan; iii) iv) gejala yang jelas dan tepat dapat atau telah dilihat dengan nyata; atau kewujudannya dapat diperhatikan dengan jelas bagi orang yang mengalami keadaan tersebut. Nota-nota penting 1) Ini adalah risalah produk dan bukan dokumen kontrak. Sila rujuk kepada dokumen keterangan produk untuk maklumat lanjut mengenai pelan dan juga dokumen kontrak untuk terma-terma dan syarat-syarat lengkap di bawah pelan ini. 2) Anda seharusnya berpuas hati bahawa pelan ini memenuhi keperluan anda dengan sebaiknya dan memastikan bahawa sumbangan yang perlu dibayar di bawah pelan ini adalah amaun yang anda mampu bayar. 3) Anda boleh membatalkan kontrak anda dalam tempoh 15 hari selepas ia dihantar dan amaun bayaran balik yang anda terima adalah jumlah sumbangan ditolak sebarang perbelanjaan yang ditanggung untuk sebarang pengunderaitan perubatan yang diperlukan.

15 4) Sekiranya anda membatalkan kontrak anda pada tahun-tahun awal, anda mungkin mendapat kembali amaun yang kurang daripada jumlah yang telah anda bayar. 5) Sekiranya anda tidak membayar sumbangan dalam tempoh masa 30 hari, terdapat kemungkinan di mana pada bila-bila masa nilai akaun peserta anda tidak mencukupi untuk menyokong kontrak dan kontrak anda akan luput. 6) Amaun sumbangan yang dibayar boleh dituntut sebagai pelepasan cukai pendapatan, tertakluk kepada Lembaga Hasil Dalam Negeri. 7) Semua cukai, termasuk tetapi tidak terhad kepada sebarang cukai barangan dan perkhidmatan, dan/atau lain-lain bentuk cukai jualan atau kepenggunaan, sama ada yang berkuatkuasa pada masa ini atau dilaksanakan selepas tarikh kontrak akan dikenakan mengikut undang-undang yang berkenaan pada kadar semasa. Jika perlu, Sun Life Malaysia akan meminda terma-terma kontrak untuk mengambil kira cukai-cukai sedemikian. 8) Amaun sumbangan dan/atau yuran wakalah dan/atau tabarru adalah tidak dijamin dan boleh disemak dengan memberi notis bertulis sekurang-kurangnya 30 hari kepada anda dan semakan tersebut akan dilaksanakan pada ulang tahun kontrak yang berikutnya selepas tamat tempoh notis tersebut. 9) Sun Life Malaysia berhak untuk membatalkan produk secara keseluruhan jika ia mengambil keputusan untuk tidak menerima risiko untuk produk ini dengan memberi notis kepada pemegang kontrak sekurang-kurangnya 30 hari sebelum ulang tahun kontrak. Dalam keadaan ini, nilai akaun peserta, jika ada, akan dibayar dan tiada caj penyerahan dikenakan. 10) Best Doctors bukanlah suatu pelan/produk insurans/takaful dan ia tidak akan menggantikan hubungan anda dengan doktor ataupun pakar perubatan anda. Sun Life Malaysia berhak untuk menarik balik perkhidmatan atau mengubah terma-terma perkhidmatan yang disediakan kepada pelanggan pada bila-bila masa. Sun Life Malaysia dan CIMB Islamic Bank tidak akan bertanggungjawab ke atas sebarang cadangan atau pendapat yang diberikan oleh Best Doctors. 11) Pelan ini diuruskan oleh Sun Life Malaysia Takaful Berhad ( M), pengendali Takaful yang berdaftar dengan Bank Negara Malaysia di bawah Akta Perkhidmatan Kewangan Islam ) Pelan ini diedarkan oleh CIMB Islamic Bank Berhad ( H). CIMB Bank Berhad dan CIMB Islamic Bank Berhad berpejabat di Menara Bumiputra-Commerce, 11, Jalan Raja Laut, Kuala Lumpur, Malaysia.
