5 [ 键入文字 ] 摘要目的 : 比较急性脑梗死血瘀证与非血瘀证之间胰岛素抵抗超敏 C 反应蛋白 NIHSS 评分和血脂四项等指标是否有差异性, 探讨急性脑梗死血瘀证与胰岛素抵抗炎症因子及神经功能缺损程度的关系, 从而提高对急性脑梗死血瘀证的认识, 为临床应用活血化瘀法治疗脑梗死提供理论依据方法 : 选取厦门市中医院脑病科 2013 年 12 月至 2014 年 12 月期间内明确诊断急性脑梗死的住院患者, 按照病例纳入标准选取血瘀证组和非血瘀证组病例各 40 例, 两组分别检测空腹血糖, 空腹血清胰岛素,Hs-CRP 血脂四项等相关生化指标, 并同时测评 NIHSS 评分量表, 利用 SPSS18.0 统计软件将所得的相关数据进行统计分析结果 : 1 急性脑梗死血瘀证组年龄(67.18±9.34) 岁高于非血瘀证组 (58.18±7.47) 岁, 收缩压 (166±18.30)mmHg 高于非血瘀证组 (150±14.38)mmHg, 差异有统计学意义 (P ) 2 急性脑梗死血瘀证组 HOMA-IR 为 ( 4.85±0.81 ) 高于非血瘀证组 (3.13±0.75), 差异有统计学意义 (P 0.01), 血瘀证组的 NIHSS 评分为 9.50 (5.15,12.17), 高于非血瘀证组 6.85(3.00,7.25), 差异有统计学意义 (P 0.01), 但是血瘀证组的 Hs-CRP 为 (4.99±5.57)mg/L 与非血瘀证组 (4.33±4.82)mg/L, 差异无统计学意义 (P 0.05) 3 Spearman 相关性分析, 急性脑梗死血瘀证积分与 HOMA-IR(r=0.456, P 0.000) NIHSS(r=0.582,P 0.000) 中度正相关, 但是急性脑梗死血瘀证积分与 Hs-CRP 不相关 (r=0.163,p= ) 4. 二分类资料的 Logistic 回归分析显示, 急性脑梗死血瘀证的危险因素为年龄 (P= ,OR= ), 收缩压 (P= ,OR= ), HOMA-IR( P= ,OR= ) 和 NIHSS(P= ,OR= ),4 个危险因素的作用强度依次为 NIHSS 评分胰岛素抵抗指数年龄和收缩压 ( ) Hs-CRP 和血脂四项不是急性脑梗死血瘀证的危险因素 (P 0.05) I

6 结论 : 急性脑梗死血瘀证出现于年龄较长者和收缩压升高患者, 血瘀证组的胰岛素抵抗及神经功能缺损较非血瘀证患者重, 血瘀证积分高, 则预后差, 临床需要重视血瘀病机关键词 : 急性脑梗死血瘀证胰岛素抵抗美国国立卫生院卒中量表

7 Abstract Objective: Discuss the difference of some criterions, which contains insulin resistance, Hs-CRP, NIHSS and blood lipids, between traditional Chinese blood stasis-syndrome of acute cerebral infarction and traditional Chinese medicine non blood stasis-syndrome of acute cerebral infarction through clinical observation. In addition, explore the further relationship between traditional Chinese medicine stasis-syndrome of acute cerebral infarction and the following three factors: insulin resistance, inflammation and NIHSS, thereby deepen the understanding of traditional Chinese medicine stasis-syndrome of acute cerebral infarction, and provide theoretical basis for eliminating blood stasis. Methods: Choose patients in Xiamen TCM Hospital department of internal neurology who have been definitively diagnosed as acute cerebral infarction between December of 2013 and December of 2014,and select 40 cases from the patients for both blood stasis-syndrome group and non blood stasis-syndrome group respectively by the selection criteria, record biochemical criterion such as Fasting plasma glucose, Fasting insulin, Hs-CRP, TG,TC,HDL-C,LDL-C, etc. for each group, and evaluate NIHSS at the same time. The corresponding relationship was evaluated by analyzing the collected data using SPSS 18. Results: 1. For the group of blood stasis-syndrome of acute cerebral infarction, the age of which (67.18±9.34) is older than the age of non blood stasis-syndrome group (58.18±7.47); and the systolic pressure of which (166±18.30mmHg) is higher than the systolic pressure of non blood stasis-syndrome group (150±14.38mmHg). The differences are statistically significant (P<0.01, 0.05) 2. For the group of blood stasis-syndrome of acute cerebral infarction, the HOMA-IR of which (4.85±0.81) is higher than the HOMA-IR of non blood stasis-syndrome group (3.13±0.75), and the difference is statistically significant (P<0.01); the NIHSS of which is 9.50 (5.15,12.17), higher than the NIHSS of non-stasis-syndrome group 6.85 (3.00,7.25), and the difference is also statistically III

8 significant (P<0.01). The Hs-CRP of stasis-syndrome group is 4.99±5.57mg/L, and the Hs-CRP of non-stasis-syndrome group is 4.33±4.82 mg/l, the difference is not statistically significant (P>0.05). 3. Spearman correlation testing proved that, for stasis-syndrome of acute cerebral infarction, the score of which is moderately correlated to HOMA-IR (r=0.456,p<0.000)and NIHSS (r=0.582,p<0.000); however the score of which is not correlated to Hs-CRP (r=0.163, P=0.147>0.05). 4. Logistic regression analysis of show that the risk factors of traditional Chinese blood stasis-syndrome of acute cerebral infarction were age(p = 0.04 <0.05, OR = 1.204> 1), systolic blood pressure( P = <0.05, OR = 1.017> 1), HOMA-IR( P = <0.05, OR = 1.350> 1) and NIHSS(P = <0.05, OR = 1.568> 1), the strength of four risk factors from high to low were NIHSS score, insulin resistance index, age and systolic blood pressure( ). Hs-CRP and blood lipid are not risk factors of traditional Chinese blood stasis-syndrome of acute cerebral infarction (P> 0.05 ). Conclusions: Blood stasis-syndrome of acute cerebral infarction occurs more frequently in the elders; the insulin resistance and neurological deficit of blood stasis-syndrome group appear to be more serious than non blood stasis-syndrome group; the higher the blood stasis-syndrome scores, the worse its prognosis. Blood stasis-syndrome pathogenesis should be attached more importance in the clinical. Keywords: acute cerebral infarction; blood stasis syndrome; insulin resistance; NIHSS. IV

15 前言前言脑血管病是目前导致世界人口死亡的前三位疾病之一, 据流行病学资料显示, 我国脑血管病的年发病率为 120~180/10 万, 死亡率为 60~120/10 万, 其致残率可高达 75% 根据年全国疾病监测系统死因监测数据集资料统计显示, 年脑血管病死亡率呈上升趋势, 全国脑血管病死亡率 2010 年比 2006 年上升 % [1], 目前已经和恶性肿瘤一起构成我国居民前 2 位死因 [2] 在脑血管中, 脑梗死 (cerebral infarction) 是最常见的脑血管疾病, 又称缺血性卒中, 占脑血管疾病中的 60%~80%, 主要是由于供应脑部血液的动脉发生闭塞性病变导致局部脑组织区域血液供应障碍, 而未能获得及时充分的侧支循环, 进而导致脑组织缺血缺氧性病变坏死, 产生临床上对应的神经功能缺失表现, 且脑梗死患者具有较高的复发率, 复发后临床症状将加重, 治疗效果较差其较高的死亡率, 致残率和复发率, 使患者的身心健康得到严重伤害, 降低了患者的生活质量, 为患者家庭带来了沉重的经济负担, 同时将对社会造成沉重的医疗经济负担, 造成社会不利影响因而积极的脑梗死一级预防和二级预防, 脑梗死后的积极治疗显得尤为迫切重要脑梗死在祖国传统医学中属于中风病范畴, 中医自两千年前就对中风有所认识, 在不断地临床实践和理论创新中, 逐渐完善了对中医中风病的认识辨证论治是中医诊断和治疗疾病的主要手段之一, 证候是辨证论治的核心, 是指在疾病发生发展过程中所处某一阶段的的病因病位病性以及病势的病理概括, 可以反映疾病的本质, 临床较为常见证名规范的证称为证型, 辨证是治疗的基础现代医家对缺血性中风的辨证分型进行了深入的探索, 但意见不一, 缺血性中风证型的分类繁多, 目前公认的标准有三个 : 一是国家行业标准 [3], 将缺血性中风分为肝阳暴亢风痰阻络痰热腑实气虚血瘀阴虚风动风火上扰清窍痰湿蒙蔽心神痰热内闭心包元气败脱心神散乱 9 个证型, 前五证属于中经络, [4] 后四证属于中脏腑 ; 二是全国脑病专业委员会诊断标准, 将缺血性中风分为风证火热证痰证血瘀证气虚证阴虚阳亢证 6 个证型 ; 三是国家中医药管理局脑病急症协作组 1996 年制定的中风病诊断与疗效评定标准 [5], 分风痰火亢风火上扰痰热腑实风痰阻瘀痰湿蒙神气虚血瘀阴虚风动 7 个证 1

16 急性脑梗死血瘀证与胰岛素抵抗超敏 C 反应蛋白及 NIHSS 评分的相关性研究型而在最新的十二五国家级规划教材中医内科学当中, 将中风病分为中经络中脏腑和恢复期, 中经络分为风痰入络风阳上扰和阴虚风动 3 个证型, 中脏腑分为闭证的痰热腑实痰火瘀闭痰浊瘀闭合脱证的阴竭阳亡等 4 个证型, [6] 恢复期则分为风痰瘀阻气虚络瘀和肝肾亏虚等 3 个证型, 共 10 个证型以目前来说, 缺血性中风尚未找到较好的辨证分型标准, 而脑病专业委员会的诊断标准将证型分为 6 个基本证候要素, 具有较好的可操作性, 基本反应中风病的病因病机, 能将临床所见症状较好全面描述, 而无遗漏或者重复证型之虞, 使用较广泛及容易掌握而受到学者们的重视杨利 [7] 等通过 1418 例中风患者证候调查发现中风急性期以血瘀证痰证风证为最多见, 恢复期则以血瘀证痰证气虚证为主, 后遗症期以血瘀证气虚证和痰证为主每个病期的患者中, 瘀血证所占比例最高, 痰瘀并见者在每个病期表现亦较突出黄燕 [8] 等调查不同地域的缺血性中风急性期证候分布, 显示血瘀证最多占 83.4%, 痰证占 81.3% 另一项大样本 4077 例缺血性中风血瘀证在不同时点 (1-3d,4-10d,11-30d) 分布比率分别为 81.0% 80.2% 77.3%, 明显高于其他证候, 显示血瘀证作为急性脑梗死的最常见最基本证型 [9] 刘璐 [10] 等对 464 例缺血性中风患者研究发现自发病第天, 血瘀证是其基本证候, 自发病第 1 天, 血瘀证患者的神经功能缺损程度较非血瘀证患者重, 且其 NIHSS 分值在疾病过程中持续高于非血瘀证组, 血瘀证是缺血性中风病的重要证候类型, 对神经功能缺损程度具有一定的影响血瘀证为缺血性中风病近期预后的独立危险因素周东梅 [11] 研究瘀证候积分与神经功能缺损积分呈中等程度正相关关系 (R=0.582,P<0.01), 瘀证候积分大者病损程度也大, 瘀证候积分小者病损程度也小, 它们之间存在着某种量化关系从临床治疗情况分析, 何婷婷 [12] 等从 CNKI 数据库检索得到的 2002 年至 2011 年中医药治疗脑梗死急性期 132 篇文献进行用药分析, 结果提示 : 活血药使用频率最高占 31%, 使用最多的是川芎丹参水蛭, 其次是平肝熄风药占 14%, 使用最多的是地龙天麻全蝎, 若将平肝熄风药中的地龙全蝎也归入动物类活血药, 则活血化瘀比例更高以方 ( 药 ) 测证是中医临床思维的重要方法之一, 它强调方证的对应性, 反向地证明方剂功效对证的针对性, 通过对脑梗死急性期方药分析的总结可以得知中风后以血瘀证为最基本证型 2
