4 摘要 细胞因子诱导的杀伤细胞 (Cytokine-induced Killer Cell,CIK) 是人外周血单个核细胞 (PBMCs) 经细胞因子诱导成的具有肿瘤杀伤能力的一群异质细胞 目前,CIK 细胞被认为是肿瘤过继免疫细胞治疗的首选细胞, 但 CIK 细胞对 肿瘤细胞并不是特异性杀伤, 其抗肿瘤效果还有待进一步提高 双特异抗体通过 抗原 - 抗体 的形式特异性地联结 CIK 细胞和靶细胞, 增强了 CIK 细胞杀伤肿 瘤细胞的靶向性和特异性, 大大提高了 CIK 细胞对肿瘤细胞的杀伤活性 本文的 目的在于建立双特异抗体 anti-cd3xanti-afp 介导 CIK 细胞对 AFP 阳性肝癌细胞 的特异杀伤 利用 anti-cd3 联合细胞因子 IL-2 活化人外周血淋巴细胞 (PBMCs) 扩增 CIK 细胞 在显微镜下观察细胞生长状况, 台盼蓝染色法进行活细胞计数, 记录生长 曲线 用流式细胞仪对第 和 21 天的 CIK 细胞进行表型鉴定 CIK 细胞 毒性检测实验中使 CIK 细胞和肝癌细胞 Smmc-7721 的靶效比分别为 3:1 6:1 12:1 25:1 50:1 和 100:1,Alarma Blue 法检测 CIK 细胞毒活性, 激光扫 描共聚焦技术观察细胞毒效应 柠檬酸三钠还原法制备金溶胶, 透射电子显微镜 观察制备的纳米金物理特性 物理吸附制备载荷 anti-cd3 和 anti-afp 两种单抗 的胶体金制剂, 利用胶体金在普通光下呈现红色的特性, 光学显微镜检测金标抗 体结合相关细胞膜的情况 Alarma Blue 法检测双特异抗体介导 CIK 细胞对肝癌 细胞 HepG-2 和宫颈癌细胞 Hela 的杀伤活性 在培养过程中,CIK 细胞第 4 天起开始出现成团现象, 进入快速生长期, 到 第 13 天时 CIK 细胞数扩增了约 39.6 倍 流式细胞术结果显示,CIK 细胞中 CD3 + 细胞亚群的比在培养过程中不断提高, 第 和 21 天时分别为 11.42%, 79.38% 94.4% 和 95.39%, 经过 14 天的培养,CD3 + CD56 + 细胞亚群也由培养之初的 3.05% 上升到 18.64%,CD4 + CD8 + 细胞亚群的比例在第 和 21 天时分 别为 13.61% 6.43% 1.02% 1.21% 和 11.39% 66.38% 77.23% 70.81% CIK 细胞毒性检测试验中, 效靶比为 3:1 6:1 12:1 25:1 50:1 和 100:1 时,CIK 细胞对 Smmc-7721 细胞的杀伤效率分别为 25.2% 32.5% 48.6% 56.8% 69.6% 和 90.3% 激光扫描共聚焦技术观察细胞毒效应时, 可见 CIK 细胞可粘附
5 在肿瘤细胞膜表面裂解肿瘤细胞, 而且还能穿入受损的肿瘤细胞膜进入肿瘤细胞内部进行裂解 柠檬酸三钠还原法制备的金溶胶, 在透射电子显微镜观察粒径比较均一, 颗粒直径平均为 14.7nm, 分散性较好 在光学显微镜下可以观察到相应的细胞膜上具有红色的胶体金颜色, 说明胶体金标记的抗体能特异地结合到相应的细胞膜 上面, 表明金标抗体具有很好的活性 Alarma Blue 法检测双特异抗体介导 CIK 细胞对肝癌细胞 HepG-2 和宫颈癌细胞 Hela 的杀伤活性中,CIK 细胞对 HepG-2 肝癌细胞杀伤率 (42%) 比对照组 (30.6%) 高出 11.4%(P<0.05), 而对 Hela 宫 颈癌细胞的杀伤率无明显变化 (P>0.05) 以上结果表明加入金标双特异抗体后, anti-cd3xanti-afp 能特异地结合到 CIK 细胞和 HepG-2 肝癌细胞的相关抗原上, 通过两者的特异接触, 实现两者的特 异性靶向, 可进一步提高 CIK 细胞对 HepG-2 细胞的杀伤作用 本文的结论是双特 异抗体 anti-cd3xanti-afp 可以建立 CIK 细胞对 AFP 阳性肝癌细胞的特异杀伤 关键词 :CIK 细胞 ; 肝癌细胞 ; 双特异抗体 ; 胶体金 ;AFP
6 Abstract Cytokine-induced killer cell(cik)is a group of heterogenous cells induced by cytokines from human peripheral blood mononuclear cells(pbmcs), which have an anti-tumor activity. To date, CIK cell is the most important effecter cells in the adoptive cellular immunotherapy of anti-tumor, but CIK cells cannot kill tumor cells specifically. There is more improvement about anti-tumor ability of CIK cell. Bispecific antibody could bridge CIK cells and target cells specifically through the binding of antibody to antigen, killing activity of CIK cell on tumor cells was greatly improved through CIK homing and specific binding to targets. The purpose of this paper was to establish the method that CIK cells mediated by the bispecific antibody anti-cd3xanti-afp lyse specifically AFP positive hepatoma cells. PBMCs from Human Peripheral Blood were induced into CIK cells by a combination of anti-cd3 antibody and IL-2, and CIK was amplified by IL-2. The growth status of CIK was observed by a microscope. The amount of cells alive was counted by trypan blue dye method, and cell proliferation was also recorded. The cell phenotype was analyzed by flow cytometry on day 1, 8, 14 and 21. Cytotoxicity experiment of CIK cell on tumor cells was tested by Alarma Blue. Effecter-target ratio was 3:1 6:1 12:1 25:1 50:1 and 100:1. Cytotoxicity effect of CIK was detected by laser scanning confocal microscope. Gold colloid was prepared with the method of citric acid three sodium revert, bispecific colloidal gold antibody anti-cd3xanti-afp were prepared by physical adsorption, activities of antibodies were detected by light microscopy with the reason of that colloidal gold is red under natural light. Cytotoxic activities of CIK cells on hepatoma cells HepG-2 and cervical carcinoma cells Hela were tested by Alarma Blue assay. In the induction of CIK, CIK appeared to proliferate by congerating into cell groups on day 4, CIK began to enter the fast growing phase after inducing day 4, and the total cell number on day 13 was 39.6 times of cell number on day 1. Flow cytometry assay revealed that the cell number of CD3 + subgroup in CIK constantly
7 increased, the percentage of this subgroup in CIK was 11.42%, 79.38%, 94.4% and 95.39% on day 1, 8, 14, 21 respectively. The percentage of CD3 + CD56 + subgroup, main effecter cells in CIK, also increased from 3.05% on day 1 to 20.56% on day 14. And on day 1, 8, 14 and 21, the percentage of CD4 + cells in CIK was 13.61%, 6.43%, 1.02%, 1.21% and the percentage of CD8 + cells was 11.39%, 66.38%, 77.23%, 70.81% respectively. Cytotoxicity experiment detected by laser scanning confocal microscope showed that CIK cell could lyse target cell by connecting to target cell, even CIK cell could enter target cell through the wound cellular membrane of target cell. Gold colloid particle average diameter was 14.7 nm, the diameter showed small difference, so gold colloid particle dispersion was good. In the staining of colloidal gold antibody anti-cd3xanti-afp, the cell membranes of both CIK cells and HepG-2 cells were stained with red, but no signal was seen in Hela cells. This meant gold labeled antibody kept activity of specific binding to antigen. Targeted by the bi-specific antibody, the efficiency of CIK lysing to Hepg-2 cells increased by 11.4% (30.6%-42%; P < 0.05), but the efficiency of CIK lysing to Hela cells showed no significant increase(27%-26.5%; P>0.05). Results above showed that gold labeled antibody anti-cd3xanti-afp could bind specifically CIK cells and HepG-2 cells, and the cytotoxicity of CIK cell on target tumor cells was greatly enhanced by this double targeting. Our conclusion was that CIK cells targeted by the bispecific colloid gold antibody anti-cd3xanti-afp could lyse and kill specifically AFP positive hepatoma cells. 12 双特异抗体 CD3xAFP 介导 CIK 细胞对 AFP 阳性肝癌细胞的杀伤应用 第一章前言 一癌症的治疗现状及展望 1.1 癌症治疗现状癌症目前仍是人类健康的一大杀手 全球癌症最新调查结果显示, 全球年新 发肿瘤患者 1090 万人, 因癌症死亡 670 万人, 带病生存人数 2460 万人 各个国家癌症的发病情况 死亡率和流行病发病趋势是不同的 中国癌症的发病人数占 世界总发病人数的 20.3%, 死亡人数占世界总死亡人数的 23.8%, 在世界范围内 均是最高的 综合调查显示, 肺癌是最常见的肿瘤, 年新发肺癌患者 135 万, 死 亡 118 万, 因其发病率及病死率高而深受关注 乳腺癌发病率位居第二, 年新发 患者 115 万, 但病死率仅位于第五位 病死率前五位的肿瘤还有胃癌 ( 年新发 93.4 万, 死亡 70 万 ), 肝癌 ( 年新发 62.6 万, 死亡 59.8 万 ), 结直肠癌 ( 年新发 102 万, 死亡 52.9 万 ) 据世界癌症报告, 到 2020 年全球癌症发病率将上升 50%, 发病人数达到 1500 万 癌症治疗方法的进展主要来自于临床实验的结果 对于现在癌症患者来说, 手术 放疗 化疗依然是基本方法 [1] 外科治疗是采取手术切除肿瘤的方法达到 治疗肿瘤的重要手段, 在治疗早期没有转移和与重要脏器及大血管无严重粘连的 肿瘤方面效果较好, 但中晚期效果则不如早期 放疗是利用同位素衰变和加速器 产生的射线治疗恶性肿瘤 主要适用于肿瘤全身扩散前 相对局限 对射线敏感 肿瘤, 如食管癌 肺癌 鼻咽癌 喉癌 上颌窦癌 乳癌 脑瘤 皮肤癌等 肿 瘤一旦扩散或局部侵润广泛, 放疗就达不到治疗效果, 且放疗本身也会带来一些 近区或远区副作用 化疗是利用化学抗肿瘤药物治疗恶性肿瘤的主要手段 主要 适用于对抗癌药物敏感性肿瘤及身体状况良好者, 但化疗药物毒副作用较大, 主 要有恶心 呕吐 脱发 白细胞减少等 但是手术仅对早期肿瘤可达到根治, 而真正对放疗 化疗敏感的癌症只占人类全部癌症的百分之十左右 然而绝大多数 的癌症患者确诊时都已处于中晚期, 失去了手术的最佳时期 特别是大部分肿瘤, 尤其是老年人的肿瘤恶性度并不高 生长缓慢, 对放疗 化疗不甚敏感 现在许多晚期癌症患者采取手术 放疗 化疗相联合的治疗办法, 这种综合治疗方法已经获得肯定 但仍然有很多癌症是不能治愈的, 这就需要其他的治疗方法 肿瘤生物治疗作为一种新的治疗技术为肿瘤的治疗带来了新的希望 - 1 -
13 双特异抗体 CD3xAFP 介导 CIK 细胞对 AFP 阳性肝癌细胞的杀伤应用 1.2 癌症生物治疗肿瘤的生物治疗是在肿瘤免疫治疗的基础上发展起来的 在肿瘤外科治疗 放射治疗和化学治疗发展的同时, 肿瘤生物治疗近年来日益受到重视, 并已成为足以与手术 放疗 化疗并列的肿瘤的第四种治疗模式, 显示良好的临床应用前景 生物技术的发展开创了肿瘤生物治疗的新纪元 淋巴因子 / 细胞因子技术 免疫活性细胞继承性输注技术 单克隆抗体及其偶联物技术和肿瘤疫苗技术是 20 世纪 80 年代提出的四大生物治疗技术 ;90 年代以来, 基因治疗技术已经显示 出新苗头 肿瘤的基因治疗就是应用基因工程技术直接纠正肿瘤细胞基因的结构 及 ( 或 ) 功能缺陷, 或者间接通过增强宿主对肿瘤的杀伤和防御能力来治疗肿瘤, 这是肿瘤生物治疗的崭新领域 这方面的研究方兴未艾, 思路日新月异 肿瘤基 因治疗是一个跨世纪的研究课题, 是 21 世纪人类攻克肿瘤的新战场 免疫治疗的基本概念 免疫系统 免疫系统的功能 : 免疫是机体识别 排除 消灭异物的一种功能, 包括体液 免疫和细胞免疫两种 体液免疫由骨髓衍生的淋巴细胞即 B 细胞所介导, 细胞 免疫则是由胸腺衍生的淋巴细胞即 T 细胞所介导 体液免疫 : 主要成分为抗体和补体 当抗原进入机体后, 先由巨噬细胞捕获 并将加工处理的抗原信息传递给淋巴细胞,B 细胞初次接触抗原 1~2 天以后即 可产生低效价的抗体 以后如果再遇到相同抗原,B 细胞在 24 小时内即可恢复 对抗原信息的记忆而产生高浓度的抗原 补体是存在于体液中包括血清 β- 球蛋 白在内的一系列复杂的蛋白系统, 被抗原抗体反应激活, 协助免疫球蛋白消灭异 物 免疫抗体在补体的协同下对异物或肿瘤靶细胞发挥细胞毒作用, 这类抗体称 补体依赖性抗体 此外, 还有淋巴细胞依赖性抗体, 抗体与淋巴细胞协同杀伤靶 细胞, 效应细胞为 K 细胞, 而抗体在靶细胞和 K 细胞这间起接触介导作用 细胞免疫 : 主要有两类细胞参与, 即致敏 T 淋巴细胞和巨噬细胞 巨噬细胞 吞噬加工抗原, 抗原信息输送到淋巴细胞后, 在致敏 B 细胞的同时也致敏 T 细胞, 致敏 T 细胞不但能直接杀伤靶细胞, 而且能调动巨噬细胞一起破坏靶细胞 同时抗原再一次刺激致敏 T 细胞, 可使之发生高代谢率, 并分泌淋巴因子 多种淋巴因子联合作用, 可聚集吞噬细胞至抗原附近, 抑制免疫活性细胞离开抗原区, - 2 -
14 双特异抗体 CD3xAFP 介导 CIK 细胞对 AFP 阳性肝癌细胞的杀伤应用 促使非致敏淋巴细胞繁殖活化, 从而加强了对其他异常细胞的吞噬作用 目前认为细胞免疫为机体抵抗肿瘤的最主要环节之一 人类肿瘤特异性抗原近年来随着免疫学技术的发展, 人们运用体内体外的特异性的免疫学方法, 已经获得比较充足的实验证据, 并证明肿瘤特异性及相关性抗原的存在和机体免 疫反应对肿瘤有抑制作用 通过对患者进行免疫抗体的检测, 发现许多肿瘤病人存在着对肿瘤细胞起特异反应的抗体 例如从鼻咽癌细胞培养中分离出 EB 病毒, 从患者血清中也发现有 EB 病毒外壳抗原 膜抗原 早期抗原和沉淀抗原的抗体, 这些抗体能与培养的鼻咽癌细胞发生反应, 证明了肿瘤抗原的存在和特异性 应 用抗体免疫荧光技术从鼻咽癌组织中检出 EB 病毒决定的核抗原, 应用核酸分子 杂交技术也证实鼻咽癌组织中含有 EB 病毒基因组 近年来, 发现约 1/3 的黑色 素瘤患者血清能识别自身肿瘤细胞抗原, 但不能识别其他患者的瘤细胞和自身其 他细胞 因此, 判定肿瘤抗原为特异性抗原, 能被自身抗血清识别 应用近代免 疫学方法, 相继发现肾母细胞瘤 结肠癌 宫颈癌 神经母细胞瘤 淋巴瘤 胃 癌 骨肉瘤等等许多肿瘤都存在着肿瘤抗原, 这些发展都为肿瘤的免疫治疗提供 了依据 肿瘤抗原这个名词概括了细胞恶变过程中肿瘤细胞出现的许多表现有免疫 原性分子的总和, 由于缺乏理想的特异性对照细胞, 而且在测定抗原抗体反应过 程中难以区别肿瘤特异性抗原组织的其他抗原成分, 所以对目前用上述技术方法 揭示的肿瘤抗原的特异性仍有争议 在这些方面, 要想获得直接的证据还有待进 一步对抗原的分离提纯和鉴定 宿主与肿瘤间的相互作用 宿主对肿瘤的免疫反应 人体抗肿瘤的免疫反应, 主要是排斥肿瘤的作用, 其中细胞免疫对肿瘤的排斥作用尤为重要 细胞免疫 : 致敏 T 淋巴细胞通过表面特异的抗原受体与靶细胞接触, 直接发挥杀伤作用, 这部分细胞称杀伤 T 细胞, 其对肿瘤细胞直接的杀伤和抑制作用发生性, 特异性较高 细胞免疫杀伤肿瘤细胞的另一个途径, 就是通过致敏 T 细胞释放的多种功能复杂的淋巴因子, 协同参与肿瘤细胞的遏止和杀伤, 扩大细胞免 - 3 -
15 双特异抗体 CD3xAFP 介导 CIK 细胞对 AFP 阳性肝癌细胞的杀伤应用 疫反应效应 ; 其中促分裂因子和转移因子可以引起免疫的连锁反应, 并使更多的 T 细胞参与反应 ; 淋巴毒素直接破坏肿瘤靶细胞 ; 巨噬细胞迁移因子 巨噬细胞武装因子 趋化因子等能促使巨噬细胞聚集发挥协同作用 巨噬细胞的功能在细胞免疫中至关重要 除了在免疫反应初始阶段吞噬处理传递抗原信息以外, 巨噬细胞还具有抗体依赖性的细胞毒作用, 借助细胞抗体与肿瘤靶细胞紧密接触发挥 杀伤细胞的抗肿瘤细胞免疫机理 自然细胞 (Natural Killer cell) 是免疫监视的第一道防线, 无需预先致敏即可迅速杀伤溶解细胞 体液免疫 : 被抗原致敏的 B 细胞分化成浆细胞, 可产生对肿瘤靶细胞起破 坏作用的抗体 一类是补体依赖性细胞毒性抗体, 这种抗体在补体协同下破坏靶 细胞, 实验证明淋巴瘤和白血病等病人血液中可以找到细胞毒抗体 第二类是淋 巴细胞依赖性抗体, 其 Fc 段与 K 细胞结合,Fab 段与肿瘤细胞结合, 介导 K 细 胞杀伤瘤细胞 此外, 还有一类亲细胞抗体, 介导巨噬细胞对瘤细胞的细胞毒作 用 在体液中存在着非特异性体液因子, 如干扰素 调理素 补体等, 在杀伤肿 瘤细胞中也具有一定的促进作用 肿瘤逃避免疫监视的机理 在机体一系列免疫系统的监视下, 肿瘤仍然可以持续生长, 这说明肿瘤存在 着某种逃避宿主免疫破坏的途径 肿瘤可以通过以下因素来实现其免疫逃避 : 可溶性肿瘤抗原 : 肿瘤的抗原成分以可溶性形式释放至细胞外液, 与肿瘤局 部的特异抗体结合, 使致敏淋巴细胞失去对瘤细胞的识别和攻击作用, 抑制和麻 痹局部淋巴结的免疫功能造成肿瘤细胞的局部逃避 当肿瘤进一步生长后又能释 放更多的可溶性抗原进入淋巴 血液循环, 导致肿瘤的全身逃避 肿瘤抗原和抗 原抗体复合物也可以同致敏 T 细胞 K 细胞和巨噬细胞表面的特异受体结合, 使 这些细胞推动对瘤组织的特异破坏作用 封闭因子 : 这种因子存在于肿瘤病人血清中, 当其覆盖在肿瘤细胞表面抗原上时, 可保护肿瘤细胞免受致敏淋巴细胞的破坏 在带瘤情况下, 封闭因子的封 闭活性较明显, 切除肿瘤后, 封闭活性可消失 最初人们认为封闭因子是宿主产生的一种抗体, 称为 封闭抗体, 但是与肿瘤切除后封闭因子可迅速消失的现象不相符合 非特异性免疫抑制因子 : 近年来大量的实验结果及临床观察都证明肿瘤细胞 - 4 -
16 双特异抗体 CD3xAFP 介导 CIK 细胞对 AFP 阳性肝癌细胞的杀伤应用 能产生免疫抑制因子, 并能全面抑制机体的免疫反应, 包括细胞免疫和体液免疫, 这种抑制作用主要通过抑制 T 细胞和 B 细胞甚至 T-B 细胞之间的协同功能完成的, 这是降低肿瘤免疫功能的非特异因素之一 经过葡聚糖凝胶 G-50 和十二烷基硫酸钠聚丙烯酰胺凝胶电泳分析, 推测免疫抑制因子可能是分子量 的蛋白质 免疫治疗的临床应用 免疫治疗的选用条件 目前肿瘤免疫治疗仍处于临床探索阶段, 下面介绍的是选择免疫治疗所能参 考的因素 肿瘤与机体的比势 : 一般认为在肿瘤负荷量小的时候选择免疫治疗效果较 好 肿瘤细胞数在 10 6 以下, 肿瘤直径在 1mm, 一般属于临床上很难发现的亚临 床期肿瘤, 最多不超过 10 8 为免疫治疗有效的治疗时机, 或者仅作为手术或放疗 后的残余肿瘤的治疗和预防转移的治疗 因此, 免疫治疗应作为恶性肿瘤治疗的 一种辅助疗法, 通过化疗 放疗和手术切除将肿瘤组织最大限度地减量, 使机体 居于有利的优势状态下给以免疫治疗, 这是肿瘤免疫治疗的重要对象 血清封闭作用 : 在治疗前应进行血清封闭性的检查 因为血清封闭作用在客 观上保护肿瘤细胞使之免受免疫反应的攻击, 是对免疫治疗效果的一个十分不利 的因素 在可能的情况下应尽量克服血清的这种封闭作用 现阶段所能选择的方 法有, 采取化疗 放疗或手术使瘤组织减少以减少和根绝封闭因子的来源 ; 利用 肿瘤恢复期病人血清中去封闭因子拮抗封闭因子的作用 ; 应用蛋白 A 等免疫吸 附剂清除血清封闭因子 机体免疫状态 : 在机体免疫反应良好的情况下进行免疫治疗才能收到预期的 效果 在带瘤状态下, 机体可因为肿瘤对免疫系统的抑制和化疗对免疫的破坏, 使免疫治疗难以奏效 因此, 设法解除机体的免疫抑制状态, 才能发挥免疫治疗的疗效 肿瘤细胞的抗原性 : 增强肿瘤细胞的抗原性, 激发较强的免疫反应性, 是打破肿瘤免疫耐受性 提高机体免疫抵抗力的一种手段 选择抗原性较强的肿瘤如恶性黑色素瘤 绒毛膜上皮癌 子宫内膜癌 神经母细胞瘤 乳腺癌 白血病 肾癌等进行免疫治疗, 其治疗效果已经在临床上得到证实 对于抗原性较弱的肿 - 5 -
